  • 牵引床辅助下锁定钢板内固定治疗高龄股骨转子间骨折手术配合

    目的 探讨牵引床辅助下锁定钢板内固定治疗高龄股骨转子间骨折的手术护理配合方法。 方法 2009年3月-2010年12月对54例高龄股骨转子间骨折患者在牵引床辅助下,实施股骨近端锁定加压钢板内固定术,其巡回护士、器械护士按护理规范在术前、术中予以积极配合。 结果 54例患者手术过程顺利,术后切口均Ⅰ期愈合,随访3~12个月,所有患者骨折全部愈合,髋关节功能恢复良好。 结论 周密的术前准备和术中娴熟的配合技术是确保手术安全顺利进行的有力保证。

  • Application of tranexamic acid in elderly patients during off-pump coronary artery bypass grafting: A randomized controlled trial

    Objective To investigate the efficacy and safety of tranexamic acid in patients with the age>70 years during off-pump coronary artery bypass grafting (OPCABG). Methods From June 2014 to August 2016, 340 patients undergoing elective OPCABG were included in this study. All the patients were more than 70 years old, among whom 282 were males and 58 were females. According to the random number generation method, the patients were randomly assigned to receive either tranexamic acid (30 mg/kg, infusion time was longer than 30 min after anesthesia induction; n=170) or a placebo (infusion equivalent volume of saline solution; n=170). The primary end point was chest tube drainage 6 h and 24 h postoperatively. The secondary end points were blood transfusion volumes, incidence of various thrombotic events, duration of mechanical ventilation, length of ICU and hospital stay. Results Compared with patients in the placebo group, the patients receiving tranexamic acid had a significant reduction in chest tube drainage at postoperative 6 h (275.6±105.1 ml vs. 459.6±110.2 ml, P<0.001) and 24 h (685.3±202.5 ml vs. 915.9±223.6 ml, P<0.001). There was also a significant reduction in allogeneic red blood cell transfusion (0.80±0.66 U vs. 1.60±1.30 U, P<0.001) and fresh frozen plasma transfusion (166±25 ml vs. 257±30 ml, P<0.001). There were no significant differences in incidence of various thrombotic events, duration of mechanical ventilation, length of ICU and hospital stay between the two groups. Conclusion Tranexamic acid can significantly reduce blood loss and transfusion in elderly patients 6 h and 24 h after OPCABG and the incidence of thrombotic events will not increase.

  • Application of Round Stapler for Anterior Resection of Gastric Fundus and Cardia Cancer in Elderly Patients

    目的 探讨圆形吻合器在高龄贲门胃底癌经腹切除术中的应用。方法 回顾性分析1999年1月至2009年5月期间在我院接受圆形吻合器经腹切除术的238例70岁以上高龄贲门胃底癌患者的临床资料,并对围手术期结果进行分析。 结果 全组手术时间3~4.5 h,平均3.5 h。出血量50~1 000 ml,平均300 ml。术中并发大出血1例,给予积极止血,无术中死亡。所有病例机械吻合术后未出现吻合口漏。术后60例患者(25%)出现不同程度的并发症,其中吻合口出血2例, 左侧胸腔积液13例,肺部感染15例, 心衰3例, 肺不张2例, 心律失常10例,右侧胸腔积液15例。无围手术期死亡。术后随访3年,出现返流性食管炎45例,吻合口狭窄30例,无吻合口肿瘤复发,3年生存率为64%(152/238)。结论 应用圆形吻合器在高龄胃底贲门癌患者经腹切除术是安全、有效的。

  • Research progress on unicompartmental knee arthroplasty for elderly patients with knee osteoarthritis

    Objective To conclude the research progress of unicompartmental knee arthroplasty (UKA) in the treatment of elderly patients with knee osteoarthritis (KOA). Methods The literature on UKA at home and abroad in recent years was reviewed to summarize the clinical characteristics of elderly patients with KOA, perioperative management (including evaluating indications preoperatively, intraoperative prosthesis selection, postoperative complication management, etc). Results Through reasonable preoperative evaluation, prosthesis selection, and advanced perioperative management, for elderly patients with KOA who meet the indications, UKA can be considered. Compared with total knee arthroplasty, the incidence of postoperative complications in elderly patients undergoing UKA is lower, joint awareness is reduced, functional improvement and satisfaction are higher. Meanwhile, choosing appropriate prostheses and fixation methods can lead to a good survival rate. ConclusionUKA can provide a safe and effective treatment option for elderly patients with KOA within a certain range of indications.

  • The Examination of Corneal Endothelial Cells and Center Corneal Thickness in the Senile Patients before Cataract Surgery

    目的:探讨高龄白内障患者术前角膜内皮功能状态。方法:对从2007年11月到2008年10月的66例年龄在80岁或以上的左眼白内障患者术前角膜内皮细胞形态及中央角膜厚度进行回顾性分析。分析参数包括角膜内皮细胞密度,内皮细胞平均面积,最大细胞面积和最小细胞面积,内皮细胞变异系数,面积标准差,六角形细胞比例及角膜中央厚度。结果:患者的角膜内皮细胞平均密度为(2720.3±324.6)个/mm2,平均细胞面积为(375.4±70.3)μm2,最大细胞面积(922.1±243.5)μm2,最小细胞面积(131.2±40.2)μm2,面积标准差(143.5±34.8)μm2,细胞变异系数(38.0±7.3)%,六角形细胞比例(52.8±10.1)%。中央角膜厚度与角膜内皮细胞密度之间无相关性。结论:高龄人白内障患者的角膜内皮细胞密度高,角膜功能基本正常,是保持角膜透明的基础,为眼内手术或治疗提供安全保障。

  • A HLM analysis on the influencing factors of cognitive performance among the old elderly

    ObjectivesTo examine the cognitive performance among the elderly and associated influencing factors.MethodsUsing data from the six issues of Chinese Longitudinal Healthy Longevity Survey (CLHLS) conducted from 1998 to 2011, selecting adults aged 80 years or above, multilevel hierarchical model was utilized to estimate the trend of the elderly's cognitive performance and its influencing factors.ResultsA total of 128 cases were studied. The descent in MMSE was not significant with the growth of age (P=0.834). Female's MMSE scores were lower than male's (P=0.011) and descended quicker (P=0.015). Furthermore, ADL status and level of leisure activity were significantly related to the cognitive performance of both male and female (P<0.05).ConclusionsParticipation in leisure activities were the crucial protective factor for cognitive performance. It's recommended that measures be taken respectively for elderly males and females, and policies be stressed to enhance their cognitive performance, thus to guarantee their healthy aging.

  • Transcatheter aortic valve replacement for severe aortic stenosis complicated by severe scoliosis: a case report

    Patients with severe aortic stenosis will gradually develop symptoms of heart failure. Aortic valve replacement is an effective treatment at present, while transcatheter aortic valve replacement is suitable for high-risk elderly patients. This article reports a patient with severe aortic valve stenosis and severe scoliosis who underwent transcatheter aortic valve replacement. Detailed preoperative evaluation and avoidance of peripheral vascular injury were the characteristics of this case. The patient was an elderly male with severe scoliosis. After fully evaluating the risks and benefits of surgery, a surgical strategy was formulated. The patient was in the supine position to complete the preoperative CT, and then the long sheath was used to pass through the thoracoabdominal aortic angle during the operation. After angiography-assisted adjustment and determination of the optimal release angle, the valve was accurately released, and the operation went smoothly without serious peripheral vascular damage.

  • Research progress on metabolic and bariatric surgery in older obese patients

    ObjectiveTo systematically review the research progress in the selection of metabolic bariatric procedures, efficacy, safety, complication prevention, and long-term management for elderly patients, so as to provide references for surgical decision-making and perioperative management in future metabolic and bariatric surgery for elderly obese patients. MethodA review of recent domestic and international literature on metabolic and bariatric surgery in elderly patients was conducted. ResultsWith the intensification of societal aging, the incidence of obesity and related metabolic diseases among the elderly population has significantly increased. Metabolic and bariatric surgery has been proven to effectively reduce weight and improve obesity-related metabolic diseases in elderly patients. Current guidelines no longer consider age an absolute contraindication for surgery. Sleeve gastrectomy (SG) and Roux-en-Y gastric bypass (RYGB) are the most commonly used procedures internationally. SG is superior to RYGB in terms of surgical safety, while RYGB has greater advantages in improving metabolic diseases. Although the postoperative mortality and complications risks in elderly obese patients are higher than those in younger obese patients, strict preoperative assessment and individualized procedure selection can significantly reduce these risks. ConclusionsMetabolic and bariatric surgery can serve as an effective treatment for elderly obese patients, offering comprehensive benefits in weight loss and metabolic improvement. Procedure selection requires individualized assessment, balancing the patient’s baseline condition and metabolic needs. Postoperative success hinges on systematic long-term follow-up and management to ensure sustained benefits and safety. Future efforts should focus on clarifying the definitions or standards of “elderly” and “obesity,” followed by more large-sample, long-term follow-up randomized controlled studies to validate the safety and efficacy of metabolic and bariatric surgery.
