  • Effects of heat-inactivated Lactobacillus gasseri TMC0356 on liver lipid metabolism in rats with metabolic syndrome and its possible mechanism

    Objective To explore the effects of heat-inactivated Lactobacillus gasseri TMC0356 on liver lipid metabolism in rats with metabolic syndrome (MS) and its possible mechanism. Methods Sixty male Sprague-Dawley rats were selected. Rats were randomly divided into 5 groups, including control group, MS model group and three TMC0356 test groups (low-, medium- and high-dose groups). The rats in each group were fed with different diets for 7 days, and the liver was dissected and removed after 15 weeks. The mRNA and protein expression levels of peroxisome hyperbioactive receptor-α (PPAR-α), sterol regulatory element binding protein-1c (REBP-1c), fatty acid synthase (FAS) and carnitine lipoacyltransferase-1 (CPT-1) genes in liver were detected. Results There was no significant difference in the mRNA expression of PPAR-α, SREBP-1c or CPT-1 among the five groups (P>0.05). The mRNA expression of FAS in low-dose TMC0356 test group was lower than that in MS model group (P=0.011), medium-dose TMC0356 test group (P=0.042) and high-dose TMC0356 test group (P=0.009). There was no significant difference in the expression of FAS mRNA between other groups (P>0.05). There was no significant difference in the protein expression of PPAR-α, SREBP-1c or FAS among the five groups (P>0.05). The protein expression of CPT-1 in low-dose TMC0356 test group was higher than that in control group (P=0.033) and high-dose TMC0356 test group (P=0.043). There was no significant difference in the protein expression of CPT-1 between the other groups (P>0.05). Conclusion Heat-inactivated Lactobacillus gasseri TMC0356 may improve the symptoms of metabolic disorder in rats by suppressing appetite, improving insulin resistance, and downregulating the expression of key fat metabolism genes such as FAS and SREBP-1c.

  • Procalcitonin-to-albumin ratio as a prognostic marker in acute respiratory distress syndrome patients: a retrospective cohort study

    ObjectiveTo explore the value of procalcitonin-to-albumin (PAR) in patients with acute respiratory distress syndrome (ARDS).MethodsA retrospective study was carried on patients diagnosed with ARDS from December 2016 to March 2018. The receiver-operating characteristics (ROC) curve was used to identify the cutoff value of PAR. The association of PAR and 28-day mortality was evaluated using univariate and multivariable Cox regression.ResultsIn the final analysis, there were a total of 255 patients included. Of whom 164 (64.3%) was male, 91 (35.7%) was female and the mean age was 52.1±14.5 years old. The 28-day mortality of all the patients was 32.9% (n=84). ROC curve revealed that the cutoff value of PAR was 0.039 (specificity: 0.714, sensitivity: 0.702) and area under the curve was 0.793 (95%CI: 0.735 - 0.850, P<0.001). The following variables were considered for multivariable adjustment: age, body mass index, pneumonia, aspiration, sepsis, surgery, PaO2/FiO2, red blood cell counts and PAR (P<0.01 in univariate analysis). After multivariable analysis, only age (HR: 1.033, 95%CI: 1.009 - 1.059, P=0.008), PaO2/FiO2 (HR: 0.992, 95%CI: 0.985 - 1.000, P=0.044) and PAR (HR: 4.899, 95%CI: 2.148 - 11.174, P<0.001) remained independently associated with 28-day mortality (P<0.05).ConclusionHigh PAR predicts a poor outcome in ARDS patients, therefore it appears to be a prognostic biomarker of outcomes in patients with ARDS.

The authors displayed the imaging pictures of one patient with HSOS and made a brief description of typical imaging features, underlying pathophysiological mechanisms, and differential diagnosis of HSOS, with the hope of improving the understanding of HSOS and reducing the rates of leak diagnosis or misdiagnosis.

  • ABSTRACTSEFFECT OF ELECTROPHYSIOLOGICAL EXAMINATION IN THE OPERATION OF CUBITALTUNNEL SYNDROME

    lectrophysiological examination was used in 15 cases of cubital tunnel syndrome before andduring opcration. The velocity, latency and amplitude of the conduction of the ulnar nerve 5cm aboveand below the elbew joint were measured by surface electrodes and direct stimulation. There is nosignificant difference(Pgt; 0.5 )between the results from the two kinds of testing. After the ulnarnerve was decompressed from the cubital tunnel, the conduction velocity increased by 50%, latency shortenee by 40%, the improvement in conduciton velocity being particularly significant(P lt; 0.02). which show that conduction velocity is a relatively sensitive testing parameter. Electrophysiological examination plays a monitoring role during cubital tunnel syndrome decompression.

  • Systemic inflammatory response syndrome after radiofrequency ablation of hepatic hemangioma

    Radiofrequency ablation for hepatic hemangioma is safe and effective, and can obtain the same curative effect as traditional surgical resection. For hepatic hemangiomas with large volume, abundant arterial blood supply and long ablation time, systemic inflammatory response syndrome (SIRS) often occurs after radiofrequency ablation, which can lead to injury or dysfunction of important organs. This paper systematically summarizes the mechanism, prevention and treatment of SIRS after radiofrequency ablation of hepatic hemangioma, so as to provide reference for improving the safety of radiofrequency ablation of hepatic hemangioma.

  • Correlation between Benign Prostatic Hyperplasia and Metabolic Syndrome

    Objective To evaluate the correlation between benign prostatic hyperplasia (BPH) and metabolic syndrome (MS). Methods Total 666 elderly male patients admitted to West China Hospital for routine physical examination in May, 2010 were included in this study. The related laboratory tests of BPH and MS were taken. The correlation among BPH, lower urinary tract Symptoms (LUTS), prostate volume (PV), MS and its component diseases were analyzed. Results Hypertension was an important risk factor for BPH (OR=1.309, 95%CI 1.033 to 1.661), low HDL-C hyperlipidemia was a risk factor for IPSS scored over 7 points (OR=1.573, 95%CI 0.330 to 0.997), and the score of PV was positively correlated to obesity, hypertension, low HDL-C hyperlipidemia and MS (all Plt;0.05). Conclusion For the patient with BPH, MS and its component diseases mainly exert their effects on PV changes rather than LUTS.

  • Analysis of incidence and risk factors of metabolic syndrome after adult liver transplantation recipients

    ObjectiveTo analyze the prevalence and risk factors of metabolic syndrome (MS) after adult liver transplantation (LT) recipients. MethodsThe clinicopathologic data of patients with survival time ≥1 year underwent LT in the People’s Hospital of Zhongshan City from January 1, 2015 to August 31, 2020 were analyzed retrospectively. The logistic regression model was used to analyze the risk factors affecting MS occurrence after LT, and the receiver operating characteristic (ROC) curve was used to evaluate the optimal cutoff value of the index of predicting MS occurrence and its corresponding evaluation effect. ResultsA total of 107 patients who met the inclusion criteria were collected in this study. Based on the diagnostic criteria of MS of Chinese Medical Association Diabetes Association, the occurrence rate of MS after LT was 32.7% (35/107). Multivariate logistic regression analysis showed that the increased age of the recipient [OR (95%CI)=1.106 (1.020, 1.199), P=0.014], preoperative increased body mass index [OR (95%CI)=1.439 (1.106, 1.872), P=0.007] and blood glucose level [OR (95%CI)=1.708 (1.317, 2.213), P<0.001], and with preoperative smoking history [OR (95%CI)=5.814 (1.640, 20.610), P=0.006] and drinking history [OR (95%CI)=5.390 (1.454, 19.984), P=0.012] increased the probability of MS after LT. The areas under the ROC curve (AUC) corresponding to these five indexes were 0.666, 0.669, 0.769, 0.682, and 0.612, respectively. The corresponding optimal cutoff values of three continuous variables (recipient’s age, preoperative body mass index, and blood glucose level) were 53 years old, 23.1 kg/m2, and 6.8 mmol/L, respectively. The AUC of combination of the above five indexes in predicting occurrence of MS was 0.903 [95%CI (0.831, 0.952)], and the sensitivity and specificity were 80.0% and 90.3%, respectively. ConclusionsIncidence of MS after adult LT recipient is not low. For recipients with preoperative hyperglycemia, obese, elderly, histories of drinking and smoking before LT need to pay attention to the early detection and early intervention of MS.

  • Establishment of Novel Premature Piglet Models for Short Bowel Syndrome

    ObjectiveTo explore a new method for establishing premature piglet model of short bowel syndrome. MethodsThirty two premature piglets were randomly divided into 4 groups, including jejunoileal anastomosis group, jejunocolic anastomosis group, sham operation group, and blank control group. Each group enrolled 8 premature piglets. Jejunoileal anastomosis group:resected 75% of small intestinal, reserved the proximal jejunum (10% of the total small intestine length) and distal ileum (15% of the total small intestine length), with jejunoileal anastomosis. Jejunocolic anastomosis group:removed 75% of the small intestine, including the distal jejunum (25% of the total small intestine length), total ileum (50% of total small intestine length), the ileocecal valve, and the colon beyond 5 cm far from the ileocecal valve, retained the proximal jejunum (25% of the total length of the small intestine), with jejunocolic anastomosis. Sham group:cut off the ileum at a distance of nearly 25 cm of the ileocecal valve ileum, then anastomosed the intestine again. Blank control group:no surgery. Data included the first defecation time, duration of diarrhea and parenteral nutrition (PN) after surgery, the length and weight of the small intestine, the weight of the colon, the villus height and crypt depth of both jejunum and ileum at 21st days postoperatively, were collected and analysed. ResultsCompared with the sham group and blank control group, jejunoileal and jejunocolic anastomosis groups showed higher villus height and the crypt depth of jejunum and ileum (P<0.050), provided enough anatomy evidence of intestinal adaptation. However, the duration of PN in jejunocolic group was longer than in jejunoilea anastomosis group (P<0.050). The length and weight of the small intestine and the weight of colon in jejunocolic anastomosis group, were lower than in jejunoileal anastomosis group (P<0.050), improving that the ability of intestinal adaptation of jejunocolic anastomosis was better than the jejunoileal anastomosis group. ConclusionsPremature piglet models of short bowel syndrome could be established by methods of jejunoileal and jejunocolic anastomosis. Both of the methods had their own different degree of intestinal adaptation and growth. The jejunoileal anastomosis is maybe a better way to establish this model.

  • Comparative Ultrasonography with Radionuclide Imaging Evaluating Parotid of Sjogren Syndrome

    摘要:目的:探讨超声与核素显像在评价干燥综合征(SS)腮腺受累情况中的价值和作用。方法:对65例SS病人分别进行超声和核素显像检查。结果:超声判为腮腺功能0级、I级、ⅡⅢ级、Ⅳ级的能力与核素显像判为正常(χ2=0.075,Pgt;0.05)、轻度(χ2=0.12,Pgt;0.05)、中度(χ2=0.27,Pgt;0.05)、重度(χ2=0.097,Pgt;0.05)受损的能力一致,差异无统计学意义;核素发现单纯摄取功能受损的超声多表现为不均匀型、单纯排泌功能受损的超声多表现为结节型、摄取和排泌功能均受损的超声多表现为纤维化型。结论:超声检查可作为SS患者腮腺受累情况评价的良好手段。Abstract: Objective: To quantitatively evaluate ultrasonography and radionuclide imaging in thediagnosis of parotid gland involvement in Sjogren syndrome (SS). Methods: Ultrasonography and radionuclide imaging were conducted on 65 cases with primary Sjogren syndrome. Results: There was no significant difference statistically between the ultrasonographic appearance of the parotid gland and radionuclide imaging (P gt;0. 05). Conclusion: As for diagnosis and evaluating the parotid gland of Sjogren syndrome, ultrasonography may be the useful choice.
