  • Developments in researches on acute exacerbation of idiopathic pulmonary fibrosis

    Acute exacerbation of idiopathic pulmonary fibrosis (AE-IPF) is defined as an acute and clinically significant respiratory deterioration characterized by evidence of new, widespread alveolar abnormality. In the past, AE-IPF was considered to be idiopathic, which was hard to be prevented and its prognosis was hard to be obviously improved; the latest researches have shown that AE-IPF can be triggered by known causes, including pulmonary infection, aspiration, etc. This review summarizes the etiology or risk factors, treatment and prevention of AE-IPF according to the latest researches.

    This paper, focusing on vascular cognitive impairment, summarizes the current situation of cognitive impairment rehabilitation at home and abroad, and makes a comprehensive and systematic introduction and review on the concept, assessment, and treatment of cognitive impairment, and so on. This paper raises people’s awareness of cognitive impairment and guides people to make appropriate choices about assessment and treatment methods according to different conditions, in order to improve the diagnosis rate of cognitive impairment, and to comprehensively adopt various rehabilitation treatment methods to improve cognitive rehabilitation efficacy. At the same time, it points out the weak points and future development trend of cognitive impairment rehabilitation in order to help the future work.

  • Pulmonary mucoepidermoid carcinoma: a case report and review of the literature

    ObjectiveTo summarize the clinical features of pulmonary mucoepidermoid carcinoma (PMEC) and improve the level of the diagnosis and therapy.MethodsA case of PMEC was reported and related literatures were reviewed in PubMed, WanFang data and China National Knowledge Infrastructure.ResultsA 21-year-old female patient, complaining of cough and expectoration for 5 months, intermittent hemoptysis for 3 months and repeated fever more than 4 days, was hospitalized in March 2017. After admission, the patient was diagnosed as low-grade PMEC and received anti-infection, fibrobronchoscope sputum aspiration and alveolar lavage therapy. During the hospitalization, the anti-infection of the patients was not much effective, and the chest CT examination suggested space-occupying lesions in the middle lobe of the right lung. After the surgical removal of the right lung, the syndrome of the patient was improved. A total of 13 patients were reviewed, there was no obvious gender difference, and the median age was about 40 years old (most frequently occurred in younger adults with an average age less than 50 years). Frequent clinical manifestations were cough, fever and hemoptysis, which were often accompanied with atelectasis. Most of the patients could be improved after surgical resection, and a few need comprehensive treatment including radiotherapy, chemotherapy and targeted treatment, etc.ConclusionsPMEC is rare and the etiology is still unclear. The pathology is mainly low-grade. Majority of patients with PMEC could be cured after surgical resection.

  • Diagnosis and Treatment of Acute Acalculous Cholecystitis: Analysis of 79 Cases

    【Abstract】Objective To analysis the clinical characteristics, pathogenesis, diagnosis and treatment of acute acalculous cholecystitis.Methods Seventy-nine cases of acute acalculous cholecystitis from January 1996 to January 2003 were retrospectively reviewed.Results Of those 79 cases, 13 cases were treated nonoperatively and 66 cases were treated operatively. Twentythree cases were suppurative, 43 cases were gangrenous with perforation in 18 cases,which were proved by postoperative pathology. Seventysix cases were cured and 3 cases were dead. Conclusion Keeping vigilant alert, observing dynamically as well as appropriate operative intervention are effective to improve the prognosis of acute acalculous cholecystitis.

  • DIAGNOSIS AND TREATMENT OF LIVER HYDATIDOSIS IN NONPASTURELAND

    Objective To investigate the diagnosis and treatment of the liver hydatidosis in nonpastureland. Methods Clinical features of 16 patients with liver hydatidosis were analyzed retrospectively. Results Only 8 of 16 patients possessed the clinical symptoms and 8 patients had had history of inhabitancy in epidemic area. Casoni test and indirect hemagglutination showed a sensitivity of 90% and the correct diagnostic rate of CT was higher than that of B-ultrasound examination. The main effective treatment of the liver hydatidosis was surgical, 15 out of 16 patients received surgical treatment. In this series, the curative effect was good without any death, allergic reaction and implantation. Conclusion The cystic lesion of liver should be considered as liver hydatidosis and Casoni test, indirect hemagglutination, together with CT and B-us examination can be used to comfirm the diagnosis though no clinical symptoms and history of inhabitancy in epidemic area presented. Surgical operation is the main effective treatment for liver hydatidosis.

  • The Diagnostic and Treatment Experiences of Plasma Cell Mastitis in 53 Cases

    ObjectiveTo summarize the clinical manifestations and treatment of plasma cell mastitis. MethodsWe retrospectively analyzed the diagnosis and treatment of 53 patients with plasma cell mastitis admitted into the hospital between January 2008 and December 2014. ResultsTwelve patients with nipple discharge underwent mammary gland segment resection in which the complete discharging tube system and part of the normal gland were resected. In the 26 patients with inflammatory masses, 12 had acute inflammation with mass formation who were treated with abscess incision drainage and silver alginate wound dressing before second-stage lesion resection. Fourteen patients with only local swelling, tenderness and no abscess formation underwent lesion resection after the abscesses were reduced by combined traditional Chinese medicine (TCM) and western medical treatment. Seven patients with painless mass all had abscesses in the areola area, among whom 5 had complete inflammatory capsule and they underwent lumpectomy. Two patients had cheese-like tissue beside the abscess, and they underwent segmental resection. In the 8 patients with chronic fistula, one at the age of 51 was cured by simple mastectomy after recurrence, and the other 7 underwent lesion resection after combined TCM and western medical treatment. All the 53 patients were diagnosed to have plasma cell mastitis by surgical biopsy, and they were all cured with no recurrence 6 months after surgery. Conclusions Plasma cell mastitis is usually misdiagnosed because of its diversified clinical manifestations. For patients whose lesion is large and not confined, it's better for them to receive combined TCM and western medical treatment. Surgical time and approach should be chosen appropriately. Surgical lesion resection is the only method for complete treatment in patients with plasma cell mastitis.

  • Treatment for Concomitant Diseases of Other Abdominal Organs in Laparoscopic Cholecystectomy

    Objective To summarize the treatment experience for concomitant diseases of other abdominal organs in laparoscopic cholecystectomy (LC). Methods The clinical data of 176 patients with LC and concomitant diseases of other abdominal organs were analyzed retrospectively, including preoperatively diagnosed cases (such as 53 with liver cyst, 15 with choledocholithiasis, 7 with chronic appendicitis, 5 with inguinal hernia, 4 with renal cyst, and 6 with ovarian cyst) and intraoperatively diagnosed cases (such as 72 with abdominal cavity adhesion, 4 with internal fistula between gallbladder and digestive tract, 3 with Mirizzi syndrome, and 7 with unsuspected gallbladder carcinoma). Results All the operation were successfully completed in 176 patients without severe complications, including 53 cases treated with LC plus fenestration of hepatic cyst, 15 with choledocholithotomy, 7 with appendectomy, 5 with tension free hernia repair, 4 with renal cyst fenestration, 6 with oophorocystectomy, 72 with adhesiolysis, 3 with fistula resection plus intestine neoplasty, 2 with intraoperative cholangiography plus choledocholithotomy, 5 with LC plus gallbladder bed complete burning, and 4 cases treated with conversion to open surgery (1 with intestinal fistula repair, 1 with choledocholithotomy, and 2 with radical resection for gallbladder carcinoma). Conclusions It is safe and effective to treat gallbladder diseases complicated with other concomitant diseases simultaneously with laparoscopic operation, if the principles of surgical operation are followed and the indications and applicable conditions are strictly followed. And conversion to open surgery is necessary.

  • Diagnosis and Treatment of 40 Cases of Pancreatic Cystic Neoplasms

    ObjectiveTo investigate the diagnosis and treatment of pancreatic cystic neoplasm. MethodsThe clinical data of 40 cases of pancreatic cystic neoplasm from October 2001 to October 2013 in our hospital were retrospec-tively analyzed. ResultsPatients with pancreatic cystic neoplasm had no specific clinical feature. Ultrasonography and computed tomography displayed a cystic tumor in 57.5%(23/40) and 72.5%(29/40) of all patients, respectively, but could not distinguish the histological types. All of the patients had been operated, among them 2 cases were misdia-gnosed as pseudocysts and internal drainage; another 38 patients were undergoing the distal pancreatectomy. Pathologic examination results after operation showed 23 cases of serous cystadenoma, 9 cases of mucinous cystadenoma, 3 cases of intraductal papillary mucinous adenoma, and 5 cases of mutinous cystadenocarcinoma. Thirty five patients were followed-up. The followed-up time range from 2 months to 8 years, verage(74.2±12.8) months. Among the 3 patients with mucinous cystadenocarcinoma, 1 patient alived with no evidence of recurrence, the other 2 patients died of tumor invasion and metastasis in 4 months and 7months after operation. The others were still alive now with no evidence of recurrence. ConclusionSurgical resection is the most effective treatment for pancreatic cystic tumor, even if the patients with no any symptoms.

  • Influencing factors in treatment decision-making for breast cancer patients: a systematic review

    ObjectiveTo systematically review the influencing factors of breast cancer patients in treatment decision-making. MethodsWeb of Science, PubMed, EMbase, The Cochrane Library, JBI Evidence Synthesis, CINAHL, CNKI, WanFang Data, VIP and CBM databases were electronically searched to collect qualitative studies about the influencing factors of breast cancer patients in treatment decision-making from inception to October 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bias of the included studies. Then, the results were combined using integrating methods. ResultsA total of 13 studies were included. Sixty-seven results were extracted, with 22 results retained after incorporation and removal of duplications. The 22 results were grouped according to their similarities to form 6 categories. These categories resulted in two synthesized findings: influencing factors of patient participation in treatment decision-making and integration results and influencing factors of patients' treatment options. ConclusionBreast cancer patient participation in treatment decisions is influenced not only by internal factors, but also by family and external situational factors. When patients choose treatment, they consider not only their personal values and preferences, but also the impact of treatment on their families and their expectations. The choice is also influenced by social and cultural aspects.
