  • HISTOLOGICAL STUDY OF TENDON IMPLANTED INTO BONE

    From the results of this experiment, it showed that the implanted tendon was gradually extruded from the tibia hole and attached to the periosteum. The dominant breeding of tissue cells, cytodynamics, the perimeter ratio of tendon/bone and the effect of revascularization were discussed in detail.

  • EXPERIMENTAL STUDY ON TENSILE STRENGTH OF CARBONTENDON IMPLANTEDINTO BONE

    A mechanical study on the bones of 29 rabbits following implantation of carbontendon was carried out. The rabbits were divided into seven groups according to the observation time (2,4,6,8,12,20 and 30 weeks after operation). A bundle of artificial tendon composed of 7,000 carbon fibers was passedthrough a tunnel in the tibia, and both ends of the artificial tendon were ligated to the muscle fibers. The mechanical strength and histological structure of the carbonbone junction and their relationship were studied in each group. Carbon fiberwas split and degradated in six to eight weeks after operation. The tensile strength of carbontendon in the soft tissue was decreased from 82±4.6N in the second week to 27±5.31N and6.3±1.81N in the sixth and eighth week respectively. The tensile strength of carbontendon increased from 3.01±1.2N to 6.1±2.01N at the carbon -tendon-bone junction in the bone. The tensile strength of carbon-tendon was unsatisfactory for implantation into bone. The carbon-tendon was split and degradated and the tensile strength was not b enough to cope with the early functional exercises.

  • INITIATIVE REPORT OF THE HYDROXYAPATITE ORBITAL IMPLANT DRILLING PROCEDURE

    OBJECTIVE: To evaluate the clinical effect of drilling procedure following the hydroxyapatite orbital implantation. METHODS: From February 1996 to April 2000, 146 consecutive patients who received hydroxyapatite orbital implant were drilled and inserted a motility peg 6 to 16 months after hydroxyapatite implantation. Among them, there were 97 males and 49 females, aged from 18 to 60 years old, of the 146 motility pegs, 36 were sleeved pegs and 110 were nonsleeved. Goldman visual field analyzer was applied to measure the degree of artificial eye’s movement before and after drilling. RESULTS: Followed up for 1 to 40 months, no secondary infection occurred. The mobility of the prosthesis increased from (18.7 +/- 3.8) degrees preoperatively to (42.3 +/- 3.7) degrees postoperatively. CONCLUSION: The delayed drilling procedure and motility peg insertion improve the range of movement and the sensitivity of the artificial eye with a low rate of complications.

  • ANALYSIS OF PURE TITANIUM IMPLANTS BONE INTERFACE BY TOF SIMS

    In order to study the Titanium-bone interaction and integration mechanism, the titanium implant was implanted in the tibia of 9 Newzealand rabbits, and the Ti-bone interface performed for 1, 3, and 6 months were examined and analyzed by fluorescence microscope and advanced TOF-SIMS techniques. The results showed that Ti-bone tissue was integrated closely in a very reactive manner. Both physical and chemical integration occurred in the Ti-bone interface. The Ti-bone could diffuse into the bone tissue though the diffusion was very limited. It was up to 100 microns in depth during the early period. The diffusion density was high, and later in a smooth distribution. Furthermore, while Ti+ diffused into the bone tissue, other elements such as Ca+, OH-, O-, etc, could also diffuse into titanium in exchange. The growth pattern around the bone tissue was in two fashions, one was implantefugal and the other was implantopetal. In this study, based on the ionic distribution, osmosis and impurity elements distribution, the Ti-bone integration mechanism was discussed at molecular and atomic level.

  • Clinical Application of New(GK) Bileaflet Mechanical Heart Valve

    Objective To introduce a new type of bileaflet mechanical prosthetic heart valve (GK bileaflet valve)and evaluate clinically the early hemodynamic effect and short term follow-up after its replacement. Methods Sixty-one patients with heart valve diseases were operated upon. The mitral valve replacement was performed in 34 patients, aortic valve replacement in 16 patients and double valve replacement in 11 patients. A total of 72 GK bileaflet mechanical valves were implanted, 45 in mitral position, and 27 in aortic position. Blood consistency and hemodynamics were monitored. Follow-up was carried out routinely to check whether there were some valve-related complications. Results There was no early mortality (〈30 d). Only one patient died of trauma 2 months after the operation. Follow-up was 100% and extended 1 to 2. 5 years. Without valve-related complications all patients had lived for more than 1 to 2.5 years. In 98% (60/61) of survivors heart functional performance had improved to New York Heart Association class Ⅰ or Ⅱ . Conclusion Early clinical results and short term follow up demonstrate that GK bileaflet prosthetic heart valve exhibits excellent hemodynamic properties, satisfied blood consistency and a low incidence of valve-related complications. Midterm and long-term results should be observed further.

  • Experimental Study on Iodine-125 Interstitial Brachytherapy in Breast Cancer

    Objective To investigate the effect of iodine-125 on inhibiting breast cancer growth and to explore its possible mechanism. Methods The animal model of the MCF-7 tumor was established firstly through injection of the cells into nude mice. The animals were divided into two groups before the implantation of the iodine-125 granule into the tumor mass: control group (n=40, to implant no-load seeds, non-iodine-125 radioelement) and experimental group 〔n=40, to implant iodine-125 seeds (1.48×107 Bq) when the length of tumor was 8-10 mm〕. The width and length of tumor, in order to calculate the volume, were measured every three days to observe tumor growth curve and to calculate the rate of the tumor inhibition. When the length of tumor was 15-20 mm in the control group, 30 nude mice were killed in every group to detect the weight of tumor and histopathological changes. Other ten nude mice of each group were remained to be observed the national life span and survival rate for 90 days. Results Within 90 days, the average survival time in control group and experimental group were significantly different (56.2 d vs. 74.8 d, P<0.05). In control group the growth curve was continuously elevated, while experimental group showed a low flat curve. With iodine-125 treatment, the tumor growth decreased in experimental group with tumor inhibition rate 55.21%. The average tumor weight in control group and experimental group was (3.26±0.39) g and (1.46±0.17) g (t′=22.8962, P<0.05). As compared with control group, under light microscope, the number of cancer cells was less, nuclear debris increased, and cancer structure was not obvious in experimental group. Conclusion This study suggests that iodine-125 seed may inhibit the growth of breast cancer, which may be involved in direct radiation breakdown of tumor cells or induction of apoptosis and inhabitation of tumor angiogenesis.

  • Current status and progress of implantable left ventricular assist devices

    Implantable left ventricular assist device (LVAD) has become an essential treatment for end-stage heart failure, and its effect has been continuously improved. In the world, magnetic levitation LVAD has become mainstream and is increasingly used as a destination treatment. China has also entered the era of ventricular assist device. The continuous improvement of the ventricular assist device will further improve the treatment effect. This article reviews the current situation and development trend of LVAD treatment in China and abroad.

  • REPAIR OF ORBITAL DEFECTS WITH IMPLANT-RETAINED PROSTHESES

    Objective To evaluate the clinical result of using osseointegratedimplants to retain removable orbital prostheses in repairing orbital defects. Methods Two patients with orbital defects caused by orbitaltumor were treated. Each of them got 4 implants. After average 6 months, we performed the secondary operation. After 7 weeks, we took impressions to make the implant-retained prostheses. The magnetic attachment was adopted. The prostheses were made of polysiloxane material(Factor II,Lakeside,Ariz). Results Both of the patients got the successful facial prostheses and were followed up 2 and 8 years respectively. All the implants were integrated well. There were no apparent inflammatory reactions in the soft tissue around percutaneous implants. The patients were satisfied with the facial appearance. Conclusion Implant-retained orbital prostheses are safe and effective in repairing orbital defects.

  • EXPERIMENTAL STUDY IN VIVO ON IMPLANTATION OF AUTOGENOUS TENDON CELLS AFTER COMBINING CULTURE WITH CARBON FIBERS

    In order to investigate the possibility of repairing injuried tendon with living artificial tendon, after combining culture, subcultured autogenous tendon cells with carbon fibers were implanted into the calcaneous tendon of rabbits. In different stages, the synthesis of type I collagen and their relevant morphological changes were observed. The results showed as follows: after implantation, tendon cells continued proliferating. Four weeks after implantation, tendon cells were detached from the carbon fibers and proliferated and produced collagen among the carbon fibers. The collagen fibrils were linked with each other to formed a dense structure. In the linkage site, the collagen fibrils originated from the implants joined to that from the ruptured end of the tendon, which meaned that the implant was healed with the recipient tendon. Observed under scanning electronic microscope, the tendon cells were lined among the carbon fibers evenly and in order, the collagen fibrils joined each other and formed an network, the fibrils were lined parallel to the carbon fibers. Under transparent electron microscope, the nucleolus were clear and organelle were abundant.

  • Rudiment Research and in Vitro Evaluation of a New Improvement Single Layer Suture Stentless Porcine Aortic Bioprosthesis

    Objective To design a novel stentless porcine aortic bioprosthesis and test the feasibility and its function in vitro after the valve was implanted by a modified method. Methods Six stentless porcine aortic bioprosthesis were divided into two groups according to different implantation, single layer suture group: new improvement stentless porcine aortic bioprosthesis sutured with single layer was implanted; double layer suture group: stentless porcine aortic bioprosthesis developmented by our laboratory used double layer suture was implanted. Each group contained three scales: 23 mm ,25 mm and 27 mm. Analogue ex vivo aortic valve replacement was performed , the feasibility of the new implantation was detected. Effective orifice area, transvalvular pressure gradient and regurgitation ratio were recorded at the cardiac output of 2.0 L/min, 3.5 L/min, 5.0 L/min and 7.0 L/min under the guideline of International Organization for tandardization (ISO)5840. Results The average aortic valve implantation time used for single layer suture and tradition double layer suture were 50 min and 70 min respectively. The transvalvular pressure gradient in the single layer suture group were significantly lower than those in double layer suture group under the flow of 5.0 L/min in 23 mm valve and 27 mm valve (13.51±0.51 mm Hg vs. 14.44±0.99 mm Hg, 7.36±0.19 mm Hg vs. 7.53±0.28 mm Hg;P<0.01);and the effective orifice area in the single layer suture group were larger than those in double layer suture group in the same case(1.87±0.06 cm2 vs. 1.76±0.08 cm2, 2.26±0.07 cm2 vs. 2.16±0.05 cm2;P<0.01). There was no statistically difference in other parameters between both groups. Conclusion The novel design of new improvement stentless porcine aortic bioprosthesis used single layer suture has good hemodynamic characteristics as the nature structure . The modified suture method decrease the implantation time.Nemerical data of the evaluation in vitro show that the difference between single layer suture group and double layer suture group in effective orifice area,transvalvular pressure gradient and regurgitation ratio haveno statistical significance. This experiment is the foundation of the animal and clinical experiment in the future.
