  • Digital drainage system versus traditional drainage system for thoracic drainage after lung surgery: A case control study

    Objective To compare the digital drainage system and the traditional drainage system in the patients after lung surgery, and to evaluate the advantages of digital drainage system. Methods A retrospective analysis of consecutive 42 patients with lung surgery between September 2016 and May 2017 in Beijing University International Hospital was done. There were 30 males and 12 females with a median age of 34 years ranging 19-81 years. After the surgery 21 patients adopted Thopaz digital drainage device (a DDS group), and the other 21 patients adopted traditional drainage (a TDS group). Duration of air leakage and chest tube placement, length of hospital stay, thoracic drainage volume within 48 h and hospitalization expenses in the two groups were compared. Results The patients in the two groups were all successfully discharged. Compared with the TDS group, duration of air leakage and chest tube placement and length of hospital stay significantly shortened in the DDS group (35.6±16.3 h vs. 48.2±20.1 h, P=0.02; 50.0±16.1 h vs. 62.0±20.4 h, P=0.03; 5.9±2.3 d vs. 7.8±3.5 d, P=0.02), and thoracic drainage volume within 48 h and hospitalization expenses showed no significant statistical difference between the two groups. Conclusion Using digital drainage system after lung surgery can significantly shorten the duration of air leakage and the postoperative drainage, at the same time, without increasing the overall hospitalization expenses.

  • Application progress of digital technology in auricle reconstruction

    Objective To review the application progress of digital technology in auricle reconstruction. Methods The recently published literature concerning the application of digital technology in auricle reconstruction was extensively consulted, the main technology and its specific application areas were reviewed. Results Application of digital technology represented by three-dimensional (3D) data acquisition, 3D reconstruction, and 3D printing is an important developing trend of auricle reconstruction. It can precisely guide auricle reconstruction through fabricating digital ear model, auricular guide plate, and costal cartilage imaging. Conclusion Digital technology can improve effectiveness and decrease surgical trauma in auricle reconstruction. 3D bioprinting of ear cartilage future has bright prospect and needs to be further researched.

  • CLINICAL APPLICATION OF VASCULARIZED SKIN FLAP WITH NERVE (REPORT OF 10 CASES)

    The digital defects which severely interfered with the hand functions must be reconstructed. The primary repair by vascularized skin flap with the dorsal branch of proper palmar digital nerve could be done. The vascularized flap was rich in blood circulation, perfect sensibility and a good contour, as well as preserving donor digital sensation. However, it was simple, safe, and with higher success. From 1989 to 1991, 10 cases of digital defects were treated, all of the patients gained very good results. The indication and technique of thismethod wasdiscussed in detail.

  • Optimize Infants Chest Radiation Dose and Image Quality in DR: A Prospective Randomized Controlled Trial

    Objective To optimize image quality and radiation dose of infant chest digital radiography and to explore feasibility of reducing tube voltage and adjusting according to infant chest area. Methods 0 to 3-year-old infants were randomly divided into two average groups of 0- and 1-3 year-old, and then each age group was randomly assigned to optimization and control groups in digital radiography. Measurement of radiation dose used dose area product (DAP). Mean DAP between groups was compared by using t test, and the image quality of optimization was compared by rank sum test. Results A total of 400 cases of 0 to 3-year-old infants were identified, and finally 391 cases of infants anteroposterior chest image were included, including 196 cases in the optimization group (0-years: n=91; 1-3 years: n=105) and 195 cases in the control group (0-years: n=103; 1-3 years: n=92). The results showed: there were significant differences in the mean DAP in 0-years, 1-3 years and total infants between the optimization group and the control group (all P valuelt;0.05). The DAP of the optimization group was lower, and reduction of DAP was approximately 21.6% compared to the control subject. The Wilcoxon signed-rank test showed the difference of subjective evaluation of image quality was significantly different (P=0.000). High-quality image of the optimization group increased approximately 43.9% more than control subject. Conclusion Reducing tube voltage and adjusting according to infants chest area can not only reduce the radiation dose but also improve image quality in digital radiography.

  • Multidetector-Row CT Angiography and Digital Subtraction Angiography of Bronchial Artery in Diagnosis of Hemoptysis: A Comparative Study

    Objective To compare the bronchial arteriography through multidetector-row CT (MDCT) with the digital subtraction angiography (DSA) via femoral artery, and evaluate the application value of bronchial arteriography through MDCT in the diagnosis and treatment of hemoptysis. Methods 133 cases complained of hemoptysis were examined by MDCT and DSA via femoral artery respectively to perform bronchial arteriography, and the differences of image results by two methods were compared. Results 129 cases with abnormal bronchial arteries were confirmed by DSA via femoral artery, 117 cases were checked by MDCT [ the positive rate was 90.7% (117/129 ) ] . 117 cases with abnormal bronchial arteries were confirmed by both MDCT and DSA via femoral artery and 4 cases did not detected any abnormal arteries by both methods. The coincidence rate of two methods was 91.0% (121 /133) . MDCT and DSA via femoral artery showed the similar origins of abnormal bronchial arteries. The coincidence rate of two methods was 100% . Conclusions There is a high coincidence rate betweenMDCT and DSA in detecting bronchial artery abnormalities. MDCT shows the origins of abnormal vessels clearly which could be a fist-choice of routine imagination for interventive operation.

  • APPLICATION VALUE OF DIGITAL SUBTRACTION ANGIOGRAPHY IN REPAIR OF FOOT AND ANKLE WOUNDS WITH POSTERIOR TIBIAL ARTERIAL PERFORATOR FLAP

    ObjectiveTo explore the application value of digital subtraction angiography (DSA) in repairing foot and ankle wounds with posterior tibial arterial perforator flaps. MethodsBetween January 2010 and May 2014, 12 cases of foot and ankle wounds were repaired using posterior tibial arterial perforator flaps. There were 7 males and 5 females with an average age of 36 years (range, 22-54 years). The causes were machine injury in 2 cases, falling injury in 3 cases, and traffic accident injury in 7 cases. The disease duration ranged from 7 to 45 days (mean, 16 days). The size of wound ranged from 6 cm×4 cm to 10 cm×5 cm. Preoperative DSA was performed to observe the orientation and distribution of the posterior tibial arterial perforator and the relationship between perforator vessels. Correspondently, the flaps were designed and harvested. The size of flap ranged from 7 cm×5 cm to 11 cm×6 cm. The donor sites were repaired with skin grafts. ResultsPosterior tibial arterial perforator vessels send out ascending branches and descending branches while going down in the superficial layer. All branches were connected to form vertical chain-form anastamosis, and its orientation was consistent with limb vertical axis. According to DSA results, the flaps were designed and harvested easily. All flaps survived after operation. Meanwhile, wounds healed by first intention. All skin grafts at donor site survived. All patients were followed up 6 months. The flaps had good appearance, color, and texture. No ulcer was found. Affected feet had normal walking function. ConclusionThe size, distribution, and chain-form anastamosis condition of the posterior tibial arterial perforator vessels can be accurately observed by DSA, which provides imaging evidence for harvesting posterior tibial arterial perforator flaps and improves the success rate for repairing foot and ankle wounds.

  • V-Y ADVANCEMENT OF VOLAR SKIN FLAPS PEDICLED BY DIGITAL AR-TERIES AND NERVES FOR RECONSTRUCTION OF DIGITAL SKIN DEFECT

    The traditional Kutler and Atasoy V-Y advancement flaps have minimal advancement degree, did not satisfy to repair large skin defect in fingers, hence, have no wide indications. The sensory function of the fingers to be influenced because of injury of sensory nerves and sear formation. Since 1985 to 1991, the V-Y advancement flaps pedicled by bilateral digital arteries and nerves have been used for reconstruction of 33 finger tip defect and 5 digital volar skin contraction. All of these cases obtained satisfactory function and excellent appearance.In this paper, the anatomical charactistics and principles of devicerecommended, the advantages and key points to success discussed.

  • The development and present situation of limb replanting and reconstruction in China

    The replantation of amputated finger (or limb) and the reconstruction of the thumb and fingers started in 1960s in China. With the development of microsurgical techniques, both finger replantation and reconstruction achieved worldwide significant success. And now it is not the survival rate but the function of replanted finger that attracted more attention. Moreover, the breakthrough of particular type of digital replantation has broadened its indications. In terms of reconstruction, not only that a great deal of experiences in dealing with different types of thumb defects was accumulated, but also more micro-surgeons focus on minimal invasion and reducing the damage to the donor area.

  • Volume Variations of Regions of Interest among Different Radiological Treatment Planning Systems

    Objective To investigate the consistency of regions of interest (ROI) volume among different radiological treatment planning systems (TPS) for the same group of patient data, and analyze the tendency and degree of differences caused by data transfer. Methods Between October 2010 and December 2013, the data of 10 nasopharyngeal carcinoma patients treated in West China Hospital were transferred from Monaco TPS into various other treatment planning systems. Based on different ROI volumes, they were divided into 8 groups. We counted the volume differences between these TPS and Monaco TPS, and carried out the statistical analysis. Results For small ROI volume, the calculated difference reached up to 65% in our study. As a general trend, differences became less and less with the increasing of volumes. But for single ROI, the volume difference was likely to vary randomly. The percentage of ROI volumes which were smaller than that of Monaco TPS was 70% for Raystation TPS, 38.75% for Pinnacle TPS, 88.75% for Eclipse TPS, 97.5% for Masterplan TPS, and 83.13% for iPlan TPS. Conclusions ROI volume differences exist generally among different treatment planning systems when ROIs are transferred among them by DICOM protocol. The volume variations may be affected by multiple factors. The volume consistency should be evaluated before any direct comparison of dose volu me histogram parameters which are done between different systems.
