  • ADVANCES IN ANATOMICAL REPAIR OF CHRONIC LATERAL ANKLE INSTABILITY

    ObjectiveTo summarize the current status and progress of the treatment of chronic lateral ankle instability (CLAI). MethodsThe literature about the anatomical repair of CLAI at home and abroad was reviewed and summarized. ResultsBroström and its modified operations are the most common surgical treatment of CLAI. The operations showed satisfactory clinical outcomes in the short-, medium-, and long-term follow-up and low complication rate. Suture anchor technique and arthroscopic techniques are gradually used in Broström and its modified operations with satisfactory short-term effectiveness, but long-term effectiveness needs further observation because of the limitation of the short clinical application time. ConclusionBroström and its modified operations are effective, convenient, and safe to treat CLAI. Based on the researches of biomechanics and dynamic anatomy, the more personalized design of the rehabilitation program is the further research direction.

  • RESEARCH PROGRESS OF EXPERIMENTAL ANIMAL MODELS OF AVASCULAR NECROSIS OF FEMORAL HEAD

    ObjectiveTo summarize the current researches and progress on experimental animal models of avascular necrosis of the femoral head. MethodsDomestic and international literature concerning experimental animal models of avascular necrosis of the femoral head was reviewed and analyzed. ResultsThe methods to prepare the experimental animal models of avascular necrosis of the femoral head can be mainly concluded as traumatic methods (including surgical, physical, and chemical insult), and non-traumatic methods (including steroid, lipopolysaccharide, steroid combined with lipopolysaccharide, steroid combined with horse serum, etc). Each method has both merits and demerits, yet no ideal methods have been developed. ConclusionThere are many methods to prepare the experimental animal models of avascular necrosis of the femoral head, but proper model should be selected based on the aim of research. The establishment of ideal experimental animal models needs further research in future.

  • RESEARCH PROGRESS OF GENDER-SPECIFIC PROSTHESIS

    ObjectiveTo summarize the research progress of the design and effectiveness of gender-specific prosthesis in total knee arthroplasty (TKA). MethodsThe relevant literature on gender-specific prosthesis in recent years was extensively reviewed and analyzed. ResultsGender-specific prosthesis is designed according to the female knee joint anatomical characteristics. In theory, it should obtain better effectiveness. But a large number of clinical studies have shown that the knee function, pain, and satisfaction has no obvious advantage when compared with conventional prosthesis after TKA for female patient. ConclusionComprehensive evaluation should be considered when gender-specific is selected; and the effectiveness needs further follow-up.

  • Bibliometric and Research Hotspots Analysis of Inflammatory Pulmonary Pseudotumor between 2010 and 2014 Year

    ObjectiveTo investigate researches on inflammatory pulmonary pseudotumor between 2010 and 2014 year and to provide reference information for the majority of professionals in deep research. MethodsBibliographies from research literature of inflammatory pulmonary pseudotumor between 2010 and 2014 year in PubMed database were downloaded, the publication year, journals, countries of publication, the first authors and the frequency of major topic headings were counted by Bicomb 2.0 software. The affiliations were analyzed artificially. Major topic headings appeared no less than three times were intercepted as high frequency terms and high frequency. Major topic headings co-occurrence matrix were formed. SPSS 22.0 statistical software was applied for clustering analysis with matrix, then to get the topic hotspots. ResultsA total of 62 literatures were screened out. The data of research trend, journals, research degree of different countries were acquired. The number of high frequency major topic headings was 12 and among which 4 research hotspots were clustered. ConclusionResearches on inflammatory pulmonary pseudotumor are mainly in terms of pathology, diagnosis and treatment, etiology, and immunoassay.

    Object ive To summar ize recent advance in the appl icat ion and research of ar t i f icial chordae tendineae. Methods The cl inical and experimental research l iterature was extensively reviewed and analyzed. Results The follow-up results showed that artificial chordae tendineae replacement was superior to other operation methods in valve repair. But, it was compl icated and difficult-to-learn. In recent years with the development of many surgical skills and new techniques, good cl inical results were achieved. Conclusion With the development of surgical equi pment, chordae material, and implanting skills, artificial chordae tendineae implanting will be easier and the scope of appl ication will be larger.

MJMEs appeared no less than two times were intercepted as high frequency ones and the high frequency MJMEs co-occurrence matrix were formed. SPSS 22.0 statistical software were applied for clustering analysis with matrix, then to get the topic hotspots. ResultsA total of 28 literatures were screened out. The data of research trend, journals, research degree of countries were acquired. The number of high frequency MJMEs were 20 and among which 5 research hotspots were clustered. ConclusionsResearches on pancreatic neoplasms and molecular imaging are mainly in terms of therapy and genetics, diagnosis and metabolism, radionuclide imaging, pharmacology and pathology.

  • OneStage Hybrid Operation for Treating Descending Aortic Diseases Involving Distal Aortic Arch

    Abstract: Objective To summarize the clinical experiences of onestage hybrid operation for treating typeB aortic dissection and aortic aneurysm involving distal aortic arch, explore the indication for this special technique and analyze shortterm followup results. Methods From October 2008 to May 2010, 16 consecutive patients received onestage hybrid operation in Fu Wai Hospital for aortic dissection or aortic aneurysm involving distal aortic arch. There were 12 males and 4 females. Their age ranged from 38.0 to 67.0 years (54.0±9.2 years). There were 14 patients of typeB aortic dissection, 1 patient of thoracic aortic aneurysm involving the aortic arch, and 1 patient of penetrating aortic ulcer. Among them, 10 were acute ones and 6 were chronic ones. Brachiocephalic artery bypass using neck incision with retrograde endovascular stent graft implantation was used. Four patients received bypass from the left common carotid artery to the left subclavian artery; 11 patients underwent bypass from the right common carotid artery to the left common carotid artery; and 1 patient had both of the bypasses. Intensive care unit(ICU) stay, hospital stay, and incidence of complications were closely monitored after operation. Computed tomography(CT) and general condition of the bypass grafts and its blood flow were followed up at three months and one year after operation.  Results No death or severe complications occurred perioperatively. All onestage surgeries were technically successful with stent grafts implanted. Angiography during the operation showed 100% patency of all the bypass grafts and no obvious translocation or endoleakage of the stent grafts. One patient had mild paraplegia and recovered soon after treatment. Ventilation time of these patients was 2.010.0 hours (5.3±2.7 hours). The ICU stay was 0.0-2.0 days (1.1±0.4 days) and hospital stay was 4.0-7.0 days (5.3±0.8 days). The expenditure of blood products was 0.0-1 016.5 RMB (134.5±281.8 RMB). All patients were followed up with a followup period of 3.0 to 26.0 months (130±5.1 months). All patients recovered to normal social life. Enhanced CT scanning at three months and one year after operation showed no endoleakage or translocation of the stent grafts and 100% patency of the bypass grafts. There was no obvious change of the distal part of the dissection except some thrombosis formation in nine patients. Conclusion Onestage hybrid operation is safe and effective in shortening the duration of the operation and hospital stay, reducing the surgical trauma and sufferings of the patients, and lowering the risk of staged operations with satisfactory shortterm results. This special technique may expand the indications of simple endovascular repair. The mid and longterm results still need to be followed up.

  • Research progress of echocardiography in transcatheter aortic valve replacement

    Before transcatheter aortic valve replacement (TAVR), echocardiography is the first choice for preoperative screening of suitable patients, which can be used to observe the morphology of aortic valve, determine the cause of aortic stenosis, and evaluate the severity of aortic stenosis and other cardiac structure and function. During TAVR procedure, echocardiography is mainly used for real-time monitoring of complications and immediate postoperative evaluation. After TAVR, echocardiography can be used to evaluate the shape and function of the prosthesis valve and monitor long-term complications. This article reviews the research progress of echocardiography in TAVR for guiding clinical practice.
