  • 带旋髂深血管蒂髂骨及髂骨膜瓣移位治疗中青年股骨颈骨折

    目的 总结带旋髂深血管蒂髂骨及髂骨膜瓣移位治疗中青年股骨颈骨折的手术方法及临床疗效。 方法 2003 年4 月- 2007 年3 月,收治22 例中青年股骨颈骨折患者。男12 例,女10 例;年龄25 ~ 53 岁,平均42 岁。均为闭合性骨折。致伤原因:交通事故伤18 例,高处坠落伤4 例。受伤至手术时间5 h ~ 43 d。新鲜骨折20 例,陈旧性骨折2 例。骨折部位分型:头下型10 例,经颈型12 例。骨折Garden 分型:Ⅲ型6 例,Ⅳ型16 例。术前Harris 髋关节功能评分(22.80 ± 8.75)分。采用带旋髂深血管蒂髂骨及髂骨膜瓣移位加空心加压螺钉治疗。 结果 术后1 例供区伤口伴液化渗出,经换药治愈;其余切口均Ⅰ期愈合。术后患者均获随访,随访时间20 ~ 60 个月,平均42 个月。21 例股骨颈骨折愈合,愈合时间3 ~ 6 个月,平均3.5 个月;1 例骨折不愈合并股骨头缺血性坏死,于术后6 个月行人工全髋关节置换术。术后14 个月Harris 评分为(86.40 ± 7.95)分,与术前比较差异有统计学意义(P lt; 0.05)。 结论 带旋髂深血管蒂髂骨及髂骨膜瓣治疗中青年股骨颈骨折可改善骨折端血运,促进骨折愈合,减少股骨头坏死的发生。

  • TREATING AVASCULAR NECROSIS OF FEMORAL HEAD IN YOUNG ADULT BY GRAFTING SARTORIUSMUSCLE ILIAC BONE FLAP

    Objective To explore an improved method of treating avascular necrosis of the femoral head in young adults by grafting the sartorius muscle iliac bone flap. Methods From September 1994 to August 2003, 68 patients (57 males, 11 females; age, 16-58 years) underwent of the transplant the sartorius muscle oliac bone flap into the femoral head after decompression of the femoral head medullary core and removal of the dead bone and the fibrous tissue in the femoral head. The transplantation was performed on 31 patients left-unilaterally, on 37 patients rightunilaterally, and on 7 patients bilaterally. The pathological causeswere as follows: alcoholism in 52 patients, prolonged use of hormones in 6, traumain the hip in 6, and undetermined cause in 4. Their illness course ranged from 8 months to 4 years. According the Ficat staging, 10 patients belonged to Stage I (11 sides), 27 patients to Stage Ⅱ (31 sides), and31 patients to Stage Ⅲ (33 sides). Results The follow-up of the 68 patientsfor 2.5-11 years averaged 5.2 years revealed that based on the Harris evaluation for the hip function, 23 patients had an excellent result, 33 had a good result, 10 had a fair result, and 2 had a poor result. The excellent and good resultsaccounted for 82.3%. There was no recurrence after operation. Conclusion Thismethod has the following advantages: the lesion focus can be eradicated; enoughdecompression can be achieved, and the blood circulation for the femoral head can be rebuilt. The grafting of the sartorius muscle iliac bone flap can bring the osteogenesis components to the femoral head, promoting the reconstruction of the bones. This method is suitable and effective for the patients with avascular necrosis of the femoral head (Ficat Grades Ⅰ, Ⅱ and Ⅲ) in young adults. 

  • CLINICAL APPLICATION OF THREE DIMENSIONAL PRINTED NAVIGATION TEMPLATES FOR TREATMENT OF OSTEONECROSIS OF FEMORAL HEAD WITH PEDICLED ILIAC BONE GRAFT

    ObjectiveTo investigate the feasibility and early effectiveness to treat osteonecrosis of the femoral head (ONFH) with pedicled iliac bone graft assisted by individual digital design and three dimensional (3D) printed navigation templates. MethodsBetween February and June 2014, 15 patients (24 hips) with ONFH underwent pedicled iliac bone graft assisted by individual digital design and 3D printed navigation templates. There were 11 males (17 hips) and 4 females (7 hips) with a mean age of 38 years (range, 18-56 years) and a mean disease duration of 7.5 months (range, 1-24 months); the left hip was involved in 2 cases, the right hip in 4 cases, and both hips in 9 cases. There were 7 cases (12 hips) of steroid-induced ONFH, 5 cases (8 hips) of alcohol-induced ONFH, 1 case (1 hip) of traumatic ONFH, and 2 cases (3 hips) of idiopathic ONFH. The preoperative Harris score was 56.60±6.97. According to Association Research Circulation Osseous (ARCO) staging system, 5 hips were classified as stage IIB, 8 hips as stage IIC, 6 hips as stage IIIB, and 5 hips as stage IIIC. The navigation templates were designed and printed to assist accurate location and debridement of necrosis area according to preoperative CT scanning at the beginning of pedicled iliac bone grafting procedure. ResultsThe mean operation time was 135 minutes (range, 120-160 minutes), mean amount of bleeding was 255 mL (range, 200-300 mL). All the wounds healed primarily, no complication of deep vein thrombosis or infection was observed. All patients were followed up 12-16 months (mean, 14 months). The location of necrosis area was in accordance with preoperative design, which was removed completely without penetration of joint surface, pedicled iliac bone graft was performed at the right site according to postoperative imaging examination. Radiographically, graft fusion was achieved at 2.7 months (range, 2-3 months) in all patients. All the hips had no collapse during follow-up. Hip pain was relieved, and range of motion was improved. The Harris score was significantly improved to 89.53±5.83 at last follow-up (t=14.319, P=0.000). The results were excellent in 12 hips, good in 10 hips, and fair in 2 hips according to Harris score standard. ConclusionPedicled iliac bone graft assisted by individual digital design and 3D printed navigation templates for treatment of adult ONFH has the advantages of accurate location and complete debridement of necrosis area, so satisfactory results can be obtained.

  • Clinical study of a new biodegradable magnesium internal fixation screw in treatment of osteonecrosis of the femoral head

    Objective To investigate the safety and efficacy of a new biodegradable magnesium internal fixation screw for vascularized iliac bone flap grafting in treatment of osteonecrosis of the femoral head (ONFH). Methods Patients with ONFH admitted between July 2020 and February 2021 were selected as the research objects, and 20 patients (20 hips) met the selection criteria and were included in the study. The patients were divided into two groups (n=10) by central random method. The iliac bone flap was fixed with a new biodegradable magnesium internal fixation screw in the trial group, and the iliac bone flap was wedged directly in the control group. There was no significant difference (P>0.05) in gender, age, and side, type, Association Research Circulation Osseous (ARCO) stage, and disease duration of ONFH between the two groups. The operation time and intraoperative blood loss of the two groups were recorded. Laboratory tests were performed at each time point before and after operation, including white blood cell (WBC), electrolytes (K, Ca, P, Mg), blood urea nitrogen (BUN), serum creatinine (Scr), glomerular filtration rate (eGFR), lymphocyte ratio (CD4/CD8), immunoglobulin G (IgG), IgM, alanine transaminase (ALT), aspartate aminotransferase (AST). After operation, Harris score was used to evaluate the hip joint function. CT of the hip joint and X-ray films in anteroposterior and frog positions of the pelvis were used to review the iliac bone flap position, fusion, and screw biodegradation in the trial group. Results The vital signs of the two groups were stable, the incisions healed by first intention, and no adverse events occurred after operation. One patient in the control group refused to return to the hospital for follow-up at 3 months after operation, and 1 patient in the trial group refused to return to the hospital for follow-up at 1 year after operation. The rest of the patients completed the follow-up at 2 weeks, 3 months, 6 months, and 1 year after operation. Laboratory tests showed that there was no significant difference in WBC, electrolytes (K, Ca, P, Mg), BUN, Scr, eGFR, CD4/CD8, IgG, IgM, ALT, and AST between the two groups at each time point before and after operation (P>0.05). The operation time and intraoperative blood loss of the trial group were significantly less than those of the control group (P<0.05). The Harris scores of the two groups at 1 year significantly increased when compared with the values before operation and at 6 months after operation (P<0.05). There was no significant difference in Harris score between the two groups at each time point (P>0.05). Postoperative CT of hip joint and X-ray films of pelvis showed that the iliac bone flap reached osseous fusion with the fenestration of the head and neck junction of femoral head in the two groups at 1 year after operation, and no loosening or shedding of iliac bone flap was observed during follow-up. In the trial group, there were signs of dissolution and absorption of the new biodegradable magnesium internal fixation screws after operation, and the diameter of the screws gradually decreased (P<0.05); no screw breakage or detachment occurred during follow-up. Conclusion In the treatment of ONFH with vascularized iliac bone flap grafting, the new biodegradable magnesium internal fixation screws can fix the iliac bone flap firmly. Compared with the traditional iliac bone flap wedging directly, it has a shorter operation time, less intraoperative blood loss, and can obtain similar joint function.

  • Application of one-stage posterior surgery via unilateral musculussacrospinalis iliac flap approach in treatment of lumbosacral tuberculosis

    ObjectiveTo evaluate the safety and effectiveness of one-stage posterior surgery via unilateral musculussacrospinalis iliac flap approach in treatment of lumbosacral tuberculosis.MethodsBetween August 2011 and October 2014, 13 patients with lumbosacral tuberculosis were treated by one-stage posterior reserved posterior ligament complex, lesion debridement, bone graft fusion, and internal fixation via unilateral musculussacrospinalis iliac flap approach. There were 8 males and 5 females, aged from 22 to 57 years, with an average age of 35 years. The disease duration ranged from 2 to 19 months, with an average of 6.7 months. According to the American Spinal Injury Association (ASIA) classification criteria, the patients were graded as grade B in 2 cases, grade C in 4 cases, grade D in 5 cases, and grade E in 2 cases before operation. The preoperative Oswestry disability index (ODI) was 36.4±5.7; the preoperative lumbosacral angle was (20.7±0.7)°; the preoperative erythrocyte sedimentation rate (ESR) was (63.4±8.4) mm/1 h; and the preoperative C-reactive protein (CRP) was (38.8±5.2) mmol/L. The operation time and intraoperative blood loss were recorded. The ODI, ASIA grade, lumbosacral angle, and ESR were recorded at last follow-up. Bridwell criterion was used to judge the interbody fusion.ResultsThe operation time was 150-240 minutes (mean, 190 minutes), and the intraoperative blood loss was 420-850 mL (mean, 610 mL). No major blood vessel, dural sac, nerve root, and lumbosacral plexus injuries occurred during the operation. Delayed wound healing occurred in 3 cases, and primary wound healing achieved in the other patients. No wound infection or sinus formation was found. All 13 patients were followed up 1.5-6.1 years (mean, 2.8 years). During the follow-up period, there was no tubercular symptom, cerebrospinal fluid leakage, loosening and rupture of internal fixator; and no complications such as retrograde ejaculation and erectile dysfunction occurred in 8 male patients. Solid spinal fusion obtained in all patients with the mean fusion time of 6.4 months (range, 4.2-9.9 months); and all iliac osteotomies healed. At last follow-up, the ODI was 7.2±3.5, the lumbosacral angle was (31.2±0.5)°, and ESR was (9.8±2.5) mm/1 h, all of which improved significantly when compared with pre-operative ones (P<0.05). The patients were classified as grade D in 2 cases and grade E in 11 cases, which improved significantly when compared with preoperative ones (Z=–3.168, P=0.002).ConclusionOne-stage posterior surgery via unilateral musculussacrospinalis iliac flap approach in treatment of lumbosacral tuberculosis is effective and safe.

  • 带旋髂深血管蒂髂骨瓣联合空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效分析

    目的 总结带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折的疗效。 方法 2006 年1 月- 2008 年12 月,采用带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗23 例外伤致股骨颈骨折患者。其中男17 例,女6 例;年龄19 ~ 47 岁,平均31.4 岁。骨折按Garden 分型:Ⅱ型3 例,Ⅲ型14 例,Ⅳ型6 例。受伤至手术时间1 ~ 9 d,平均5 d。 结果 术后患者切口均Ⅰ期愈合。23 例均获随访,随访时间13 ~ 36个月,平均15.3 个月。骨折愈合时间9 ~ 15 个月。术后15 个月Harris 评分为73 ~ 92 分,其中优5 例,良16 例,中2 例,优良率91.3%。1 例术后33 个月发现股骨头缺血性坏死(avascular necrosis of femoral head,ANFH),其余患者无ANFH及股骨头晚期塌陷等并发症发生。所有患者无神经损害并发症,无术区感染;1 例术后伴供骨区疼痛,12 个月后疼痛自行消失。 结论 带旋髂深血管蒂髂骨瓣联合松质骨空心加压螺钉内固定治疗青壮年股骨颈骨折手术操作简便、安全,可获得较好疗效。

  • 缝匠肌蒂髂骨瓣及加压螺钉在中青年

    目的 总结切开复位、加压螺钉内固定及缝匠肌蒂髂骨瓣移植术治疗中青年股骨颈骨折患者的临床效果。方法 2003年2月~2005年11月,采用缝匠肌蒂髂骨瓣移植,3根空心加压钛螺钉内固定术,治疗中青年股骨颈骨折患者52例。男37例,女15例;年龄19~52岁,平均46.4岁。车祸伤29例,坠落伤15例,跌伤8例。左侧23例,右侧29例。股骨颈骨折头下型27例,经颈型18例,基底型7例。骨折按Garden分型:Ⅱ型16例,Ⅲ型25例,Ⅳ型11例。伤后至手术时间4h~15d。结果 术后切口均Ⅰ期愈合。39例获随访6~41个月。35例术后23~39周骨折愈合,4例股骨头坏死。疗效评定按粱雨田等标准:优19例,良14例,可2例,差4例。髋关节功能按Harris评分标准:优良33例,平均86.1分;可2例,平均65.0分;差4例,平均51.3分。结论以缝匠肌为蒂的髂骨瓣移植及3枚空心加压钛螺钉内固定,可提供股骨颈骨折后股骨头的血供,有植骨支撑,促进骨折愈合,是手术治疗中青年股骨颈骨折的有效方法之一。
