Testing Treatments is a book to help the public understand how to validate the efficacy of testing treatments and the possible bias and error in clinical trial, as well as to call for help to promote good study thus to improve the quality of health care. No matter for the first or the second edition, this book is very popular around the world, and its second edition has been translated into more than ten languages. To help the readers understand the content of the book, we established a website (www.testingtreatments.org) and other sibling sites in different languages. The website not only provided the full-texts to download, but also collected various popular science resources (videos, audios and cartoons) to help the readers assimilate more knowledge. The editors of all the different language websites have established an TTi Editorial Alliance to share experience and provide each other with mutual support, thus to promote health professionals, patients and public around the world to use reliable research to inform their health decisions.
Objective To study the distribution of HIV/AIDS high-risk population, HIV infection and the main risk factors for developing HIV/AIDS’ controllable measures and exploring appropriate health education and behavior intervention models. Methods A total of 360 commercial sex workers (CSW) joined together through convenience sampling and 360 drug users (DU) joined together through convenience sampling or snow-balling sampling whose relevant behavior factors were investigated by questionnaires. Results The general rate of knowing knowledge about AIDS was 75.2% among 360 CSW, 67.8% CSW used condom in commercial sex activities; none of 149 CSW blood samples was detected HIV or syphilis antibody positive. The general rate of knowing knowledge about AIDS was 83.7% among 360 DU who injected drugs last month, the rate of sharing needles was 47.6% and the low rate of condom used; 1 HIV antibody and 5 syphilis antibodies positive were found among 198 DU blood samples, so HIV and syphilis infection rate were 0.51%and 2.53%, respectively. Conclusion The rate of HIV infection is a very low level and there are many risk factors among CSW and DU. A good job should be done to integrate AIDS health education with behavioral intervention and the monitoring system for the AIDS/HIV high-risk population should be improved.
ObjectiveTo study the examinees' cognitive level of the X-ray radiation, and to provide references for popularizing radiation protection knowledge. MethodWe chose patients who were checked in the Department of Radiology in our hospital between August 30 and October 29, 2013 as our study subjects, and carried out investigation by anonymous questionnaires. The investigation content included the respondents' basic information, ways to obtain the radiation knowledge, how well the patients knew about the fundamental radiation knowledge and their attitudes towards radiological examination and so on. A total of 2 315 patients were investigated, and 2 315 questionnaires were issued. Finally all questionnaires were retreated. ResultsAmong the 2 315 patients, 2026 (87.52%) knew the radiation symbol and 1 821 (78.66%) knew that X-ray radiation was harmful to the human body. Different age, gender, occupation, educational level and previous X-ray examination made significant differences in the fundamental knowledge on X-ray (P<0.05) . Attitudes towards X-ray examination was significantly different due to different age, occupation, and educational level (P<0.05) . Most examinees got the X-ray fundamental radiation knowledge from the publicity and medical staff in hospitals. ConclusionsTo make the examinees keep right attitudes and behaviors in radiographic examination, further effective methods of popularizing the basic knowledge of radiation should be carried out. Those publicity and medical staff in hospitals are the main force in the popularization of radiation knowledge.
Objective To explore the effectiveness of situational experiential teaching mode in emergency internship teaching. Methods Interns from the Department of Emergency, Jiangyou Fifth People’s Hospital from July 2022 to May 2023 were selected as the research subjects. The interns were randomly divided into a trial group and a control group using a random number table method. The trial group adopted a situational experiential teaching mode, while the control group adopted a traditional teaching mode. Theoretical knowledge testing, clinical comprehensive ability assessment, and clinical information feedback were used to evaluate the effectiveness of different teaching methods. Results A total of 90 interns were included, with 45 people in each group, aged 18-23 years old. Both groups consist of 18 clinical medicine students and 27 clinical nursing students. There was no statistically significant difference in academic performance in school between the two groups of interns (P>0.05). The theoretical knowledge test score (92.98±2.71 vs. 85.29±6.24), clinical comprehensive ability assessment score (90.52±2.58 vs. 83.35±5.25) and clinical feedback (44 excellent and 1 fine in the trial group vs. 25 excellent, 5 fine, and 15 poor in the control group) of the trial group were better than those in the control group (P<0.05). Conclusions The situational experiential teaching mode can enhance interns’ learning interest, improve memory effectiveness, help students master theoretical knowledge, and enhance their comprehensive abilities in clinical evaluation and decision-making. It is worth promoting in clinical practice.
Objective To investigate the current status of self-management knowledge, attitude and behavior and influencing factors in patients with chronic obstructive pulmonary disease (COPD) in Qianbei area, and to analyze the influencing factors. Methods A total of 329 COPD patients were recruited and investigated with self-compiled questionnaire for self-management knowledge, attitude and behavior of chronic obstructive pulmonary disease, and SPSS 17.0 software was used to analyze the data. Results The mean score of self-management knowledge, attitude and behavior in COPD patients was 132.6±17.0, in which the scores of each dimension from high to low were as follows: smoking, medication treatment, exercise and family oxygen therapy. The analysis of stepwise regression indicated that the main influencing factors of self-management of COPD patients were age, sex, address, degree of education, course of disease and smoking. Conclusions The self-management level of the disease recognition, attitude and behavior in COPD patients should be improved, in which the knowledge is key factor to promote the health behavior of patients. So the healthy workers should pay close attention to the influence factors and apply the new thinking pattern and methods to improve the self-management ability and the quality of life of patients.
As evidence-based practice (EBP) continues to be valued, the guideline implementation has become an important field for research and practice in health care. In order to better guide the guideline implementation in the field of health care, this paper combines the EBP experience gained from several EBP programs carried out in our hospital those years, in aim of exploring and sharing the methodology of guideline implementation in domestic clinical settings.
The Clearinghouse plays an important role in knowledge translation in western developed countries. It consistently provides the latest evidence to target customers according to its strict criteria in the specific fields. Clearinghouse is objectively and scientifically designed to assess the scientific nature and the practical applicability of evidence to provide the best current evidence to different service practitioners. It emphasizes much on the process from "knowledge production" to "knowledge utilization", which effectively fills the gap between scientific research and practice. It will play an increasing important role in the knowledge translation in the future. This article specifically introduces its history of development, characteristics and application and sets the construction of Chinese Clearinghouse for Evidence Translation in Child & Aging Health (CCET) as an example, fully elaborating the application status of Clearinghouse and providing assistance for evidence informed decision making.
ObjectiveTo explore the application and effect of clinical nursing pathway based on information-knowledge-attitude-practice (IKPA) theory in percutaneous renal needle biopsy.MethodsThis is a historical control study. A total of 460 patients who underwent percutaneous renal biopsy in West China Hospital of Sichuan University between April and December 2020 were prospectively recruited as the pathway group, and a clinical nursing pathway based on IKAP theory was implemented. By contrast, the data of 617 patients who underwent percutaneous renal biopsy and received routine care in the same hospital between April and December 2019 were retrospectively collected as the control group. The length of hospital stay, hospitalization expenses, the incidences of postoperative complications (macroscopic haematuria, perirenal hematoma, and acute urinary retention), the incidence of postoperative postural hypotension, and the management enrollment rate of chronic kidney disease patients were compared between the two groups.ResultsThe length of hospital stay [median (lower quartile, upper quartile): 8 (7, 11) vs. 8 (7, 12) d] and the hospitalization expenses [median (lower quartile, upper quartile): 7380.50 (6401.86, 8789.21) vs. 8167.00 (6816.50, 10044.50) yuan] were less in the pathway group than those in the control group, the incidences of postoperative macroscopic haematuria (1.52% vs. 6.97%) and perirenal hematoma (2.61% vs. 5.02%) were lower in the pathway group than those in the control group, the management enrollment rate of chronic kidney disease patients was higher in the pathway group than that in the control group (26.09% vs. 6.16%), and the differences between the two groups were statistically significant (P<0.05). The differences in the incidences of acute urinary retention (8.26% vs. 11.18%) and postoperative postural hypotension (0.00% vs. 0.81%) between the two groups were not statistically significant (P>0.05).ConclusionThe application of clinical nursing pathway in percutaneous renal needle biopsy can effectively reduce the length of hospital stay and hospitalization expenses, and improve the management enrollment rate of chronic kidney disease patients.
ObjectiveBased on the method of bibliometrics, to visually analyze the current research status of DNA polymerase epsilon catalytic/DNA polymerase delta catalytic subunit (POLE/POLD1) mutations in colorectal cancer, and further to explore its current hot spots and to look forward to future development trends.MethodsUsed the Web of Science database as the data source to retrieve the published related literatures in the 30 years from the establishment of the database to September 2020, and used VOSviewer 1.6.11 and CiteSpace 5.7 software to analyze the the distribution of research countries/institutions, authors and keywords in the included published literatures. And to perform cluster analysis, time evolution analysis and Burst analysis on keywords, draw corresponding visual maps and analyzed them.ResultsA total of 299 published articles were included. Bibliometric analysis showed that the application of POLE/POLD1 mutations in colorectal cancer had developed steadily in recent years. After 2012, the number of published studies had increased significantly; the journals with the most articles were Familial Cancer and Proceedings of the National Academy of Sciences of the United States of America. The author with the highest number of articles was Tomlinson I. The analysis of countries and regions showed that the United States was the main country for research in this field and had contributed the most to the amount of publications. After analyzed the academic institutions/universities where the researchers were located, it could be seen that the University of Oxford was the research university/institution with the most publications. Keyword cluster analysis and time evolution analysis showed that the research hotspots in this field focus on the impact of this gene mutation on immunotherapy. Burst analysis showed that the current published research in this field started two years after publication and continues to be highly cited.ConclusionsPOLE/POLD1 mutation has undoubtedly become a hot area of current research in colorectal cancer research. Among them, tumor immunotherapy is the focus of research in this field.
目的 调查临床护士对静脉输液治疗相关知识的知晓度,分析存在的问题,以便有针对性地进行专业指导和培训。 方法 2011年8月,自制静脉输液治疗相关知识知晓度的调查问卷,利用护士参加医院集体培训的机会对158名临床护士进行问卷调查。 结果 ① 70.3%的护士静脉输液治疗知识主要来源于医院培训,护士对静脉输液治疗知识的内涵认识欠全面。② 44.2%护士对留置针的规范化固定及冲、封管方法了解不充分,仅约1/3的护士了解留置针A-C-L维护法,规范维护管理知识掌握不充分。③ 护士对静脉输液常见并发症认识欠深入,认为常见并发症发生的主要原因与患者的血管有关(静脉炎占78.5%,渗出占83.5%,导管堵塞占81.0%)。④ 9%的护士发生针刺伤后不能正确处理,护士对针刺伤原因认识清晰,处理方法还需加强培训。 结论 临床护士对静脉输液治疗相关知识的掌握欠全面性、系统性和专业性,需要对护士采取多种形式、多种途径的专业指导和培训,才能使临床护士全面掌握静脉输液治疗相关知识,促进静脉输液治疗规范化和专业化,有效保障护理质量和安全。
