    Objective To observe whether the animal model of optic nerve injury in rats can be set up by fluid percussion brain injury device (FPI) or not.Methods Seventyone healthy female Wister rats were randomly divided into 2 groups, inlcuding model group with 66 rats and control group with 5 rats.The rats in model group were randomly divided into 3 groups. Eight rats in group 1 were examined by flashvisual evoked potential (F-VEP) and magnetic resonance imaging (MRI) examines before and 1, 3 days,1,2,4,6,and 8 weeks after injury; 56 rats in group 2 were randomly divided into 7 subgroups with 8 rats in each subgroup,and were detected by histopathological and terminal deoxynucleotidyl transferasemediated dUTP nick end labeling (TUNEL) apoptosis examines 1, 3 days, 1,2,4,6,8 weeks after injury;2 rats in group 3 were examined by electron microscopy 4 and 8 weeks after injury.According to the degree of injury, the injured eyes were divided into 2 groups including severe injury group with the beat pressure of (699.14plusmn;60.79) kPa and mild injury group with the beat pressure of (243.18plusmn;20.26) kPa.The right and left eyes in rats in each group were in severe and mild injury group, respectively.Results One day after injury, the latency duration of FVEP prolonged in severe injury group,wich differed much form which in the normal control group (P<0.05);the amplitude was gradually reduced during the first 2 weeks after injury and kept steady after that (P>0.05). The latency duration prolonged in mild injury group,and its difference with the normal control group was statistically significant (P<0.05);the amplitude was gradually reduced during the first 4 weeks after injury and kept steady after that (P>0.05). The abnormal high signal could be seen on optic nerve 1 day after injury, and was still obvious 8 weeks later. The results of histopathological examination showed ruptured capillary in ganglion cell layer 1 day after injury;retinal ganglion cells without nucleus could be seen 4 weeks after injury. The apoptosis of positive cells was found in each layer of the retina 3 days after injury.TUNEL results indicated that the number of apoptotic positive cells increased significantly 1-2 weeks after injury.Conclusion An animal model of optic nerve injury can be successfully set up using FPI in rats.

  • Studies on the changes of ultrastructure and function of blood-retinal barrer of retinal pigment epithelium in spontaneously hypertensive rat

    Objective To investingate the ultrastructural changes of retinal pigment epithelium(RPE) and its permeability in spontaneously hypertensive rats(SHR)and explore the relation between these changes and hypertensive retinopathy.MethodsThe ultrastructure of RPE cells in the SHR aged five,six,seven months wasobserved with transmission electronmicroscope and compared to its normotensive control strain(WKY) with the same age.Then,lanthanum tracer procedures were carried out to investigate pathological changes of the blood-retinal barrier.Results (1)In SHR the main pathological changes involved swelling of mitochondria,enlargement of endoplasmic reticula,decrease of RPE cell infolding,and sparseness of microvilli.These degenerations were more serious in older rats with higher blood pressure.(2)The breakdown of outer blood-retinal barrier with permeation of lanthanum tracers were evident in SHR aged six or seven month,however,in WKY and five-month SHR the traces were prevented from passing by tight junctions.ConclusionThe degeneration of RPE owing to ischemia and anoxia arises in early periosd of hypertensive retinopathy.The pathological changes of ultrastructure and permeability might interact with the damage of visual cells and play a main role in the hypertensive retinopathy.

  • Clinical features and treatment of uveitis combined with cystoid macular edema

    Objective To explore the frequency, clinical features, and characteristics of results of fundus fluorescein angiography (FFA) of uveitis related cystoid macular edema (CME). Methods The clinical data and FFA results of 67 patients (106 eyes) with posterior uveitis examined in our hospital from July 2002 to June 2005 were collected. The clinical features and characteristics of FFA images of CME were observed and analyzed. Results Among the106 eyes of 67 patients with uveitis,the CME was observed in 28 eyes (26.4%) of 18 patients, including 7 males and 11 females with the average age of (42.5plusmn;10.8) years. The dark area due to the choroidal fluorescence blocked by the macular edema was found at the early FFA phase, and th en followed by the punctate and sheetlike leakage of fluorescein; the capillar y was dilated at the venous phase, and the typical petaloid appearance was seen at the late phase because the fluorescein cumulated in several small vesicles in the macular area. After treated by corticosteroids and topical non-steroidal anti-inflammatory medicine and carbonic anhydrase inhibitors, the extent of CME diminished, and the visual acuity improved in varying degrees. Conclusi ons Uveitis may seriously harm the visual function, in which CME induces the damage of visual acuity. Early detection and timely treatment may prevent thepermanent visual damage. (Chin J Ocul Fundus Dis, 2006, 22: 394-396)

  • Hotspots and problems of basic research for diabetic retinopathy

    Complications of proliferative diabetic retinopathy have become the major indications of vitrectomy. The surgery, however, is not basically a causative therapy. The visual function after operation depends on the degree of retinal ischemia and damage induced. The surgery itself has a potential for severe complications. Therefore it is important to better understand the pathology and to master surgical strategy and techniques in order to improve surgical outcomes and reduce the surgical complications.   • Retinopathy in systemic-lupus-erythematosus nephritis

    Objective To investigate the relationship of clinical indexes between retinopathy in systemic-lupus-erythematosus (SLE)nephritis and SLE,and to discuss its clinical significance. Method The clinical data of 43 cases of SLE nephritis with ocular fundus diseases were retrospectively analyzed.The relationships between retinopathy and kidney defect,general lesions,SLEDAI grade,antinuclear antibody (ANA),anti-double stranded DNA,complement 3 and antiphospholipid antibodies were analyzed with Logistic regression respectively. Results In 86 eyes of the43paitents,there were retinal cotton wool spots in51eyes(59.3%),edema of optic disk in 43 eyes(50.5%),retinal haemorrhage in 12 eyes(14.0%),retinal artery occlusion in 5 eyes(5.8%),central retinal vein occlusion in 2 eyes(2.3%),retinal detachment in 3 eyes(3.5%),optic atrophy in 2 eyes(2.3%),and neovascularization in 2 eyes(2.3%).Logistic regression analysis revealed that SLEDAI grade had linear dependent relation with cotton wool spots and optic disk edema(chi;2=42.154,6.498;P<0.001),and didnrsquo;t have any correlation with proteinuria,hematuria and kidney function.Linear relation between retinal vascular occlusion and antiphospholipid antibodies was found(chi;2=24.475,P<0.001).Retinal haemorrhages,retinal detachment,optic atrophy and neovascularization did not correlate with clinical features. Conclusion SLEDAI grade had linear dependent relation with cotton wool spots and optic disk edema in patients with SLE nephritis,and Linear relation between retinal vascular occlusion and antiphospholipid antibodies is ascertained. Ocular fundus diseases are clinically important for evaluating the therapeutic responses and prognosis in lupus nephritis. (Chin J Ocul Fundus Dis, 2006, 22: 239-241)

  • Clinical analysis of nine patients with presumed tubercular retinal vasculitis

    Objective To observe the clinical features and treatment outcomes of presumed tubercular retinal vasculitis. Methods This is a retrospective non-comparative interventional clinical research. A total of nine patients (11 eyes) with major presentation of retinal vasculitis were included in this study. Patients first consulted the eye clinic and were diagnosed presumed tubercular retinal vasculitis. The patients, seven males and two females, aged from 19 to 66 years, with an average of 43.89 years. The time interval from symptoms to diagnosis ranged from two weeks to six months with an average of 76.27 days. Visual acuity, slit lamp ophthalmoscopy, fundus fluorescein angiography (FFA), optical coherence tomography (OCT), hematological and tuberculosis related investigations were examined and analyzed. All patients had standard anti-tuberculosis treatment. Treatment outcomes were followed for six to 37 months with an average of 14.11 months. Results Baseline visual acuity ranged from hand movement to 0.8 with an average of 0.28. Among 11 eyes, six presented mild to moderate vitritis, five presented as retinal vein occlusion with no obvious vitirits. Fundus examination showed six cases with retinal hemorrhage, four cases with macular edema, two with macular epiretinal membrane, and two with vitreous hemorrhage. FFA revealed 11 cases with leakage of vessels, 11 with nonperfusion area, four with macular edema, three with retinal neovascularization, and two with choroidal lesions. OCT of nine eyes suggested six eyes with retinal edema, three with macular edema, three with macular epiretinal membrane. TST of seven patients were all b positive. T-SPOT.TB of four patients were all positive. Three of eight patients who had chest X-ray or chest CT were suggested tuberculosis infection. Four to six weeks after the start of anti-tuberculosis treatment, vitritis, exudates, retinal and macular edema subsided. During follow up, inflammation was stable with no recurrence observed. The visual acuity of last follow-up ranged from 0.15 to 0.8 with an average of 0.51. Conclusions The main presentations of presumed tubercular retinal vasculitis are vitritis, retinal vein occlusion, and retinal hemorrhage. Standard anti-tuberculosis treatment can improve inflammation and retinal hemorrhage.

  • Relationship between endothelin and nitric Oxide in plasma with retinopathy in the pregnancy-induced hypertension

    0bjective To explore the effect of endothelin(ET)、nitrioxide (NO) in plasma on retinopathv in the pregnancy-induced hypertension(PIH). Methods The 1evel of ET and NO in plasma of 75 cases of in-patient women with PIH and 20 cases of women with the full terms and normal pregnancy before and after delivery was determined by radioimmunoassay.The retinopathy of the patients with FIH before and after delivery was detected by appointed doctor.The levels of ET and N0 in both groups were compared and the relationship between ET and N0 in plasma and the retinopathy before and after the delivery was detected.Results The levels of ET[(145.oo±54.41)ng/L] in serious PIH patients were much higher than that in the control[(81.50±43.80)ng/L],the minor[(85.30±33.33)ng/L]and middling PIH group[(90.20±39.25)ng/L].The levels of ET in plasma before and after pregnancy were not changed in PIH patients [(118.70±33.44)ng/L],but were higher than that in the control group. The levels of plasma NO in serious[(87.56±35.58)ng/L]and middling[(78.11±28.96)ng/L] PIH group were both higher than that in the control group[(46.70±32.64)ng/L],and the levels in minor(52.56±28.35)ng/L]and middling PIH group were lower than that in the serious PIH group.The level of N0 in plasma of PIH patients after the delivery was much lower than that before the delivery,while higher than that in the control.The positive correlation between levels of ET and NO and retinopathy was found in PIH patients.Conclusions The 1evels of plasma ET and N0 in PIH patients are related to the extent of the disease,and the level of ET in plasma is highly related to the retinopathy in PIH patients, ET and NO might be played an important role in pathogenesis of retinopathy and ET might be a good index in reflecting the rank of retinopathy in PIH.(Chin J Ocul Fundus Dis,2004,20:12-15)

  • Clinical obsrevation on early postoperative intraocular hypotension and ciliary deatchment after vitrectomy

    Objective To observe the incidence of ciliary detachment and its relationship with intraocular hypotension soon after vitrectomy. Methods A total of 46 eyes of 46 patients who underwent vitrectomy were examined by ultrasound biomicroscope (UBM). The patients were divided into three groups according to different tamponade: gas group (11 eyes), silicone oil group (8 eyes) and balanced saline solution (BSS) group (27 eyes). Basing on the postoperative intraocular pressure (IOP), the patients were divided into two groups: IOPlt; 10 mm Hg (25 eyes) and IOPge;10 mm Hg (21 eyes). UBM was applied to determine the tomographic features of the ciliary body 3 days after the surgery. IOP were monitored by noncontact tonometer daily after the surgery. The eyes with ciliary detachment were then examined once a week till the ciliary detachment reattached. The followup period was from 14 to 35 days. Results After vitrectomy, ciliary detachment was observed in 20 eyes of 46 eyes (43.5%), The incidence of ciliary detachment was 27.3% in gas group, 25.0% in silicone oil group, and 55.6%in BSS group. The average IOP in eyes with ciliary detachment [(6.47plusmn;4.49) mm Hg (1 mm Hg=0.133 kPa)] was significantly lower than that in the eyes without ciliary detachmen [(15.61plusmn;7.72) mm Hg] (t=8.031,Plt;0.001). The incidence of ciliary detachment was higher in eyes with postoperative IOP lt;10 mm Hg (68.0%) than that in the eyes with postoperative IOP ge;10 mm Hg (14.3%) (chi;2=15.60, Plt;0.001). All eyes with postoperative ciliary detachment got spontaneous reattachment within 30 days after the surgery except that one eye had optic disc edema due to severe intraocular hypotension. Conclusions Early postoperative ciliary detachment is a common complication after vitrectomy. Eyes filled with BSS have the highest incidence of postoperative ciliary detachment. Most of the patients with ciliary detachment may get spontaneous reattahment within 30 days after the surgery.

  • THE EFFECTS OF SILICONE OIL REMOVAL ON THE COMPLICATIONS AFTER RETINAL DETACHMENT SURGERY

    PURPOSE:To assess the effects of silicone oil removal on the complications associated with its use. METHODS:Retrospective analysis of the results of silicone oil removal after vitreous surgery for retinal detachment in 913 eyes in National Ophthalmology Centre of French. The follow-up period was at least six months and the mean duration of oil tamon- ade was 6.4 months. RESULTS :Retinal detachment recurred in 7.3%. The development of cataract continued after silicone oil removal. No patient retained a clear lens in the eye with oil remained in situ for more than 3 months. TWO of 5 eyes with keratopathy at the time of oil removal recovered after the oil removal ;but the corneas of another 4 eyes became dystrophic after the oil removal. Twelve of 15 eyes with secondary ocular hypertension incontrollable through medicinal treatment relieved after oil removal ,but another 11 eyes became hypertensive after oil removal. Persistent emulsive droplets wre found in anterior chamber angles of the above 11 hypertensive eyes and the 7 eyes with keratopathy. CONCLUSINOS:Early removal of silicone oil after retinal detachment operation might delay the development of cataract but can not avoid its occurence ,and ocular hypeitension and keratopathy can be prevented by early and complete removal of silicone oil.   • Pars plana vitrectomy for myopic traction maculopathy

    Objective To observe the therapeutic effect of pars plana vitrectomy (PPV) on myopic traction maculopathy (MTM).Methods The clinical data of 31 eyes of 29 patients with MTM diagnosed by timedomain optical coherence tomography (TDOCT) and slitlamp ophthalmoscopy were retrospectively analyzed. The cases were divided into 2 groups according to the stage of MTM: 12 eyes of 10 patients at the early stage of MTM were in group 1; 19 eyes of patients at the most Advanced stage of MTM were in group 2. All of these eyes had undergone PPV with 10%15% inert gas filling. The patients were followed postoperatively for 6 to 12 months with the average of 8 months, and the best corrected visual acuity, reattachment of macular and retina was examined. Results The improvement rate of visual acuity after surgery for 6 months was 100% in group 1, and 63.2% in group 2 had (12/19); the visual acuity in group 1 was apparently better than that in group 2 (Z=-5477, P=0000). The macular hole disappeared without exposure of the pigment epithelium in all eyes of Group 1, but only 3 eyes in Group 2. For Group 2 patients, 3 eyes had reattached retina with macular holes, and 3 eyes had detached retina with macular holes. The recovery of macular configuration in Group 1 was obviously better than that in Group 2 (Z=-4318, P=0000). Conclusion The surgical intervention of MTM before the formation of macular hole and retinal detachment may prevent the formation of macular holes.
