  • STUDY ON THE EFFECT OF COMPOSITE OF BASIC FIBROBLAST GROWTH FACTOR AND PARTIALLY DEPROTEINIZED BONE ON THE REPAIR OF FEMORAL HEAD DEFECTS

    Objective To evaluate the effect of composite (bFGF/PDPB) of basic fibroblast growth factor(bFGF) and partially deproteinized bone (PDPB) on the repair of femoral head defect. Methods Forty-eight femoral heads with defect derived from 24 New Zealand rabbits were divided into 3 groups at random, which were implanted with bFGF/PDPB(group A), PDPB(group B) and nothing(group C) respectively.The rabbits were sacrificed at 2,4,and8 weeks after operation, and then the femoral heads were obtained. The specimens injected with Chinese ink were created. Then X-ray examination, histopathological and morphological examination of blood vessel, and image analysis were made. Results The bone defects healed completely 8 weeks after operation in group A. The implants in the repaired tissue were not substituted completely in group B. The bone defects did not heal completely in group C. Two weeks after operation, affluent newly formed vessels were seen in repaired areas in groupA. No significant difference between group A and group B was observed 8 weeks after operation. In group C, newly formed vessels were scarce 2, 4, and 8 weeks after operation. There were 3 sides rated excellent, 2 good and 1 fair in group A; 1 excellent, 2 good, 2 fair and 1 poor in group B; and 1 fair and 5 poor in group C according to the X-ray evaluation 8 weeks after operation. Eight weeks after operation, the volume fraction of bone trabecula in repaired tissue was higher in group A than that in group B (Plt;0.05), and the fraction in group C was thelowest among the 3 groups (Plt;0.05). Conclusion The composite ofbFGF and PDPB can effectively promote the repair of femoral head defect of rabbit. 

  • TREATMENT OF OSTEONECROSIS OF FEMORAL HEAD WITH FREE VASCULARIZED FIBULA GRAFTING

    Objective To evaluate the effect of the treatment of necrosis of femoral head with the free vascularized fibula grafting. Methods From October 2000 to February 2002, 31 hips in 26 patients with ischemic necrosis of the femoral head were treated with free vascularized fibula graft. Among these patients, 21 patients (25 hips) were followed up for 6-18 months(12 months on average). According to Steinberg stage:Ⅱ period, 5 hips;Ⅲ period,8 hips; Ⅳ period, 12 hips.Results Among 25hips, their Harris Hip Score at all satges were improved during the follow-up. The symptom of pain diminished or disappeared after operation. The patient’s ability to work and live was notlimited or only slightly limited during the follow-up. Radiographic evaluation showed that most femoral heads improved (18 hips) or unchanged (6 hips) and only oneworsened.Conclusion The free vascularized fibular grafting is a valuable method for femoral head necrosis. With this method, we can prevent or delay the process of the disease.

  • 人工双极股骨头置换术治疗老年股骨颈骨折

    【摘要】目的观察人工双极股骨头置换术治疗老年股骨颈骨折的临床疗效。方法2004年4月2007年10月,采用人工双极股骨头置换术治疗21例老年股骨颈骨折。男8例,女13例;年龄68~80岁,平均年龄726岁。致伤原因:摔伤16例,车祸伤5例。骨折类型:头下型11例,经颈型8例,基底型2例。合并骨质疏松17例,内科疾病31例。伤后至入院时间2~22 h,平均16 h。结果平均手术时间70 min,平均输血量300 mL,手术后引流量平均40 mL。手术后切口均I期愈合。并发尿路感染3例,肺部感染1例,均经抗生素治疗后痊愈。21例均获随访,随访时间6个月~4年,平均28年。X线片检查示髋关节间隙正常。末次随访时按Harris评分标准评定髋关节功能:优14例,良5例,可1例,差1例,优良率90%。结论人工双极股骨头置换术治疗老年股骨颈骨折具有能及时解除患者疼痛、恢复关节功能、防止并发症等优点。

  • AN EXPERIMENTAL STUDY ON OSTEOCYTE APOPTOSIS IN STEROID INDUCED EARLY OSTEONECROSIS OF FEMORAL HEAD

    Objective To explore the significance of osteocyte apoptosis in steroidinduced osteonecrosis of the femoral head. Methods SixtyNew Zealand rabbits were divided into experimental group and control group(n=30). The experimental group was given 10 ml/kg of horse serum intravenously 2 times at 2 weeks intervals and an intraperitoneal injection of 45 ml/kg·d of methylprednisolone acetate for 3 days;the control group was given equal isotonic Na chloride. Osteocyteapoptosis was observe by means of TUNEL. Results The number of apoptosis in the experimental group(112.33‰±26.12‰) was significantly higher than that in the control(47.01‰±22.95‰) (Plt;0.01)in the 4th week. With time, osteocytes apoptosis progressively increased. In the 6thand 8th weeks, the percentage of empty osteocyte lacunae in the experimental group (17.23%±3.44%, 28.56%±3.45%) was significantly higher than that in the control group (11.29%±2.89%,11.26%±2.75%,Plt;0.05). The transmission electron microscope showed that the characteristics of osteocyte apoptosisincluded intact nuclear membrane,comdensed chromatin and increased electron dense. Conclusion Osteocytes apoptosis may play a key role in the process of steroidinduced early osteonecrosis of the femoral head.

  • TRANSPOSITION OF MUSCULAR SKELETAL FLAP PEDICLED WITH STRAIGHT HEAD OF RECTUS FEMORIS FOR TREATMENT OF AVASCULAR NECROSIS OF FEMORAL HEAD

    OBJECTIVE: To investigate the clinical results of transposition of muscular skeletal flap pedicled with straight head of rectus femoris for treatment of avascular necrosis of adult femoral head. METHODS: Eight patients with avascular necrosis of femoral head were adopted in this study. There were 6 males and 2 females, the ages were ranged from 24 to 56 years. According to the criteria of Ficat, there were 5 cases in stage II and 3 cases in stage III. The Smith-Peterson incision was used to expose the capsule of the hip. After complete curettage of the necrotic bone from the femoral head, the muscular skeletal flap pedicled with straight head of rectus femoris was resected and transposited into femoral head. Finally, conventional decompression of head was performed. RESULTS: All the cases were followed up for 1 to 3 years. There were excellent results in 5 cases, good in 2 cases and moderate in 1 case. The rate of excellent and good results were 87.5%. CONCLUSION: Comparing with other pedicled bony flaps, the muscular skeletal flap pedicled with straight head of rectus femoris is characterized by its convenience and efficacy. It is suitable for the treatment of avascular necrosis of femoral head in stage II or III, but the contour of the femoral head should be nearly normal.

  • THE CLINICAL EFFECT OF BIPOLAR LONGSTEM PROSTHETIC REPLACEMENT ON THE TREATMENT OF COMMINUTED INTERTROCHANTERIC FRACTURE OF HIP IN THE ELDERLY OSTEOPOROTIC PATIENTS

    Objective To evaluate the clinical effect of bipolar long-stem prosthetic replacement on the treatment of comminuted intertrochanteric fracture of hip in the elderly osteoporotic patients. Methods From March 2000 to August 2003, 18 patients who were more than 72 years old with comminuted intertrochanteric fracture were treated with bipolar longstem prosthetic replacement. There were 8 males and 10 females (aged from 72 to 91). Fractured fragments of large trochanter and minor trochanter should be preserved during the operation so that the fragments can be fixed with steel wires and insertion of artificial femoral head.The surgical approach and operative technique, the average course of treatment in hospital, the average time to ambulate with full-weight bearing on the operated limb after the operation, complication, and mortality were observed. Results All patients were followed up 6 to 28 months(16.2 months on average). The average course of stay at hospital was 16 days. The average time to ambulate was 32 days. The incidence of the pain of the hip joint was one-eighteenth, and one patient due to another disease was unable to walk without using twocrutches one month after the surgery. The mortality of the patients was oneeighteenth one year after the surgery. Conclusion Bipolar longstem prosthetic replacement for the treament of comminuted intertrochanteric fractures in the elderly osteoporotic patients proves to be a suitable alternative.Patients have better prognosis, early full-weight bearing, rapid rehabilitation, and fewer complications.

  • 一期人工全髋关节置换治疗髋臼骨折合并股骨头颈骨折

    目的  总结髋臼骨折切开复位内固定、一期人工全髋关节置换治疗髋臼骨折合并股骨头或颈骨折的临床疗效。  方法   2005 年 1 月- 2008 年 12 月,采用髋臼骨折切开复位内固定、一期人工全髋关节置换治疗髋臼骨折合并股骨头、颈骨折 6 例。男 5 例,女 1 例;年龄 45 ~ 65 岁。高处坠落伤 2 例,车祸伤 4 例。均为新鲜闭合骨折。受伤至入院时间为 2 h ~ 2 d。其中 2 例合并股骨颈头下型骨折,4 例合并髋关节后脱位及股骨头骨折,2 例合并颅脑损伤。   结 果  手术时间50~90 min,术中失血量400~800 mL,术中输压积红细胞2~4 U。术后切口均Ⅰ期愈合,无感染、血栓形成等并发症发生。5 例患者获随访,随访时间 9 ~ 36 个月,平均 20 个月。髋臼骨折于术后 8 ~ 16 周达骨性愈合,无感染及假体松动发生。末次随访髋关节功能根据 Harris 评分为 75 ~ 95 分,获优 1 例,良 2 例,中 2 例。  结论  一期髋臼骨折内固定、人工全髋关节置换治疗髋臼骨折脱位合并股骨头、颈骨折,可减少股骨头缺血性坏死、创伤性关节炎等并发症,避免二次手术,缩短住院时间。

  • RELATIONSHIP BETWEEN ALCOHOL INDUCED OSTEONECROSIS OF FEMORAL HEAD AND SINGLENUCLEOTIDE POLYMORPHISMS OF METHYLENE TETRAHYDROFOLATE REDUCTASE 677 C/T

    Objective To explore the relationship between alcohol induced osteonecrosis of the femoral head (ONFH) and the single nucleotide polymorphisms (SNP) of methylene tetrahydrofolate reductase (MTHFR) 677 C/T. Methods From July 2005 to May 2008, eighty-nine male patients with alcohol induced ONFH were selected as the patient group, aged from 24 to58 years old (mean 44.3 years old). The time of drinking was about 17 years, 375 mL/day. The imaging evidence showed ONFH with no other history associated to ONFH. Seventy-seven male healthy adults were selected as the control group, aged from 23 to 52 years old (mean 42.7 years old). The time of drinking was about 14 years, 335 mL/day. The imaging evidence showed no ONFH. The 2 mL blood sample was acquired from every subject. DNA was purified from leucocyte at first, then was ampl icated by PCR, the product of PCR was sequenced at last. The SNP of MTHFR 677 C/T was analyzed with SPSS 12.0 software package. Results The TT genotype and T allele frequencies of MTHFR 677 C/T were 27.2% and 52.0% (P gt; 0.05) in the control group, and the distribution of genotype was consistent with Hardy-Weinberg equil ibrium. The genotype frequencies of CC, CT and TT were 23.4% (18 cases), 49.4% (38 cases) and 27.2% (21 cases) in the control group, were 14.6% (13 cases), 36.0% (32 cases) and 49.4% (44 cases) in the patient group; showing statistically significant differences (P lt; 0.05). The allele frequencies of C and T were 48.0% (74) and 52.0% (80) in the control group, and were 32.6% (58) and 67.4% (120) in the patient group; showing statistically significant differences (P lt; 0.05). The frequencies of C, T alleles and TT genotype were higher in the patient group than in the control group, showing statistically significant differences (P lt; 0.05). The odds ratios were 0.523, 1.914 and 2.607, respectively; the 95% confidence interval were 0.335-0.816, 1.226-2.987, 1.359-5.001, respectively. Conclusion The relationship may exist between the SNP of MTHFR 677 C/T and alcohol induced ONFH.

  • TREATMENT OF EARLY AVASCULAR NECROSIS OF FEMORAL HEAD BY CORE DECOMPRESSION COMBINED WITH AUTOLOGOUS BONE MARROW MESENCHYMAL STEM CELLS TRANSPLANTATION

    Objective To compare the cl inical outcomes of the core decompression combined with autologous bone marrow mesenchymal stem cells (BMSCs) transplantation with the isolated core decompression for the treatment of earlyavascular necrosis of the femoral head (ANFH). Methods From May 2006 to October 2008, 8 patients (16 hips) with earlyANFH were treated. There were 7 males and 1 female with an average age of 35.7 years (range, 19-43 years). According to the system of the Association Research Circulation Osseous (ARCO): 4 hips were classified as stage II a, 2 as stage II b, 1 as stage II c, and 1 as stage III a in group A; 2 hips were classified as stage II a, 2 as stage II b, 3 as stage II c, and 1 as stage III a in group B. The average disease course was 1.1 years (range, 4 months to 2 years). The patients were randomly divided into 2 groups according to left or right side: group A, only the core decompression was used; group B, both the core decompression and autologous BMSCs transplantation were used. The Harris score and visual analogue scale (VAS) score were determined, imaging evaluation was carried out by X-rays and MRI pre- and post-operatively. The erythrocyte sedimentation rate, C-reactive protein, l iver function, renal function, and immunoglobul in were detected for safety evaluation. Results All incisions healed by first intention. Eight patients were followed up 12-42 months (23.5 months on average). The cl inical symptoms of pain and claudication were gradually improved. The Harris scores and VAS scores of all patients were increased significantly at 3, 6, and 12 months after operation (P lt; 0.05). There was no significant difference between groups A and B 3 and 6 months after operation (P gt; 0.05), but there was significant difference between groups A and B 12 months after operation (P lt; 0.05). The necrosis area of femoral head in groups A and B were 18.13% ± 2.59% and 13.25% ± 2.12%, respectively, showing significant difference (P lt; 0.05). In group A, femoral head collapsed 12 months after operation in 1 case of stage III. No compl ication of fever, local infectionoccurred. Conclusion The core decompression and the core decompression combined with BMSCs transplantation are both effective for the treatment of early ANFH. The core decompression combined with BMSCs transplantation is better than core decompression in the rel ief of pain and postponing head collapse.
