Objective To summarize the application and advancement of liver transplantation for hepatic metastasis from neuroendocrine tumor. Methods Domestic and overseas publications on the study of liver transplantation for hepatic metastasis from neuroendocrine tumor in recent years were collected and reviewed. Results Liver transplantation can offer good relief of symptoms, long disease-free intervals, and potential cure in individual patients with hepatic metastatic tumor. Important selection criteria are well-differentiated tumors and a low proliferation rate (Ki67＜10%). Conclusion In carefully selected patients with metastatic neuroendocrine tumors, liver transplantation is an appropriate option.
Objective To observe the changes of blood biochemistry during orthotopic liver transplantation in pigs. Methods Fourteen healthy pigs were anesthetized and intubated, the right carotic artery was cannulated for blood pressure monitoring and blood biochemical sampling. The right internal jugular vein was cannulated for central venous pressure monitoring. No exogenous glucose was administrated during operation, the donor liver was perfused with Collin’s solution. During the anhepatic stage, the veno-venous bypass was established, blood was pumped from the portal and femoral veins using an extracorporeal circuit, and returned to the axillary vein. The arterial blood was sampled for monitoring arterial blood gas tensions, Na＋,K＋,Ca2＋, and blood glucose levels.Results During preanhepatic stage, blood glucose levels increased progressively, blood electrolytes and acidbase balance were stable. During anhepatic stage, pH was decreased, base deficit was increased significantly, blood glucose levels were significantly decreased, blood electrolytes were stable. During neohepatic stage, acidosis became more severe, blood potassium increased abruptly on reperfusin of the grafted liver, blood glucose was higher than that in prehepatic and anhepatic stage. Conclusion During operation of liver transplantation, metabolic acidosis developed progressively, blood glucose decreased in anhepatic stage, blood glucose and potassium increased significantly on reperfusion of the grafted liver.
Objective To introduce the imaging modalities used for the evaluation of postoperative complications of orthotopic liver transplantation (OLT) and to present the imaging findings of these complications. Methods The literatures related to the imaging methods and imaging manifestations of OLT complications were reviewed. Results Ultrasound was the initial imaging technique used for the detection of complications in the early postoperative period. Spiral CT and MRI yielded more accurate and comprehensive evaluation of postoperative complications in later stage. So far, there had been no specific imaging findings to suggest rejection reaction. The spectrum of imaging manifestations of OLT complications, such as vascular complications, biliary complications, liver parenchymal complications, and so on, were summarized and illustrated. Conclusion Imaging examination (especially ultrasound, spiral CT and MRI ) plays an important role in the evaluation of postoperative complications of OLT.
肝脏移植作为终末期肝病的治疗，自上个世纪80年代在欧美国家获得公认以来，已在世界各国得到迅速开展。我国自90年代后期以来，在全国掀起了第二个肝移植的热潮，迄今已完成1 000余例肝移植，在围手术期处理、手术技术、介入放射、移植免疫、抗感染治疗等各个方面均获得丰富的经验，我国肝移植的效果及长期生存率亦逐步赶上国际先进水平。总的来讲，适合进行肝移植的病种可分为良性及恶性肝病，鉴于恶性肝病行肝移植后复发率高，长期生存率低，因而其作为肝移植的指征长期以来存在争议，而良性终末期肝病则是肝移植的主要指征。我院自1999年2月以来连续施行肝移植114例，其中良性肝病为69例，占60.5%。本文仅针对良性肝病肝移植的一些特点谈谈我们的经验和体会。
【Abstract】ObjectiveTo establish the stable model of orthtopic liver transplantation in rats.MethodsIn the light of Kamada’s method, the donor’s liver was perfused through portal vein before it was harvested,and the anastomosis was modified as continous suture with one suture for the suprahepatic inferior vena cava. Two hundred and ten orthotopic liver transplantations were performed in Wistar male rats according to this method. ResultsThe mean time of donor operation was 35 min, and that of recipient operation was 51 min. The mean cold preserving time of graft was 60 min.The anhepatic phase was about 17 min 〔(17.6±4.5) min〕. Nineteen rats died during operation. The causes of death included: bleeding of suprahepatic inferior vena cava, infrahepatic vena cava thrombosis, biliary obstruction, portal vein constriction and thrombosis, liver injury, bleeding of left subphrenic vein, infection, excessively deep anesthesia and respiratory failure. The 24hour survival was 91.0%(191/210),and the oneweek survival was 85.2%（179/210）.ConclusionThrough the modification of the anastomosis of suprahepatic inferior vena cava, the nonhepatic time of the recipients could be shortened,and the complications could be decreased. In this way, the survival of recipient rats after liver transplantation could be increased.
【Abstract】 Objective To study the effects of ischemic preconditioning (IP) on the activity of nuclear factor-κB （NF-κB） and the expressions of TNF-α and intercellular adhesion molecule-1 (ICAM-1) during early reperfusion following liver transplantation in rats. Methods The models of rat orthotopic liver transplantation were established. The donor livers were stored for 2 hours in Ringers solution at 4 ℃ before transplantation. All rats were randomly divided into sham operation group （SO group）， control group and IP group. IP group was achieved by clamping the portal vein and hepatic artery of donor liver for 10 minutes followed by reperfusion for 10 minutes before harvesting. The activity of NF-κB and expressions of TNF-α and ICAM-1 at 1 h, 2 h, 4 h and 6 h after reperfusion were measured. Serum ALT, LDH were also determined. Results The liver function of recipients with IP were significantly improved. Compared with SO group， the graft NF-κB activity increased after transplantation in control group and IP group (P＜0.05), while compared with control group that was significantly attenuated at 1 h and 2 h in IP group. Similarly, hepatic levels of TNF-α and ICAM-1 were significantly elevated in control group and were reduced in IP group. Conclusion IP might down-regulated TNF-α and ICAM-1 expression in the grafts after orthotopic liver transplantation through depressed NF-κB activation， and attenuate neutrophil infiltration in the grafts after reperfusion.
通过对1999年1月至2007年8月，肝移植病人术后门诊复查的情况做一总结，通过护理干预，使病人复查得到极大方便，并拓展了护理工作领域，提高了门诊护理工作的质量，并提升了医院的形象。
自1963年美国Starzl施行世界第1例人体原位肝移植以来，历经40年的发展，随着对手术适应证及手术时机选择的新认识，手术方式的改进，安全有效的免疫抑制药物的开发及其它肝移植围手术期治疗的综合进展，肝移植已在全世界步入成熟时期。我国70年代末曾掀起了肝移植的第一个高潮，80年代处于停滞阶段，至90年代又掀起了第二个高潮，但直到近3～4年，才开始取得较好疗效，跻身于国际先进行列。现拟从肝移植指征、手术技术改进及免疫抑制药物的发展三方面来谈谈肝移植近年的重大进展。
