  • Intra-pulmonary Artery Infusion of Antitumor Necrosis Factor-α Antibody Attenuates Lung Injury after Cardiopu-lmonary Bypass

    Objective To investigate the protective effects of antitumor necrosis factor-α antibody (TNF-αAb) on lung injury after cardiopulmonary bypass (CPB) and their mechanisms. Methods Forty healthy New Zealand white rabbits,weighting 2.0-2.5 kg,male or female,were randomly divided into 4 groups with 10 rabbits in each group. In groupⅠ,the rabbits received CPB and pulmonary arterial perfusion. In group Ⅱ,the rabbits received CPB and pulmonary arterial perfusion with TNF-αAb. In group Ⅲ,the rabbits received CPB only. In group Ⅳ,the rabbits only received sham surgery. Neutrophils count,TNF-α and malondialdehyde (MDA) concentrations of the blood samples from the left and right atrium as well as oxygenation index were examined before and after CPB in the 4 groups. Pathological and ultrastructural changes of the lung tissues were observed under light and electron microscopes. Lung water content,TNF-α mRNA and apoptoticindex of the lung tissues were measured at different time points. Results Compared with group Ⅳ,after CPB,the rabbitsin group Ⅰ to group Ⅲ showed significantly higher blood levels of neutrophils count,TNF-α and MDA(P<0.05),higherTNF-α mRNA expression,apoptosis index and water content of the lung tissues (P<0.05),and significantly lower oxyg-enation index (P<0.05) as well as considerable pathomorphological changes in the lung tissues. Compared with group Ⅱ,after CPB,the rabbits in groups Ⅰ and Ⅲ had significantly higher blood concentrations of TNF-α (5 minutes after aortic declamping,220.43±16.44 pg/ml vs.185.27±11.78 pg/ml,P<0.05;249.99±14.09 pg/ml vs.185.27±11.78 pg/ml,P<0.05),significantly higher apoptosis index (at the time of CPB termination,60.7‰±13.09‰ vs. 37.9‰±7.78‰,P<0.05;59.6‰±7.74‰ vs. 37.9‰±7.78‰,P<0.05),significantly higher blood levels of neutrophils count and MDA (P<0.05),significantly higher TNF-α mRNA expression and water content of the lung tissues (P<0.05),and significantly loweroxygenation index (P<0.05) as well as considerable pathomorphological changes in the lung tissues. Compared with groupⅠ,rabbits in group Ⅲ had significantly higher above parameters (P<0.05) but lower oxygenation index (P<0.05) only at 30 minutes after the start of CPB. Conclusion Pulmonary artery perfusion with TNF-αAb can significantly attenuate inflammatory lung injury and apoptosis of the lung tissues during CPB.

  • THE RECONSTRUCTION OF LARGE BONY DEFECT IN UPPER LIMB AFTER RESECTION OF TUMOR

    Seven cases with bone tumor in upper limb were reported. Five cases were treated by using free vascularized fibular graft, 2 cases by using fusion between humorus and clavicle. A follow-up study of six patients showed that the graft bone was united within 3 months in 5 cases, in 6 months in one case. Partial function of upper limb in 6 patients have been restored.

  • Significance of p16 and Rb Gene Protein Expression in Primary Gallbladder Carcinoma

    Objective To study the expression of p16 and Rb gene in primary gallbladder carcinoma (PGC). MethodsFiftysix cases of PGC and 26 of gallbladder benign lesions were observed by immunohistochemistry (SP). ResultsThe positive rates of p16 and Rb protein in PGC were 46.3% and 71.4% respectively. No association of Rb expression with histological grade, histological type, invasion, metastasis and prognosis was found. There were close correlation between p16 expression and invasion, metastasis as well as prognosis. The inverse correlation of p16 and Rb gene expression in PGC was confirmed. Conclusion The loss of p16 or Rb gene protein is an important molecular event in PGC,and closely related to the block in the negative regulatory pathway of all cell cycle. The detection of p16 protein may be useful in evaluating biological characteristics and prognosis in PGC.

  • TREATMENT OF PROXIMAL HUMERAL DEFECT DUE TO BONE TUMOR BY USE OF NONINTERNAL FIXATION FIBULARAUTOGRAFT

    Objective To study the reparative and reconstructive for proximal humerus defect due to the excision of bone tumor with noninternal fixation non-vascularised fibular autografts. Methods From June 1991 toDecember 2003, 26 non-vascularised fibular grafts were used as substitutes for repair and reconstruction after resection for bone tumors on proximal humerus. Fifteen cases were given curettage and fibular supporting internal fixation, the other 11 cases were given tumor resection and joint reconstruction with proximal fibular graft. The age ranged from 6 to 41 years. Out of 26 patients, 5 had giant cell tumor, 9 had bone cysts, 8 had fibrous dysplasia and 4 had enchondroma. Results Twenty-six patients were followed up from 1 to 12 years (3.4 years on average). Local recurrence was found in 2 cases, and 1 of them died of lung metastasis. Both outlook and function of the reconstructed joints have good results in 15 proximal humeral joint surface reserved cases. Of them, 3 children gained normal shoulder function 3 weeks after operation. Part function were obtained in the other 11 fibular grafts substituted proximal humeral defect. Conclusion Non-vascularised fibular grafts is an appropriate treatment option for proximal humerus bone defect due to excision of bone tumor.

  • Relation between disulfidptosis-related genes and prognosis or immunotherapy of pancreatic cancer: based on bioinformatics analysis

    ObjectiveTo investigate the relation between disulfidptosis-related genes (DRGs) and prognosis or immunotherapy response of patients with pancreatic cancer (PC). MethodsThe transcriptome data, somatic mutation data, and corresponding clinical information of the patients with PC in The Cancer Genome Atlas (TCGA) were downloaded. The DRGs mutated in the PC were screened out from the 15 known DRGs. The DRGs subtypes were identified by consensus clustering algorithm, and then the relation between the identified DRGs subtypes and the prognosis of patients with PC, immune cell infiltration or functional enrichment pathway was analyzed. Further, a risk score was calculated according to the DRGs gene expression level, and the patients were categorized into high-risk and low-risk groups based on the mean value of the risk score. The risk score and overall survival of the patients with high-risk and low-risk were compared. Finally, the relation between the risk score and (or) tumor mutation burden (TMB) and the prognosis of patients with PC was assessed. ResultsThe transcriptome data and corresponding clinical information of the 177 patients with PC were downloaded from TCGA, including 161 patients with somatic mutation data. A total of 10 mutated DRGs were screened out. Two DRGs subtypes were identified, namely subtype A and subtype B. The overall survival of PC patients with subtype A was better than that of patients with subtype B (χ2=8.316, P=0.003). The abundance of immune cell infiltration in the PC patients with subtype A was higher and mainly enriched in the metabolic and conduction related pathways as compaired with the patients with subtype B. The mean risk score of 177 patients with PC was 1.921, including 157 cases in the high-risk group and 20 cases in the low-risk group. The risk score of patients with subtype B was higher than that of patients with subtype A (t=14.031, P<0.001). The overall survival of the low-risk group was better than that of the high-risk group (χ2=17.058, P<0.001), and the TMB value of the PC patients with high-risk was higher than that of the PC patients with low-risk (t=5.642, P=0.014). The mean TMB of 161 patients with somatic mutation data was 2.767, including 128 cases in the high-TMB group and 33 cases in the low-TMB group. The overall survival of patients in the high-TMB group was worse than that of patients in the low-TMB group (χ2=7.425, P=0.006). ConclusionDRGs are closely related to the prognosis and immunotherapy response of patients with PC, and targeted treatment of DRGs might potentially provide a new idea for the diagnosis and treatment of PC.

  • THE ADVANCE OF MARKERS FOR THYROID NEOPLASM

    Objective To evaluate the markers which contribute to diagnosis and prognosis of thyroid neoplasm. Methods The references about thyroid markers in recent years were reviewed. Results CD26 and galectin-3 could be regarded as a simple, potent markers to differentiate thyroid carcinoma in preoperative diagnosis, CD97 was a specific marker for undifferentiated thyroid carcinoma and its metastasis, CD15 and telomerase could be used in fine-needle aspirate biopsy (FNAB) of thyroid mass, and to improve its diagnostic evaluation, RET/PTC was mainly expressed in thyroid medullary carcinoma, oncofetal fibronectin (oncFN) was specific to papillary and anaplastic carcinoma, thyroid peroxidase was used to identify benign and malignant thyroid tumor. Conclusion Although there are a lot of markers for thyroid neoplasm, but there is no marker which are completely specific to certain histotype of thyroid neoplasm at present.

  • Expression of Thromhospondin-1 in Gastric Cancer and Metastatic Lymph Node Tissues and Its Relationship to Tumor Angiogenesis

    Objective To detect the expression of thromhospondin-1 (TSP-1) in gastric cancer and metastaticlymph node tissues, and to study its relationship of TSP-1 to clinicopathologic parameters or tumor angiogenesis. Methods The TSP-1 and vascular endothelial growth factor (VEGF) expressions and microvessel density (MVD) were evaluated by immunohistochemistry in 72 specimens obtained by gastric resection from patients with gastric cancer, including corres-ponding adjacent normal gastric mucosa tissues (distant from cancer ≥5 cm) and lymph nodes surrounding cancer. A semiquantitative scoring system was used for evaluating the staining. The relationship of TSP-1 to VEGF expression, MVD, or clinicopathologic parameters was analyzed. Results ① TSP-1 positive expression rate was 45.8% (33/72) in the primary gastric cancer tissues, 90.3% (65/72) in the corresponding adjacent normal gastric mucosa tissues, and 50.8% (30/59) in the metastatic lymph nodes tissues. The expressions of TSP-1 in the primary gastric cancer tissues and metastatic lymph nodes tissues were significantly lower than those in the adjacent normal gastric mucosa tissues (χ2=32.710,P=0.000;χ2=25.298, P=0.000). The expression of TSP-1 had no statistical significance in the primary gastric cancer tissues as compared with in the metastatic lymph nodes tissues (χ2=0.327, P=0.568). ② The expression of TSP-1 in the metastatic lymph nodes tissues was significantly lower than that in the non-metastatic lymph nodes tissues (Z=-2.573, P=0.010). ③The expression of TSP-1 in the primary gastric cancer tissues and metastatic lymph nodes tissues suggested a negative correlation with VEGF (rs=-0.309, P=0.008;rs=-0.269, P=0.040) and MVD (rs=-0.348, P=0.003;rs=-0.272, P=0.037). Conclusions TSP-1 expression is down-regulated and has a negative correlation with VEGF and MVD in the primary gastric cancer and the metastatic lymph nodes tissues. According to the present results, it seems likely that TSP-1 is a tumor angiogenesis inhibitor.

  • Clinical Analysis of Laparoscopic Radical Resection on 20 Patients with Rectal Cancer in Primary Hospital

    目的 总结基层医院初期开展腹腔镜直肠癌根治术的临床经验。 方法 回顾分析2008年10月-2009年10月收治的20例腹腔镜直肠癌手术的临床资料。 结果 16例顺利完成手术,4例中转开腹,1例发生吻合口瘘。随访2~10个月,均无操作孔种植及肿瘤复发。 结论 采用手术者已习惯的手术路径,电刀、超声刀相结合游离直肠肠管,直视下裸化和用凯途闭合肠管,缩短了手术学习曲线,降低了手术成本,有助于在基层医院推广。

  • Laparoscopic Resection for Colorectal Neoplasms (Report of 18 Cases )

    Objective To explore the safety, effectiveness, operation mode and clinical value of the laparoscopic colorectal resection. Methods The clinical data and experiences of laparoscopic resection for 18 cases with colorectal neoplasm from Jun. 2007 to Mar. 2008 were studied retrospectively. Results Among 18 cases, there were 5 cases of rectal cancer, 6 cases of sigmoid colon carcinoma, 2 cases of sigmoid colonic polyp, 2 cases of descending colon carcinoma, 2 cases of ascending colon carcinoma and 1 case of ascending colonic lipoma. Fifteen cases of laparoscopic colorectal resection were performed successfully, including Dixon procedure 4 cases, Miles operation 1 case, radical resection of sigmoid colon 5 cases, palliative resection of sigmoid colon 2 cases, left hemicolectomy 2 cases and right hemicolectomy 1 case. Three cases converted to laparotomy due to adiposity or advanced status of local disease. Average intraoperative blood loss was 110 ml. The average number of lymph nodes dissected was 13.5. It took about 40 hours to restore intestinal function. The average time of hospitalization was 9 days. No one died during operation and no complications such as anastomotic leakage and postoperative hemorrhage occurred. Conclusion Laparoscopic resection for colorectal neoplasms possesses less trauma and rapid postoperative recovery. Laparoscopic colorectal surgery is safe and effective with skill and indication.

    目的 了解血液科护士在对患者抗肿瘤药物治疗护理过程中的防护现状。 方法 2009年12月-2010年3月对血液科一、二病区和骨髓移植层流病房的护理人员,采用便利抽样方式进行问卷调查。 结果 血液科化学疗法防护现状与国际水平有差距,护理人员虽有一定的防护意识,但仍较薄弱。 结论 加强护理人员化学治疗药物毒性及防护知识的教育,提高防范意识,并制定严格的防护措施及管理制度是确保护理人员职业安全的根本与途径。
