  • The Effect of UltraShort Wave on the Healing of Wound after Operation for Hemorrhoids

    摘要:目的:观察超短波治疗对痔术后创面愈合的影响。方法:将100例混合痔术后患者分为治疗组和对照组各40例,治疗组于术后24小时给予超短波治疗和复方紫草油纱条换药,对照组仅给以复方紫草油纱条换药,观察两组创面愈合时间和创面上皮生长速度。结果:治疗组较对照组创面愈合时间更短(Plt;0.01),创面上皮生长速度更快(Plt;0.01)。结论〗:超短波治疗能够加速痔术后创面愈合时间,减少痛苦,疗效确切安全。Abstract: Objective: To observe the clinical efficacy of ultrashort wave on the healing of wound after operation for hemorrhoids. Methods: One hundred cases of disease subjected to operation were divided into the treatment group (50 cases) and the control group (50 cases).The treatment group had been given ultrashort wave 24 hours after operation and Fufangzicaoyousa ointment gauze. The control group had been give Fufangzicaoyousa ointment gauze. Results: The results showed that the woundhealing time was much shorter in the treatment group than in the control group (Plt;0.01), the epidermis growth was much faster in the treatment group than in he control group (Plt;0.01). Conclusion: Ultrashort wave can promote the healing of wound after the operation for hemorrhoids and relieve pain, and it can be externally used safely.

  • Metronidazole for Treatign the Post Hemorrhoidectomy Pain: A Systematic Review

    Objectives To evaluate the effect of metrinidazole treatment after conventional hemorrhoidectomy pain in patients with third and fourth degree hemorrhoids. Methods We searched the Cochrane Library (Issue 1 2009), PubMed (1966 to March 2009), EMbase (1974 to March 2009), SCI (1974 to March 2009), CBM (1978 to March 2009), CNKI (1994 to March 2009), and VIP (1989 to March 2009) to identify randomized controlled trials or quasi- randomize controlled trials of metronidazole versus placebo for treating post hemorrhoidectomy pain. We evaluated the quality of the included studies by using the Handbook 4.2.6 recommend standards and analyzed data using the Cochrane Collaboration’s RevMan 4.2.10. Results We included seven randomized controlled trials or quais-randomized controlled trials (n=553). Meta-analyses showed that there were statistical differences between metronidazole and placebo in pain after hemorrhoidectomy and the use of an additional dose of analgesia. Conclusions The current evidence shows that metronidazole relieves the pain after conventional hemorrhoidectomy and reduces the additional used of analgesics. Further high quality, large sample randomized controlled trials should be carried out.

  • Problems Concerning the Treatment of Hemorrhoids from PPH

    目的:总结吻合器痔上黏膜环切术(PPH)治疗痔病的经验。探讨该术式的有关问题,以便提高疗效。方法:回顾性分析了2001~2006年8月收治痔病541例,其中PPH术42例的临床资料。本组均为Ⅲ度或Ⅳ度痔。局部合并症共22例次:血栓外痔愈合后遗留皮赘14例,肛裂2例。有关全身合并症:前列腺增生7例,慢性泌尿系统感染1例。结果:42例中,35例术后2~5天出院,6例自愿留院到7天,1例慢性尿路感染者术后2~7天多次便血,再次手术后28日痊愈出院。随访13~24个月:32例恢复满意,10例有肛门包块感及包块排粪时轻度脱出,其中3例经进一步处理缓解,另7例观察治疗。结论:(1)PPH手术简单易行,疼痛轻,住院时间短,效果好。(2)为了提高效果术中应注意:①禁忌症为单个痔块脱出和肛管皮肤不平滑并纤维化的Ⅳ度病变者外。我们还发现慢性泌尿系统感染未彻底治愈者也应视为相对禁忌。明显前列腺增生者也应慎重。②术中按经典PPH手术的要求进行操作,扩肛器插入前后,不宜扩肛和下牵痔块。③个别巨大痔块及皮赘性痔块(tag)宜先行切除,再行本术。④重度脱出者,黏膜切除应宽大,必要时切除部分肛垫。

  • Analysis of the effect and postoperative recurrence of grade Ⅳ mixed hemorrhoids treated by C-shaped mucosal resection and anastomosis above the dentate line

    ObjectiveTo investigate the curative effect of C-shaped mucosal resection and anastomosis above the dentate line in the treatment of mixed hemorrhoids and its effect on anal function. MethodsA total of 78 patients with degree Ⅳ mixed hemorrhoids treated in Nanjing Liuhe District People’s Hospital from June 2015 to February 2018 were retrospectively collected. The patients were divided into control group (n=39) and observation group (n=39) according to treatment methods. Patients of the control group received traditional procedure for prolapse and hemorrhoids operation, while patients of the observation group received C-shaped mucosal resection and anastomosis above the dentate line. The perioperative indexes (operation time, intraoperative blood loss, hospital stay, etc.), subjective function evaluation indexes (Wexner constipation score, Kirwan grade, etc.), clinical efficacy and recurrence rate were compared between the two groups. The random walking model was used to evaluate the clinical curative effect. ResultsThe intraoperative blood loss [(27.9±3.4) mL vs. (43.2±5.2) mL, P<0.001], 24 h visual analogue scale score [(4.2±1.5) points vs. (5.6±1.5) points, P<0.001], duration of first defecation pain [(22.1±3.2) min vs. (34.2±5.0) min, P<0.001], the time of carrying blood [(4.1±0.4) d vs. (5.7±0.6) d, P<0.001], and the time of edema [(3.2±0.6) d vs. (4.7±0.9) d, P<0.001] in the observation group were shorter (lower) than those in the control group. The difference between pre-and post-operation of Wexner constipation score [(13.2±2.4) points vs. (11.7±2.1) points, P=0.004], resting pressure [(23.1±4.9) mmHg vs. (17.8±3.4) mmHg, P<0.001] and maximum squeeze pressure [(33.5±7.3) mmHg vs. (23.1±5.6) mmHg, P<0.001] in the observation group were significantly higher than those in the control group. There was a long-term correlation between changes in random fluctuating power rate values of Wexner constipation score, Kirwan grade, rectoanal inhibitory reflex positive rate, resting pressure, maximum squeeze pressure and the surgical procedure received by the patient of the two groups. The total effective rate [97.4% (38/39) vs. 66.7% (26/39)] and non-recurrence rate [92.3% (36/39) vs. 76.9% (29/39)] in the observation group were higher than those in the control group, while there was no significant difference in the incidence of total complications between the two groups [5.1% (2/39) vs. 12.8% (5/39), P=0.235)]. ConclusionCompared with PPH, C-shaped mucosal resection and anastomosis above the dentate line for the treatment of degree Ⅳ mixed hemorrhoids can improve the therapeutic effect, reduce postoperative recurrence, maintain anal function and facilitate the recovery of patients.

  • Treatment of four degree mixed hemorrhoids by ultrasonic scalpel flap-retentiong surgery in combination with automatic hemorrhoid ligation and Milligan-Morgan hemorrhoidectomy: a randomized controlled study

    ObjectiveTo observe the therapeutic efficacy of ultrasonic scalpel flap-retentiong surgery combined with automatic hemorrhoid ligation for the treatment of four degree mixed hemorrhoids. MethodsThe patients with four degree mixed hemorrhoids admitted to the Third Affiliated Hospital of Henan University of Traditional Chinese Medicine from January 2023 to June 2023 were included as the study subjects, then were randomly averagely divided into an observation group (underwent ultrasonic scalpel flap-retentiong surgery combined with automatic hemorrhoid ligation) and a control group (underwent Milligan-Morgan hemorrhoidectomy) by a random number table method. The therapeutic efficacy, total hospitalization time, operation time, intraoperative blood loss, wound healing time, hospitalization costs, postoperative pain score, bleeding score, wound edema score, postoperative complications (anal stenosis, urinary retention, anal incontinence), and recurrence were compared between the two groups. ResultsA total of 100 patients with four degree mixed hemorrhoids treated in this hospital were included, with 50 patients in each group. There were no statistically significant differences in the general information such as the gender, age, disease course, body mass index, hematochezia, and tumor prolapse between the two groups (P>0.05). ① The overall therapeutic efficacy both the observation group and the control group was well (100% versus 96.0%, Fisher exact test, P=0.495). ② The observation group had less total hospitalization time, operation time, intraoperative blood loss, hospitalization costs, and wound healing time as compared with the control group (P<0.05). ③ The differences in the interaction effects of the time-group for the points of postoperative pain, bleeding, and wound edema were not statistically significant (χ2=6.668, P=0.083; χ2=5.500, P=0.139; χ2=6.204, P=0.102). However, the main effects of group (χ2=5.073, P=0.024; χ2=7.107, P=0.008; χ2=8.857, P=0.003) and time (χ2=188.526, P<0.001; χ2=212.472, P<0.001; χ2=185.110, P<0.001) showed statistically significant differences. The points of postoperative pain, bleeding, and wound edema in the observation group were statistically lower than thoes in the control group (χ2=9.323, P=0.002; χ2=18.133, P<0.001; χ2=6.232, P=0.013). ④ The incidences of postoperative urinary retention and anal stenosis in the observation group were lower than those in the control group (P=0.046 and 0.031, respectively), there were no statistically significant differences in the incidences of incontinence and postoperative recurrence between the two groups (P>0.05). ConclusionFrom the preliminary research results of this study, ultrasonic scalpel flap-retentiong surgery in combination with automatic hemorrhoid ligation has a better efficacy in treating four degree mixed hemorrhoids, with a smaller wound, faster healing, higher safety, and fewer complications.

  • The Efficacy of Cicatrisation-fumigant Chinese Herbal at Different Concentrations on Patients after Hemorrhoidectomy

    ObjectiveTo observe the efficacy of "Fuchuang Xunxi Formula" at different concentrations on patients after mixed hemorrhoidectomy. MethodsTwo hundred and ten patients having undergone mixed hemorrhoidectomy between October and November 2013 were randomized into seven groups, with 30 patients in each group. Group A was treated with 6% "Fuchuang Xunxi Formula"; Group B was treated with 6% "Zhiji Fumigation Formula"; Group C was treated with 4% "Fuchuang Xunxi Formula"; Group D was treated with 4% "Zhiji Fumigation Formula"; Group E was treated with 3% "Fuchuang Xunxi Formula"; Group F was treated with 3% "Zhiji Fumigation Formula"; And Group G was treated with branch water. Each group was fumigated at 45℃ for five minutes, then hip bath at 40℃ for five minutes with a course of 10 days. The curative effect of pain of wound, edema, effusion, urinary disturbance were observed. ResultsThe therapeutic effect in group C was the best (P<0.05), and the relief of postoperative complications (pain, edema, effusion) was better than the other groups (P<0.05). ConclusionTreatment with 4% "Fuchuang Xunxi Formula" for postoperative complications is the most effective.

  • A comparative study on the clinical efficacy and anorectal dynamics of two combined operations in the treatment of mixed hemorrhoids

    ObjectiveTo study the curative effect and postoperative anorectal dynamics change of tissue-selecting therapy stapler (TST) and procedure for prolapse and hemorrhoids (PPH) respectively combined with mixed Milligan-Morgan and lauromacrgol injection in the treatment of Ⅲ–Ⅳ degree mixed hemorrhoid.MethodsClinical data of 158 patients with Ⅲ–Ⅳdegree mixed hemorrhoid who received operation in the Department of Colorectal and Anal Surgery, Chaoyang Central Hospital, from May 2016 to March 2018 were analyzed retrospectively, the observation group (TST+Milligan-Morgan+lauromacrgol injection, 80 cases) and control group (PPH+Milligan-Morgan+lauromacrgol injection, 78 cases). The clinical efficacy, adverse reactions, postoperative complications and recurrence of the two groups were observed, and the changes of anorectal dynamic indexes before and after operation were observed.ResultsPostoperative symptoms of mixed hemorrhoid prolapse could be alleviated by 100% in both the observation group and the control group, and the relief rate of hematochezia was 93.8% and 92.3%, respectively, and the effective rate of 1 year after surgery was 97.5% and 94.9%, respectively. Comparison between the two groups showed no statistical difference (P>0.05). The operative time, intraoperative blood loss, VAS pain score and hospital stay of the observation group were all better than those of the control group (P<0.05). In terms of postoperative massive hemorrhage and anastomotic stenosis, although the incidence rate of the control group was higher than that of the observation group (3.8% vs. 1.3% and 2.6% vs. 0.0%, respectively), there was no statistical significance in the two groups (P>0.05). Postoperative anal drop, stool urgency, postoperative urinary retention and postoperative stimulation of anal papilla hypertrophy and proliferation complications were significantly better in the observation group than in the control group (P<0.05). The difference of maximum anal systolic pressure (MASP) in the two groups of patients between before and after surgery and the comparison of MASP results between the two groups after surgery showed no statistical differences (P>0.05). The differences of resting anal sphincter pressure (RASP) and rectal sensory threshold volume (RSTV) between before and after operation were not statistically significant (P>0.05). The difference of rectal maximum threshold volume (RMTV) value and the comparison of RASP, RSTV and RMTV value between the two groups after surgery showed that the observation group were superior to the control group and the differences were statistically significant (P<0.05).ConclusionsTST or PPH combined with Milligan-Morgan and lauromacrgol injection both are effective and minimally invasive methods for the treatment of Ⅲ–Ⅳ degree mixed hemorrhoid. However, the observation group has shorter operative time and hospital stay, less intraoperative blood loss, and lower postoperative pain score. What is more important is that the postoperative complications are less and the anal function is protected to the maximum extent, and the quality of life of patients after operation is greatly improved.

  • Application of Parecoxib Sodium on Patients Undergoing Procedure for Prolapse and Hemorrhoids for Preemptive Analgesia

    目的 观察帕瑞昔布钠超前镇痛在痔上黏膜环切术中对丙泊酚半数有效效应室靶浓度和对术后视觉模拟评分法(VAS) 的影响。 方法 2010年3月-2011年10月择期手术患者60例,随机分为帕瑞昔布组(试验组)和生理盐水组(对照组),每组各30例,分别于术前10 min静脉注射帕瑞昔布钠40 mg或生理盐水2 mL。痔上黏膜环切时靶控输注丙泊酚,其靶控浓度按序贯法确定,相邻靶浓度之间对数差为0.05。观察两组丙泊酚的有效浓度及术后2、4、8、24 h的VAS评分。 结果 试验组的半数有效效应室靶控浓度(4.30 μg/mL)低于对照组(4.95 μg/mL),差异有统计学意义(P<0.05)。术后4、8、24 h试验组VAS评分明显低于对照组,差异有统计学意义(P<0.05)。 结论 帕瑞昔布钠可以减少术中丙泊酚的用量,并可得到满意的术后镇痛效果。

  • 音乐疗法对混合痔外剥内扎术后疼痛的影响

    目的探讨音乐疗法对混合痔外剥内扎术后患者疼痛的影响。 方法2012年10月-2013年3月采用随机数字表法将120例混合痔外剥内扎术后患者随机分成两组,每组60例。对照组只接受常规护理;试验组除给予常规护理外,加以音乐疗法,2次/d(晨起和睡前),45 min/次。于术后24 h及48 h,采用视觉模拟评分法(VAS)评估两组患者的疼痛程度并进行比较。 结果术后24 h及48 h,试验组VAS评分均明显低于对照组,差异有统计学意义(P<0.05)。 结论音乐疗法能降低混合痔外剥内扎术后患者对疼痛的敏感性,缓解其疼痛,值得临床推广。

  • PPH and Milligan-Morgan Treatment on 200 Cases with Mixed Hemorrhoid

    目的 比较吻合器痔上黏膜环切术(PPH)与传统痔切除术(Milligan-Morgan,MM)治疗混合痔的临床疗效。方法 200例混合痔患者中行PPH和MM治疗各100例,比较2组患者的疗效及并发症。结果 PPH在疗效及术后并发症发生方面与MM组比较,差异均无统计学意义(P>0.05)。结论 在治疗混合痔方面PPH不优于MM。
