  • Vitreous surgery for severe ocular trauma

    Objective To summarize the visual outcome of patients with severe ocular trauma treated with vitreous surgery. Methods Clinical data of 188(191 eyes) with severe ocular trauma treated with vitreous surgery in a period from November 1996 to April 1998 were analysed retrospectively. Results The study included penetrating injury in 56 eyes, foreign bodies in the posterior segment in 70 eyes, blunt injury in 41 eyes , and globe rupture in 24 eyes. Main complications included endophthalmitis in 35 eyes, choroidal bleeding in 20 eyes, retinal detachment in 60 eyes, and vitreous hemorrhage in 97 eyes. Post-opera-tively, out of 188 eyes, except for 3 of patients too young to examine, visual acuity improved in 133(70.7%), including 85(45.2%) with visal acuity 0.02-1.0, 46(24.5%) remained unchanged; and 9(4.8%) had worse vision. Among 34 with no-light-perception, 12 had light-perception or over. Conclusion A majority of severe trauma eyes can be salvaged with considerable visual recovery after adequate and timely vitreous surgery. (Chin J Ocul Fundus Dis,1999,15:4-6)

  • Curative effects of vitreoctomy and scleral buckling on retinal multiple-tear detachment associated with tracted anterior flap

    Objective To evaluate the curative effects of vitreoctomy or simple scleral buckling on retinal multiple-tear detachment associated with tracted anterior flap. Methods The clinical data of 89 eyes in 89 patients with retinal multiple-tear detachment associated with tracted anterior flap diagnosed in Jan, 1999-Jan, 2002 were retrospectively analyzed. In the 89 patients, 41 had undergone vitreoctomy and 48 had undergone scleral buckling without vitrectomy. In the duration of 2- to 36-month follow-up with the mean of (11.02±7.90) months, visual acuity, retinal reattached rate and postoperative complication were examined and the results in the 2 groups were compared. Results In 41 eyes underwent vitreocotmy, successful reattachment was found in 38 (92.7% ); visual acuity increased in 33 (80.5%), didn′t change in 6 (14.6%), and decreased in 2 (4.9%); leakage of flocculent membrane in anterior chamber occured was found in 2 (4.9%), complicated cataract in 3 (7.3%),and severe proliferative vitreoretinopathy (PVR) in 3 (7.3%). In 48 eyes underwent scleral buckling, 41 (85.4%) had success reattachment; visual acuity increased in 36 (75.0%), didn′t change in 4 (8.3%), and decreased in 8 (16.7%); leakage of flocculent membrane in anterior chamber was found in 6 (12.5%), complicated cataract in 9 (18.8 %), and severe PVR in 8 (16.7%). Conclusion There isn′t any difference of the success rate of the surgery between vitrectomy and scleral buckling for retinal multiple-tear detachment associated with tracted anterior flap.The better visual acuity and less complications are found in the vitrectomy gro up than those in the scleral buckling group.   • Experimental study on targeting transfer of reporter gene in proliferative vitreoretinopathy of rabbit

    Purpose To study the possibility of prevention of proliferative vitreoretinopathy(PVR) by transduction of exogenous gene in vivo. Methods PVR model of rabbits was induced by intravitreal injection of fibroblasts.beta;-galactosidase (lacZ) gene as a reporter gene was transfered into the vitreous of PVR model eyes mediated by retroviral vector, and the expression of the gene in eye tissues was determined . Gene transfection was done on the 6th day after fibroblasts injection,and the dosage of intravitreal injection of reporter gene was 0.1ml PLXSN/lacZ serum-free supernatant (1.1times;106 cfu/ml). Results lacZ gene expression was seen in proliferative membranes after gene transfection, and the expression was located maily at the surface of PVR membrane.The reporter gene expression lasted at least more than 30 days.No expression was found in retinal tissues. Conclusions Retrovirus mediated gene can be directionally transducted in PVR membrane,and might possess the feasibility of gene therapy for PVR. (Chin J Ocul Fundus Dis, 2001,17:224-226)

  • To improve the macular function and visual quality after vitreoretinal surgeries

    Surgical treatments for macular hole and rhegmatogenous retinal detachment are the most common and principle procedures for vitreoretinal specialists. The surgical success rate reached 95.0% and above for vitrectomy, macular surgeries with ILM peeling, or local/total scleral buckling. However, the postoperative visual function recovery is nowhere near good enough. Specialists must pay more attention to the visual function recovery of those patients. Postoperative macular anatomical and functional rehabilitation for macular hole and scleral buckling procedures need a long period of time. At present, the postoperative visual acuity for macular hole depends on many factors, such as macular hole closure conditions, surgical procedures, microsurgical invasive ways, skills of membrane peeling, usage of dye staining, and tamponade material choice. It also depends on residual subretinal fluid under macular area for patients received scleral buckling. It is important for us to investigate these factors affecting recovery of macular anatomy and function, and thus develop some drugs to improve the macular function recovery.

  • Prognosis of idiopathic and traumatic macular holes treated by pars plana vitrectomy

      Objective To evaluate and compare the prognosis of idiopathic macular holes (IMH) and traumatic macular holes (TMH) treated by pars plana vitrectomy (PPV).Methods The clinical data of 72 IMH eyes and 55 TMH eyes, which were treated by PPV between November 2001 and December 2007, were retrospectively reviewed. The visual outcomes and macular anatomic closure were evaluated, and their relationships with prognostic factors including the size of macular hole (MH), preoperative visual acuity (VA) and duration of disease were analyzed.Results The closure rate of IMH (100.0%) was significant higher than that of TMH (85.5%) (P=0.001). The postoperative VA of IMH and TMH were (0.25plusmn;0.02) and (0.21plusmn;0.21) respectively,both significantly increased compare to their preoperative VA (t=-6.841,-4.093; P=0.000). VAincreased IMH and TMH eyes had same VA (chi;2=3.651,P=0.07). PrePPV VAge;0.1 IMH eyes had better outcomes than PrePPV VA<0.1 IMH eyes (chi;2=12.04, P=0.001), while PrePPV VA had no effects on TMH outcomes (chi;2=0.371,P=0.486). IMH eyes with small holes had better outcomes (t=2.476,P=0.016), and TMH eyes with small holes had better closure (t=-4.042, P<0.001). The duration of disease had no significant influence on TMH visual (chi;2=0.704, P=0.401) and anatomic (chi;2=0.166, P=0.684) outcomes. Conclusions PPV is an effective treatment for MH. The closure rate of IMH is higher than that of TMH. The diameter of MH and preoperative VA are major factors for IMH outcomes, and the duration of disease and preoperative VA have no effects on postoperative VA in TMH.

  • Research progress on postural management of patients with retinal detachment undergoing vitrectomy combined with intraocular filling

    Pars plana vitrectomy (PPV) combined with intraocular tamponade surgery is one of the main surgical methods for treating retinal detachment. Due to the use of filling substances, patients need to maintain specific postures after the operation to promote retinal reattachment and reduce the occurrence of complications. Currently, during the process of performing postural management for such surgical patients, there are problems such as low compliance and insufficient monitoring and management. Clinical medical staff have actively explored ways to improve the quality of positioning management, but no corresponding norms and consensus have been formed. The standards implemented by different places are closely related to the degree of medical experience. The evaluation of the performance of the patient's position by the medical care mainly relies on the patient's self-report or the nurse's inspection record, which lacks continuity and accuracy. In the future, it is necessary to further promote the development and transformation of auxiliary tools, implement scientific monitoring and management of patients with the help of artificial intelligence, formulate individualized plans and dynamically adjust them according to the patient's condition, and ensure the effect and improve patient satisfaction.

  • The Applicative Effect of Posture Nursing by Specially-assigned Person and Psychological Nursing on Patients after Vitrectomy Combined with Intraocular Silicone Oil Tamponade

    ObjectiveTo investigate the effect of posture nursing by specially-assigned person and psychological nursing on patients after vitrectomy combined with intraocular silicone oil tamponade. MethodsFrom February 2012 to February 2013, traditional nursing method or posture nursing by specially-assigned person and psychological nursing was carried out respectively for 140 patients who had undergone vitrectomy and intraocular silicone oil tamponade. Adverse effects and retina reattachment effect were compared between the two nursing methods. ResultsAdverse effects after the surgery were significantly fewer in the experimental group than in the control group (P<0.05). The number of patients with retina reattachment in the experimental group was significantly higher than that in the control group (P<0.05). ConclusionPosture nursing by specially-assigned person and psychological nursing reduce postoperative adverse consequences significantly, and achieve the expected operative effect.
