  • Vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system under topical anesthesia

    Objective To evaluate the indications, effectiveness and complications of vitreoretinal surgery using the 25G transconjunctival sutureless vitrectomy system (TSV25G) under the topical anesthesia. Methods The clinical and follow-up data of 22 eyes of 22 patients undergone vitreo-retinal surgery using TSV25G under the topical anesthesia were retrospectively analyzed. All of the patients were monocular sickened, including idiopathic macular hole in 10 eyes, idiopathic macular pucker in 6, vitreoretinal traction syndrome in 4, and vitreous hemorrhage associated with branch retinal vein occlusion in 2. Peeling of epiretinal membrane and/or internal limiting membrane, intra ocular laser coagulation, air-fluid exchange and tamponiding of C3F8 were performed according to the condition of diseases. The postoperative follow-up was 1-11 months, with the mean duration of 6.4 months. The effect of analgesia, cooperation with the patients, operative effect and complications in and after the surgery were observed. Results The operations finished successfully in all of the eyes under the topical anesthesia. The operation duration ranged from 20 to 25 minutes with average of 22 minutes. The patients cooperated with the doctor well without any discomfort. Two days after the surgery, edema of the wounded conjunctiva was found, and recovered 7 days later. A light pigment dot on the surface of the sclera could be seen at the first month. The complic ations included transient increasing of intraocular pressure in 2 eyes, feather-like opacity of lens in 5 eyes, vitreous hemorrhage in 1 eye, and air-bleb under conjunctiva in 2 eyes. No other complications related with the cut were fo und. The macular hole closed in 9 eyes with idiopathic macular hole, and the other 1 had the smaller but not closed hole. Idiopathic macular pucker, vitreoretinal traction syndrome, and vitreous hemorrhage associated with branch retinal vein occlusion were cured successfully. Conclusions Vitreoretinal surgery using the TSV25G under the topical anesthesia has many advantages such as simple procedure, short operation time, micro-invasion, less complications and rapid revovery, and mainly serves simple manipulation in some simple diseases such as idiopathic macular hole, vitreo-retinal traction syndrome, and simple hemorrhage.   • Level and clinical significance of vitreous microparticles in proliferative diabetic retinopathy

    ObjectiveTo observe the level of microparticles in the vitreous of patients with proliferative diabetic retinopathy (PDR), and preliminarily explore the role of microparticles in the pathogenesis of PDR.MethodsA case control study. From January to December 2018, 54 cases of 54 eyes of PDR patients (PDR group) and 20 cases of non-diabetic retinopathy patients (control group), who were diagnosed and treated with vitrectomy (PPV) in the Department of Ophthalmology, Tianjin Medical University General Hospital vitreous samples were included in the study. Among 54 eyes in the PDR group, there were 42, 21, and 17 eyes with vitreous hemorrhage (VH), traction retinal detachment (TRD), and previous intravitreal injection of drugs, respectively. Among the 20 eyes of the control group, idiopathic macular hole, idiopathic anterior macular membrane, vitreous macular traction syndrome, and complete lens dislocation were 6, 6, 2, and 6 eyes, respectively. The PDR group was divided into uncombined TRD group and combined TRD group according to PDR stage and whether TRD occurred, with 33 and 21 eyes, respectively. According to the presence or absence of VH, they were divided into groups with VH and without VH, with 42 eyes and 12 eyes, respectively. According to whether anti-vascular endothelial growth factor (VEGF) drugs were injected into the intravitreal cavity 3 days before PPV, they were divided into anti-VEGF drug group and no anti-VEGF drug group, with 17 eyes and 37 eyes respectively. The levels of retinal photoreceptor cells (RMP), platelets (PMP), endothelial cells (EMP) and phosphatidylserine (PS-MP) expressing on the membrane surface in the sample were detected by flow cytometry. The comparison between the two groups of samples was performed by t test, and the comparison between multiple groups of samples was performed by one-way analysis of variance or Mann-Whitney test.ResultsCompared with the control group, the vitreous RMP level of the PDR group was significantly decreased, and the EMP and PMP levels were significantly increased. The differences were statistically significant (t=−2.361, 5.064, 3.531; P=0.018, <0.001, 0.001). There was no statistically significant difference in PS-MP levels between the two groups (t=−1.617, P=0.110). Compared with the TRD group, the levels of RMP and PMP in the vitreous of the TRD group were significantly increased, and the difference was statistically significant (t=−2.221, −2.098; P=0.031, 0.041). The level of EMP in the vitreous body of the anti-VEGF drug group was significantly lower than that of the non-anti-VEGF drug group, however, it was still higher than the control group. The difference was statistically significant (Z=−2.430, −2.499; P=0.015, 0.012). The level of PMP in the vitreous body of the eye without VH was significantly higher than that in the group with VH, and the difference was statistically significant (t=−3.097, P=0.003).ConclusionsThe elevated levels of EMP and PMP in the vitreous of PDR patients may be related to the damage of retinal capillaries; intravitreal injection of anti-VEGF drugs before surgery can reduce the level of EMP. VH may be related to the procoagulant effect of PMP.

  • Experimental research of intraocular retention of domestic perfluorocarbon liquid

    Objective To investigate the effect of long-term intraocular retention of domestic perfluorocarbon liquid (PFCL) on morphology and histology of ocular tissues. Methods A total of 18 New-Zealand rabbits were randomly divided into 3 experimental groups, whose left eyes underwent intraocular injection with 0.3, 0.6, and 1.0 ml PFCL, respectively. All of the right eyes of the rabbits were in the control group. The morphological, electrophysiological and histological changes of the ocular tissue were observed 4, 8, and 12 weeks after the injection. Results No clinically significant retinopathy but only mild morphological changes were found in group 1 and 2, while obvious morphological and histological changes were found in group 3. Mild morphological and histological changes were found in all of the rabbits 4-8 weeks after the injection while significant ones were found 8-12 weeks after the injection. The results of electroretinography indicated a statistically significant decline of amplitude of b wave in group 3. Conclusions Long-term intraocular retention of few PFCL may cause mild histological changes but not affect the clinical function. Plentiful PFCL remains in eyes may lead to toxic reaction to the ocular tissue. (Chin J Ocul Fundus Dis, 2006, 22: 128-130)

  • Know the vitreous metabolic function and intervene it cautiously

    Vitreous is composed of transparent gel-like tissue with complex molecular structure, accounting for 80% of the volume of the eyeball. In addition to support the eye structure, vitreous has important functions such as regulating eye development, refraction and cellular barriers, participating in intraocular oxygen metabolism and the development of retinal diseases. Age, myopia and systemic diseases and other factors have an impact on vitreous function. Intra-vitreous injection and vitreous surgery may interfere with vitreous function, damage the intraocular metabolic balance. To better maintain intraocular metabolic balance and visual function, it is important to study vitreous metabolic function carefully and intervene it cautiously.

  • Vitreous condition of the eye with diabetic macular edema

    ObjectiveTo investigate the relationship between the posterior vitreous detachment (PVD) and the diabetic macular edema (DME).MethodsA total of 169 eyes with DME which were diagnosed by funduscopy,fundus fluorescein angiography (FFA) and optical coherence tomography (OCT) were retrospectively analysed. PVD and partial PVD were detected by OCT and biomicroscopy. The characterizations of OCT images of DME were analysed.ResultsIn 169 eyes, 156 (92.3%) had no PVD with the average thickness of fovea of 297 μm; 11 (6.5%) had complete PVD with the average thickness of fovea of 229 μm; 2 (1.2%) had partial PVD with the average thickness of fovea of 347 μm.ConclusionsOnly a few PVD were found in DME eyes, so traction of PVD may not be the main cause of DME.   • Anatomic evaluation of vitreous surgery of idiopathic macular hole with optical coherence tomography

    Purpose To evaluate the surgical effect of idiopathic macular hole (IMH),with optical coherence tomography (OCT). Methods To measure the diameter of holes and hole halos,the thickness of hole in edge,the changes of hole pattern in 19 cases (19 eyes) with IMH using OCT. Results By quantitatively measuring,OCT showed that after surgery the mean hole diameter lessened from (570.95plusmn;264.59 )mu;m to (337.05plusmn;335.95)mu;m.The mean diameter of hole halo lessened from (1043.53plusmn;278.80)mu;m to (695.00plusmn;483.00)mu;m and the mean thickness of the hole edge thinned from (389.78plusmn;60.58)mu;m to (298.78plusmn;109.80)mu;m.In 17 eyes the holes become closed or reduced in size or the hole halo,and thinned in hole edge.The anatomic successful rate of the surgery is 89%. Conclusion OCT can objectively,accurately and effectively judge the anatomic effect of IMH surgery. (Chin J Ocul Fundus Dis,2000,16:71-138)

  • Relationship between retinal extracellular edema and vitreous contraction in rabbits

    Objective To explore the correlation between retinal extracellular edema and vitreous contraction in rabbits. Methods Seventeen pigmented rabbit models with retinal vein occlusion (RVO)was set up by laser photocoagulation. Retinal vascularity area was pathologically examined 1 month later.The vitreous gellength under the gravity condition and the percentage of its weight in the rabbits with extraeellular edema was observed. The mechanisms were investigated by Western immunoblotting of type II collagen.Results Extracellular edema was found in 13 experimental eyes 1 month after the formation of RVO (76.5~) with contracted vitreous gel and released watery liquid, and the a component of type II collagen was cross-linked together to form high-molecular-weight components of 1] and 7, which weakened the stability of collagen net structure.Conclusions Vitreous contraction and retinal extracellular edemawere correlated. The main reason may be the cross-links of vitreous collagen that damages the stability of collagen structure. (Chin J Ocul Fundus Dis,2004,20:2-32)

  • The clinical analysis on bacterial isolates from the aqueous humor and the vitreous body of patients with suspected endophthalmitis

    Objective To review the distribution and shifting trends of cultured bacteria from the aqueous humor and the vitreous body. Methods A retrospective analysis on distribution of Gram′s stain, the distribution and change of isolates was performed in 522 specimens (aqueous humor,261 and vitreous body,261) of patients with suspected endophthalmitis during a 10-year period (1989-1998). Results The positive cultures were 119 (aqueous humor,44 and vitreous body,75) of 522 specimens. The average positive rate was 22.8%. Gram-positive cocci constituting 45.4%(54) of total isolates followed by Gram-negative bacilli,34.5%(41);Gram-positive bacilli, 20.2%(24). In the positive bacterial cultures, enterobacteriaceae was the most common isolate, 18.5%, and the next was micrococcus, 16.0%; coagulase-negative staphylococcus,12.6%; and pseudomonas,10.9%.Comparing the data from 1989 through 1993 with the data from 1994 through 1998, the frequency of Gram-positive cocci had no significant change, while the frequency of Gram-positive bacilli was decreased and the percentage of Gram′s-negative bacilli was increased. Conclusions Gram-positive cocci and Gram-negative bacilli are the predominant pathogens of bacterial endophthalmitis. The percentage of Gram′s-negative bacilli has increased for 5 years. It is very important to comprehend the distribution and shifting trends of these pathogenic bacteria for diagnosis, prevention and treatment of bacterial endophthalmitis.   • The visual function of idiopathic macular hole before and after surgery

    Objective To investigate the visual function of idiopathic macular hole before and after macular surgery. Methods Vitreous-retinal surgery was performed in 19 patients(19 eyes) with idiopthic macular hole.Among them,3 eyes belonged to stage 2,10 eyes stage 3 and 6 eyes were stage 4. The visual function including international standard visual acuity,laser retinal visual acuity,photopic ERG intensity responses- and Humphrey 30deg;-2 visual field were tested and compared before and after the surgery. Results (1)International standard visual acuity was from 0.01 to 0.1 preoperatively,and from 0.04 to 0.4 post operatively,the difference was not significant(P>0.0 5).(2)Laser retinal visual acuity was from 0.12 to 0.4 preoperatively,and from 0.2 to 0.63 postoperatively,the difference was significant (P<0.05).(3)In photopic ERG intensity responses:the Log K value of bwave decreased after surgery (P<0.05),but no obvious change found in Rmax and n Value after operation.(4)Humphrey visual field examination revealed the thresholds for 0-10deg; increased after surgery(P<0.05),but not obvious for 15-30deg;.   • Effects of human vitreous fluid on the proliferation of cultured human retinal pigment epithelial cells and vascular endothelial cells

    Purpose To investigate the effects of human vitreous fluid on proliferation of cultured human retinal pigment epithelial (RPE) cells and vascular endothelial cell lines(VEC304). Methods Human RPE cells and VEC304 were cultured and treated in different human vitreous-conditioned medium (VCM) with or without serum, vitreous volume concentrations of VCM were 1∶8, 1∶4 and 1∶2. Cells proliferation was assayed by tetrazolium (MTT) colorimetry at the 2nd, 4th and 6th day respectively. Results In the presence of serum, 1∶4, 1∶2 VCM had a significantly stimulative effect on RPE cells proliferation compared with control group at the 2nd, 4th, and 6th day retrospectively (P<0.01), but exerted a bly inhibitory effect on VEC304 proliferation compared with control group at the 2nd, 4th, and 6th day retrospectively (Plt;0.05). In the absence of serum, only 1∶2 VCM had a stimulative effect on RPE cells growth compared with control group at the 2nd day (P<0.05) and obviously at the 4th and 6th day respectively (P<0.01). Conclusion Human vitreous fluid influences human RPE cells and VEC304 growth in vitro. This result suggests that vitreous may play different role in proliferative vitreoretinopathy and intraocular neovascular disease. (Chin J Ocul Fundus Dis, 2002, 18: 140-142)
