  • Effect of resveratrol on high mobility group box-1 protein signaling pathway in cartilage endplate degeneration caused by inflammation

    Objective To investigate the effect of resveratrol (RES) on inflammation-induced cartilage endplate (CEP) degeneration, and its regulatory mechanism on high mobility group box-1 protein (HMGB1) signaling pathway. Methods The intervertebral CEP cells of Sprague Dawley (SD) rats aged 3 weeks were extracted and identified by toluidine blue staining and immunofluorescence staining of rabbit anti-rat collagen type Ⅱ. The cell counting kit 8 (CCK-8) method was used to screen the optimal concentration of RES on intervertebral CEP cells. Gene chip analysis was used to determine the target of RES on intervertebral CEP cells. Interleukin 1β (IL-1β) was used to construct the intervertebral CEP cell degeneration model caused by inflammation and the 7-8-week-old SD rat intervertebral disc degeneration model, and pcDNA3.1-HMGB1 (pcDNA3.1) was used as the control of RES effect. Flow cytometry and TUNEL staining were used to detect the apoptotic rate of intervertebral CEP cells and rat intervertebral disc tissue cells, respectively. ELISA kit was used to detect the content of interleukin 10 (IL-10) and tumor necrosis factor α (TNF-α) in the cell supernatant and rat serum. Western blot was used to detect the expressions of HMGB1, extracellular signal-regulated protein kinase (ERK), phosphorylated ERK (p-ERK), B cell lymphoma/leukemia 2 gene (Bcl-2), and Bcl-2-associated X protein (Bax). ResultsThe extracted cells were identified as rat intervertebral CEP cells. CCK-8 method screened out the highest activity of intervertebral CEP cells treated with 30 μmol/L RES. The gene chip analysis confirmed that the HMGB1-ERK signal was the target of RES. Both cell experiments and animal experiments showed that RES treatment can significantly down-regulate the apoptosis rate of intervertebral CEP cells, inhibit the release of TNF-α, and increase the content of IL-10; and down-regulate the expressions of HMGB1, p-ERK, and Bax, and increase Bcl-2; and pcDNA3.1 could partially reverse these effects of RES, and the differences were all significant (P<0.05). ConclusionRES can significantly inhibit the apoptosis of intervertebral CEP cells induced by inflammation, which is related to inhibiting the expression of HMGB1.

  • Research progress on significance of ABO blood group in colorectal cancer

    ObjectiveTo comprehend the role of ABO blood groups antigens in the occurrence, development, screening, treatment, and prognosis of colorectal cancer (CRC). MethodThe literature on the researches relevant to relation between ABO blood groups and CRC in recent years was reviewed and analyzed. ResultsThere were two possible mechanisms relevant to the relation between the ABO blood groups antigens and the occurrence of CRC—Nucleotide polymorphisms in regulatory genes of the ABO blood groups antigens and lack of ABO blood groups antigens expression, and its abnormal expression in the cancer cells provided the clues for the screening of CRC. At present, it was found that the ABO blood groups were associated with the treatment and prognosis of the patients with CRC in the relatively fewer researches, but the detailed mechanism did not be clarified. ConclusionsFrom the summary of the literature results, researchers have studied the role of ABO blood groups in CRC, and have obtained some conclusions with clinical significance in the occurrence, development, screening, treatment, and prognosis of patients with CRC, suggesting that it has certain research prospects. However, relevant to research is less, the conclusions need to be further verified.

  • Effect on Expression of High Mobility Group Box-1 mRNA for Expressions of Zonula Occludens Protein-1 mRNA and Protein in Rat with Acute Necrotizing Pancreatitis

    Objective To study the effect on expression of high mobility group box-1 (HMGB1) mRNA for the expression of zonula occludens-1 (ZO-1) in ileum tissues, and to explore the possible mechanism of intestinal mucosal barrier injury in rats with acute necrotizing pancreatitis (ANP). Methods Ninety-six male Wistar rats were divided randomly (random number method) into ANP group, ethyl pyruvate (EP)group, and sham operation group. Eight rats of 3 groups were killed to get abdominal aortic blood and ileal tissues at 6, 12, 24, and 48h after operation, respectively.The levels of plasma amylase (AMY) , D-lactate acid, and the activity of malonyl dialdehyde (MDA) in the ileum tissues were determined by using automatic biochemical analyzer, improved enzymatic spectrophotometry, and thiobarbituric acid (TAB) colorimetry respectively. The pathological changes of ileum tissues were observed under microscopy by HE staining, the expression of ZO-1 protein in ileum tissues was observed by immunohistochemistry (SP method), and the expressions of HMGB1 mRNA and ZO-1 mRNA in ileum tissues were detected by reverse transcription-polymerase chain reaction (RT-PCR). Results Compared with ANP group at the same time, levels of AMY, D-lactate acid, and MDA in ileum tissues of EP group were all significantly lower (P<0.05). The expression level of HMGB1 mRNA increased at 6 h while ZO-1 mRNA decreased in ANP group. Compared with ANP group at the same time, the expression level of HMGB1 mRNA of EP group was significantly lower while ZO-1 mRNA was higher (P<0.05), and the pathological damage in ileum tissues was lighter. Conclusions The decreased expression of ZO-1 in ileum tissues is one of the vitalcauses for intestinal mucosal barrier injury in ANP, and it probably occurs in case of the excessive expression of HMGB1.

  • Adoption of PBL in Medical Education: Experience at Chung Shan Medical University

    The main approaches to medical education reform that have been introduced elsewhere in the world, including the integration of basic and clinical sciences into organ-based model, the establishment of a Center for Faculty Development and problem-based learning (PBL) have been introduced to the College of Medicine, Chung Shan Medical University (CSMU) since the mid 1990s. The process of developing the PBL approach can be divided into the following stages: Observation (1994-2000): In this stage, CSMU observed the practice and effectiveness of PBL programs in other domestic and foreign medical schools. At the same time, we assessed the possibility of introducing PBL to Chung Shan. When the authority decided to introduce PBL, a PBL committee was inaugurated. Preparation (2000-2002): In this stage, tutor training took place, as well as PBL case writing workshops to reach consensus among the faculty. To reduce the total amount of curricula and traditional teaching hours, we combined and simplified related curricula, so that one and half weekdays were reserved for PBL tutorials and students’ self-study. A preliminary course about how to learn well, including the philosophy and methodology of learning as well as evidence-based medicine (EBM), was integrated into premedical curricula. Practice (2002-2004): In light of the number of trained tutors and written cases that have been prepared, and the desire that the PBL would be successful from the start, CSMU chose to introduce PBL to the clinical curricula first in a hybrid curriculum design. This meant that the traditional teaching was retained at the beginning. Evaluation of PBL by the students was done at the end of each semester. Tutor meetings were held twice per semester. Advancement (2004-2006): In this stage, PBL was introduced to the basic medical curricula as well as to the premedical general curricula. Based on our experience and comprehensive evaluation, a PBL guidebook for tutors and students has been published and delivered to both groups. We also developed an on-line evaluation system containing the evaluation forms for students and tutors. Excellence (2006 onwards): In this stage, the PBL website and learning resources will be further developed, and we are designing the system for on-line PBL practice.In conclusion, the introduction to PBL for medical education is reasonable and feasible. It requires b administrative support, a long-standing and high commitment of the PBL committee, consensus among the faculty and an appropriate planning and evaluation mechanism.

  • Application of the subpopulation treatment effect pattern plot (STEPP) method in clinical trials

    Subpopulation treatment effect pattern plot (STEPP) method is a method for examining the relationship between treatment effects and continuous covariates and is characterized by dividing the study population into multiple overlapping subpopulations to be analyzed based on continuous covariate values. STEPP method has a different purpose than traditional subgroup analyses, and STEPP has a clear advantage in exploring the relationship between treatment effects and continuous covariates. In this study, the concepts, advantages, and subpopulation delineation methods of the STEPP method are introduced, and the specific operation process and result interpretation methods of STEPP method analysis using the STEPP package in R language are presented with examples.

  • CHONDROGENESIS OF BONE MARROW MESENCHYMAL STEM CELLS INDUCED BY TRANSFORMING GROWTH FACTOR β3 GENE IN DIANNAN SMALL-EAR PIGS

    ObjectiveTo observe transforming growth factor β3 (TGF-β3) gene expression and the chondrogenesis of bone marrow mesenchymal stem cells (BMSCs) after TGF-β3 gene is transfected into BMSCs of Diannan small-ear pig. MethodsRecombinant adenovirus 5 (rAd5) was extracted as gene vector and packed into recombinant adenovirus rAd5-TGF-β3, double enzyme digestion and PCR identification were performed. BMSCs were isolated and cultured from bone marrow of 2-month-old Diannan small-ear pigs (weighing, 12-15 kg), and the 2nd generation of BMSCs were harvested for experiments. The experiments were divided into 3 groups. BMSCs were transfected with rAd5-TGF-β3 as experimental group and with empty vector as control group, and non-transfected BMSCs were used as blank control group. The transfection efficiency of exogenous gene was identified by flow cytometry, TGF-β3 protein expression by immunofluorescence and Western blot. The cell morphology of experimental group was observed by inverted phase contrast microscope, and the expression of collagen type II in each group was detected by Western blot. ResultsThe rAd5-TGF-β3 recombinant adenovirus was successfully constructed and transfected into BMSCs. Green fluorescence was observed by immunofluorescence microscope. Flow cytometry test showed the best transfection at 72 hours (transfection efficiency of 84.86%). Immunofluorescence staining showed that the expression of TGF-β3 protein was obvious at 72 hours; Western blot showed that there was a TGF-β3 positive band with a relative molecular mass of 30×103, while the control group and blank control group had no positive band. Obvious chondrogenic differentiation was observed in the experimental group after transfection in vitro, while the control group and blank control group had no obvious chondrogenic differentiation. Western blot showed that there was collagen type II positive band with a relative molecular mass of 130×103 at 21 days after culture, while the control group and blank control group had no positive band. ConclusionrAd5-TGF-β3 gene can be successfully transfected into BMSCs via adenovirus vectors, and stable expression of TGF-β3 protein can be observed, enhancing BMSCs differentiation into chondrocytes, which may provide an experimental basis for gene therapy of joint cartilage defects.

  • Application of an R-group Search Strategy into Three-dimensional Quantitative Structure-activity Relationship of HEA β-secretase Inhibitors and Molecular Virtual Screening

    The β-secretase is one of prospective targets against Alzheimer's disease (AD). A three-dimensional quantitative structure-activity relationship (3D-QSAR) model of Hydroethylamines (HEAs) as β-secretase inhibitors was established using Topomer CoMFA. The multiple correlation coefficient of fitting, cross validation and external validation were r2=0.928, qloo2=0.605 and rpred2=0.626, respectively. The 3D-QSAR model was used to search R groups from ZINC database as the source of structural fragments. As a result, a series of R groups with relatively high activity contribution was obtained to design a total of 15 new compounds, with higher activity than that of the template molecule. The molecular docking was employed to study the interaction mode between the new compounds as ligands and β-secretase as receptors, displaying that hydrogen bond and hydrophobicity played important roles in the binding affinity between the new compounds and β-secretase. The results showed that Topomer CoMFA and Topomer Search could be effectively used to screen and design new molecules of HEAs as β-secretase inhibitors, and the designed compounds could provide new candidates for drug design targeting AD.

  • Primary Study on Metastatic Rate and Metastasis of Subpyloric Lymph Nodes in Gastric Cancer

    ObjectiveTo explore the value on excision of subpyloric (No.6 group) lymph nodes of stomach by detection of metastatic rate and metastasis of lymph nodes of No.6 group and its subgroups. MethodsThe clinical data including complete information on No.6 group and its subgroups lymph nodes in 80 patients underwent gastrectomy and subpyloric lymph nodes dissection for gastric cancer from January 2006 to December 2009 were retrospectively analyzed. Referring to the right gastroepiploic vein, the No.6 lymph nodes were divided into three subgroups (No.6a, No.6b, and No.6c subgroup), and the relationship between the metastasis of No.6 lymph nodes and clinicopathologic features as well as the metastasis of No.7, No.8a, and No.9 lymph nodes were analyzed by logistic regression analysis. ResultsThe metastatic rate of No.6 group lymph nodes was 41.3% (33/80) and with 26.0% (108/415) of the resected lymph nodes involved. The metastatic rate of lymph nodes in No.6a subgroup (7.5%, 6/80) was significantly lower than that in No.6b (16.3%, 13/80) and No.6c subgroup (36.3%, 29/80), Plt;0.001. The metastasis of the resected lymph nodes in No.6a, No.6b, and No.6c subgroup was 25.0% (8/32), 17.6% (13/74), and 28.2% (87/309), respectively, and the difference was not significant (P=0.292). The metastasis of lymph nodes in No.6a subgroup was correlated to T stage (P=0.042) and N stage (P=0.006). The metastasis of lymph nodes in No.6b subgroup was correlated to N stage (P=0.002) and TNM stage (P=0.013). The metastasis of lymph nodes in No.6c subgroup was correlated to differentiation degree of tumor (P=0.008), T stage (P=0.003), N stage (P=0.000), and TNM stage (P=0.000). The logistic regression analysis showed that the metastasis of lymph nodes was correlated to the metastasis of No.8a lymph nodes (P=0.023) and N stage (P=0.002) in No.6 group, the metastasis of No.8a lymph nodes (P=0.018) in No.6a subgroup, N stage (P=0.005) in No.6b subgroup, and the metastasis of No.8a lymph nodes (P=0.016) and N stage (P=0.004) in No.6c subgroup. ConclusionAttentions should be paid to the complete dissection of subpyloric lymph nodes in gastric cancer surgery, especially for the lymph nodes of No.6a and No.6b subgroups.

We should put forward hospital improvement strategies from the top-level design of the whole hospital and from the aspects of improving the quality of the first page of the cases and the quality of the coding, strengthening the cost accounting and control of the disease, carrying out in-hospital and out-of-hospital training, establishing a liaison model, finding gaps with benchmark hospitals, enhancing the core competitiveness of innovative technologies, and improving internal performance appraisal, etc., to promote the high-quality development of hospitals.

  • Gastroscope Observation of 3661 Cases about Special Diet in Ethnic Minority Areas

    摘要:目的: 研究分析特殊饮食结构和生活习惯人群发生上消化道疾病的病种及发生率。 方法 :对1998年11月至2008年11月我院经胃镜检查的3661例患者进行统计,并参考饮食结构和生活习惯进行分析。 结果 :汉族调查2343例,彝族调查1318例:汉族患病率胃溃疡1626%、十二指肠溃疡858%、复合性溃疡098%、糜烂出血性胃炎1434%、胃癌、食道癌089%;彝族患病率胃溃疡2527%、十二指肠溃疡1282%、复合性溃疡25%、糜烂出血性胃炎1988%、胃癌、食道癌152%。 结论 :少数民族地区特殊饮食人群上消化道疾病发病率及胃、十二指肠溃疡的发生率和癌变比率明显增高。Abstract: Objective: To study the structure of the special diet and the lifestyle of people who occurred the upper digestive tract diseases and disease incidence. Methods : From 199811 t0 200811,3661 cases in our hospital carryed out statistics and analysis with reference to diet and lifestyle. Results : Han people with 2343 cases, Yi people with 1318 cases, in Han people, the rate of ulcer was 1626%,858% of duodenal ulcer,098% of compound ulcer,1434% of erosive hemorrhage gastritis,089% of stomach and esophagus; in Yi people,2527% of gastric ulcer,1282% of duodenal ulcer,25% of compound ulcer,1988% of erosive gastritis hemorrhage,152% of stomach and esophageal cancer. Conclusion : The upper digestive tract diseases, the incidence of stomach, duodenal ulcer and cancer incidence rate increased significantly in special diet groups in ethnic minority areas.
