  • PREVENTION AND TREATMENT OF POSTOPERATIVE COMPLICATIONS FOLLOWING SKIN SOFT TISSUE EXPANSION

    Since 1987, One hundred and fifty-four patients suffered from alopecia, neck and facial scar, and nasal defect had been treated with skin soft tissue expansion. The incidence of complication was decreased markedly, compared to previons report which was 11.7%. Two cases of this group were given up this procedure. The lessous learned from these case were as following. Strictly evaluated the case according to the indication, examined the expander carefully, improved the techniques to inbed the expander and infilled the sailine, those of which could obtain satisfactory result.

  • EXPERIMENTAL STUDY ON PERMEABILITY OF SKIN EXPANDERS TO METRONIDAZOLE AND PROCAINE

    An in vitro experiment showed that the skin expanders were permeable to metronidazole and procaine. Twenty kidney shaped skin expanders were divided into four groups. Group 1. 100ml 0.2% metronidazole solution was injected into the expanders and the expenders were immersed in a flask filled with 100ml saline solution, and then were placed in a hermetically sealed glass chamber; Group 2.the whole procedure was the same as that of Group 1 except the expander was previously boiled in water for 30 minutes; Group 3. 100 ml 2% procaine was injected instead of metronidazole, other step was the same as that ofgroup I; and Group 4. the whole procedure was the same as that of Group 2 except the solutioninjected was 2% procaine. The concentration of metronidazole and procaine in the surrounding saline was measured at 1st, 2nd, 4th, 16th, 24th, 48th, 72nd 120th hours. The rate of diffusion of a drug was highest at 2 and 4 hous. The rate of diffusion was inversely proportional to its molecular weight, i.e., the smaller the molecular weight the greater the permeability. In view of this, during the process of expansion, metronidazole and procaine would diffuse out of the expander which might be beneficial for preventing infection and controlling pain.

  • APPLICATION OF SKIN AND SOFT TISSUE EXPANSION IN TREATMENT OF BURN INJURY

    OBJECTIVE: To evaluate the application of skin and soft tissue expansion in the treatment of deformity due to extensive severe burn injury and repair of severe deep electrical burned scalp and skull with fresh wound. METHODS: From 1988, 83 cases of application of skin and soft tissue expansion were reported. In those patients with deformity due to severe burn of large area and with whole nasal defect, soft tissue expander was used under the forehead skin graft and venter frontalis, followed by reconstruction of nose with the expanded vascularized skin flap and carved cartilago costalis as nasal frame. In patients of severe deep electrical burned scalp and skull with fresh wound, skin and soft tissue expansion were used to repair the wound simultaneously with scalp burn alopecia, anesthetics and antibiotics injected into the extracapsular space of the expander in case of pain and infection. RESULTS: All of the cases were successfully treated with little pain and minimized infection. CONCLUSION: Skin and soft tissue expansion in a safe and reliable measure in the treatment of deformity due to extensive severe burn injury and repair of severe deep electrical burned scalp and skull with fresh wound.

  • THE PROBLEMS IN THE USE OF SUPERFICIAL CERVICAL ARTERY SKIN FLAP AND IT’S RELEVANT SOLUTION

    Superficial cervical artery skin flap is widely used in clinical practice. In order to inprove the outcome of the flap in clinic, eleven cases of skin defect of scalp who were treated with the flap was discussed. After operation, the donor area healed but there was no hair growth on recipient area. Among them, six cases occurred partial necrosis of skin flaps. In order to avoid these problen, the relevant solution discussed as follows: 1. Handle well the pedicle of the skin flap to prevent the interference with venous returm. 2. Adhere strictly to indications. 3. Apply skin expander to obtain "extra" skin, then carryout the tranfer of skin flap and 4. Better use the skin flap with residual hair.

  • APPLICATION OF VACUUM SEALING DRAINAGE TO TREAT LATE-STAGE LARGE SKIN AVULSION INJURYWITH INFECTION

    Objective To investigate the cl inical effect of vacuum seal ing drainage (VSD) on late-stage large skin avulsion injury with infection. Methods From May 2007 to August 2008, 9 patients with large-area skin avulsion injury and infection were treated. There were 1 male and 8 females aged 9-52 years old (median 27 years old). All patients suffered from closed skin avulsion injury involving the lower back, buttock, and part of the thigh. The injury area varied from 30 cm × 25 cm to92 cm × 38 cm. The time between injury and hospital admission was 15-23 days. The skin avulsion injury was compl icated with pelvis fracture, urethral injury, anal injury, sacrum exposure, and l imb fractures. The interval between hospital admission and operation was 3-23 hours. Free spl it-thickness skin graft was performed after the focus debridement and three VSD treatments (40-60 kPa). Results After three VSD treatments, no patient had general pyemia and severe local tissue necrosis or infection, the tissue edema in the skin avulsion area was alleviated obviously, and all the wound cavities were closed. All the wounds in the graft site healed after 28-45 days of treatment (average 39 days), and all the donor sites healed. Nine patients were followed up for 4-14 months (average 10 months). The appearance of the reparative area was good, and there was no occurrence of joint dysfunction in the injured area due to scar contracture. Conclusion VSD is effective in treating late-stage large skin avulsion injury with infection.

  • EFFECT OF CALCIUM ON RECONSTRUCTION OF BASEMENT MEMBRANE IN COMPOSITECHITOSAN TISSUE ENGINEERING SKIN

    Objective To search the most suitable concentration of calcium in the medium for the basement membrane reconstruction in tissue engineering skin in vitro. Methods Composite chitosan tissue engineering skin was prepared according to previous studies. Four groups were included according to the concentrationof calcium (1.00, 1.45, 1.65 and 1.95 mmol/L respectively). After 7 days and 15 days of culture, the histological manifestation of basement membrane in tissue engineering skin was observed by hematoxylin amp; eosin staining and PAS staining, and collagen Ⅳ of basement membrane was detected immunohistochemicallyatthe dermalepidermal junction. Results This tissue engineering skin shared some histological features of normal skin, including a welldifferentiated stratifiedepidermis and a dense dermis. The epithelium in the group of 1.95 mmol/L calcium differentiated better than those in other groups. PAS staining showing a regularly red dying strap domain at the dermal-epidermal junction. Collagen Ⅳ was positively stained immunohistochemically at the dermalepidermal junction inthe tissue engineering skin. Conclusion The above results suggest that the medium with 1.95 mmol/L calcium should be suitable for the growth of composite chitosan tissue engineering skin and the reconstruction of basement membrane.
