  • 氟尿嘧啶缓释颗粒植入食管癌瘤床对食管癌根治术后局部复发的影响

    目的 观察氟尿嘧啶缓释颗粒瘤床植入对食管癌根治术后局部复发的疗效和预后影响。 方法 纳入2009年1~12月期间重庆三峡中心医院60例行食管癌根治术患者,按其治疗方法分为试验组和对照组两组,每组30例。试验组男24例、女6例,年龄(62.00±7.70)岁;对照组男23例、女7例,年龄(60.20±8.20)岁。试验组患者术中在瘤床植入氟尿嘧啶缓释颗粒300 mg,对照组不植入任何物质。比较两组患者临床结果差异。 结果 两组患者术后主要并发症发生率差异无统计学意义(P>0.05),试验组术后复发时间较对照组显著延长(P<0.05)、试验组术后复发病灶大小及复发病灶的个数较对照组显著减少(P<0.05);两组患者1年生存率差异无统计学意义,但试验组患者2年及3年生存率显著高于对照组(P<0.05),试验组中位生存时间较对照组显著延长[(29.2±1.9)月 vs. (23.4±1.4)月,P<0.05] 。 结论 食管癌术中瘤床植入氟尿嘧啶缓释颗粒能延缓肿瘤复发时间、提高食管癌患者2年及3年生存率,延长患者中位生存时间,而不增加术后并发症的发生,是一种安全、有效的局部化疗方法。

  • Study on Lymphatic Targeting in Rats of New Dosage Form of 5-Fluorouracil Carbon Nanoparticles

    ObjectiveTo investigate the lymphatic targeting of 5-fluorouracil (5-FU) carbon nanoparticles in rats. Methods5-FU concentration in lymphoid tissue of rats was determined by reversed phase high performance liquid chromatography after intraperitoneal injection of 5-FU carbon nanoparticle and 5-FU ordinary form (20 mg/kg body weight). Results5-FU concentration of lymphoid tissue in the 5-FU carbon nanoparticle group was higher than that in the 5-FU ordinary form group, and could sustain a longer time. Conclusion5-FU carbon nanoparticles injection can improve the drug concentration of target lymphatic organs, also has a good lymphatic targeting

  • AN EFFECT OF 5-FLUOROURACIL ON TENDON ADHESION FORMATION AFTER FLEXOR TENDON REPAIR IN CHICKENS

    Objective To assess an effect of 5-fluorouracil (5-FU) applied topically on the tendon adhesion and the healing process after the flexor tendon repair in Leghorn chickens. Methods Thirtytwo white Leghorn chickens, aged 4 months and weighing 1.5-1.7 kg, were randomly divided into 2 groups: Group A andGroup B, with 16 chickens in each group. The flexor digitorum profundus tendons of the 2nd, 3rd and 4th toes were transected and repaired. The repair site in Group A was given 5-FU in a concentration of 25 mg/ml with a soaked sponge that wascut into pieces 7 mm×20 mm×1 mm in size, and the synovial sheath of the repair site was wrapped with the 5-FU-soaked sponge for 1 min for 4 times. The repair site in Group B was served as a control, with no 5-FU but with the sterile normal saline. At 3 and 6 weeks postoperatively, the repaired tendons and the tendon adhesion formation were examined macroscopically and histologically,and the repaired tendons were tested biomechanically. The tissue blocks from the tendon repair site were examined under the transmission electron microscope. Results At 3 and 6 weeks postoperatively, the macroscopic and histological observation showed that the peritendinous adhesions in Group A were looser when compared with those in Group B. The length of the tendon gliding and the extent of yieldance to exercise were found to be 4.85±1.31 mm, 0.67±0.42 mm and 5.74±1.61 mm, 1.55±0.35 mm respectively at 3 and 6 weeks after operation in Group A,but 2.99±0.51mm,0.24±0.14 mm and 3.65±0.54 mm, 1.22±0.16 mm in Group B.Group A was significantly greater in the abovementioned parameters than Group B (P<0.05).At 3 weeks after operation, the ultimate breaking strength was 20.28±4.92 N in Group A and 21.29±4.88 N in Group B, with no statistically significant difference found between the two groups (P>0.05). At 6 weeks, the ultimate breaking strength was 47.12±6.76 N in Group A but 39.31±7.20 N in Group B, with a significant difference between the two groups (P<0.05). Conclusion 5-fuorouracil, when appliedtopically, can reduce the tendon adhesion, with no inhibition of the intrinsic healing mechanism. It is an ideal treatment strategy to prevent peritendinous adhesion. 

  • Effect of Mesentery Imbedding Chemotherapy on The Healing of Colonic Stoma in Dog

    Objective To investigate the effect of imbedding chemotherapy of sustained release of 5-fluorouracil on the healing of colonic stoma in dog. Methods Twenty-eight adult hybrid dogs were randomly divided into chemotherapy group (n=22) and control group (n=6). The canine sigmoid colon were firstly detached and then anastomosed via median abdominal incision, 200 mg sustained release of 5-fluorouracil was imbedded in the mesentery 1.0-1.5 cm away from colonic stoma in chemotherapy group, whereas the control substance was injected into the dogs in control group. Tissue samples were collected from mesentery and stomas on 3, 5, 7, 10 and 15 days after operation, respectively, in order to observe the healing of stoma. The drug concentrations in the stoma and in the tissues that were 0, 1, 3, 5, 7, 10 and 15 cm away from the imbedding point were also measured by high performance liquid chromatographymethod at different phases. Results The tissues from colonic stoma only showed inflammatory reaction at early stage, with no necrosis and cellular degeneration. It was observed that the stoma healed basically on the tenth day after operation. The drug concentrations in the tissues gradually decreased at the range of 0-15 cm over time, but all of which were higher than the anti-tumor effective concentration (0.10 μg/g). Conclusion The imbedding chemotherapy of sustained release of 5-fluorouracil in mesentery has little effect on the healing of stoma, and it could remain an effective anti-tumor concentration in a period of time.

  • 两种新辅助化疗方案在宫颈癌治疗中的临床应用

    【摘要】目的通过比较两种宫颈癌新辅助化疗方案的疗效及化疗不良反应,以期找到宫颈癌的最佳新辅助化疗方案。方法将68例Ib2~Ⅱb期宫颈癌患者术前随机分为A、B组,接受不同化疗方案化疗2个疗程。A组采用顺铂+多西他赛方案治疗30例,B组采用顺铂+阿霉素+氟尿嘧啶方案治疗38例。患者于化疗后2周左右行宫颈癌根治术。结果两种化疗方案总有效率为765%。两组患者在病灶缩小、症状缓解及不良反应方面比较差异均无统计学意义(Pgt;005)。B组血清鳞状细胞癌抗原下降明显低于A组(Plt;005)。结论新辅助化疗能有效的控制宫颈局部病灶,顺铂+阿霉素+氟尿嘧啶是宫颈癌新辅助化疗较理想的方案。

  • A Meta-analysis of Capacitance Combined with Irinotecan versus Fluorouracil Combined with Irinotecan for Advanced Metastatic Colorectal Cancer

    ObjectiveTo assess the effectiveness and safety of capacitance combined with irinotecan (CAPIRI) versus fluorouracil combined with irinotecan (FOLFIRI) for patients with advanced metastatic colorectal cancer. MethodsDatabases such as Pubmed, Embase, Wanfang data, CNKI, Cochran Library were searched from January 2000 to October 2015. We evaluated the quality of randomized controlled trials (RCTs) and then extracted data from them. RevMan 5.2 software was used to perform the meta-analysis. ResultsEight RCTs studies with 1 634 advanced metastatic colorectal cancer patients were included based on our standard. CAPIRI regimen was equal to FOLFIRI regimen in complete response rate [RR=1.17, 95%CI (0.70, 1.96), P=0.56], overall respond rate [RR=0.90, 95%CI (0.79, 1.03), P=0.12], disease control rate [RR=0.93, 95%CI (0.87, 1.00), P=0.06], median progression-free survival [HR=1.00, 95%CI (0.72, 1.37), P=0.99], and median overall survival [HR=0.94, 95%CI (0.63, 1.40), P=0.77]. For safety, higher incidence rate of grade 3/4 vomiting [RR=1.91, 95%CI (1.13, 3.22), P=0.02], diarrhea [RR=2.84, 95%CI (2.22, 3.63), P<0.000 01], hand-foot syndrome [RR=4.55, 95%CI (2.15, 9.61), P<0.000 1] were confirmed for CAPIRI. The two methods had similar toxicities: nausea [RR=0.77, 95%CI (0.64, 0.93), P=0.005], fatigue [RR=1.19, 95%CI (0.73, 1.94), P=0.47], febrile neutropenia [RR=1.59, 95%CI (0.89, 2.87), P=0.12], anemia [RR=1.74, 95%CI (0.59, 5.18), P=0.32], and leukopenia [RR=0.77, 95%CI (0.64, 0.93), P=0.005]. ConclusionCapecitabine combined with irinotecan treatment for advanced colorectal cancer is effective and its toxicity is acceptable.

  • Efficacy and Safety of Interstitial Chemotherapy with 5-Fluorouracil Sustained Release Agent in Radical Operation of Colorectal Cancer

    ObjectiveTo observe effect and safety of interstitial chemotherapy with 5-fluorouracil sustained release agent in radical operation of colorectal cancer. MethodsOne hundred and sixty patients with colorectal cancer from October 2011 to December 2013 were randomly divided into observation group and control group according to an incomplete random method, 78 cases of them were in the observation group and 82 cases of them were in the control group. All the patients were performed radical resection of colorectal cancer. The abdominal cavity and pelvic cavity were washed after surgery. 5-fluorouracil sustained release agent was implanted in the observation group patients for interstitial chemotherapy, the implant site was at the tumor resection area and the mesenteric artery. Routine chemotherapy was performed in these two groups after operation. The postoperative complications were observed. The postoperative local recurrence rate, liver metastasis rate, and 24-month survival rate were recorded. ResultsThe rates of abdominal complications and toxic effects had no significant differences between these two groups (P > 0.05). The rates of 12-month and 24-month local recurrence and the rate of liver metastasis in the observation group were significant lower than those in the control group[1.3% (1/78) versus 8.5% (7/82), x2=8.934, P=0.023; 5.2% (4/78) versus 23.2% (19/82), x2=14.834, P=0.004; 10.3% (8/78) versus 18.3% (15/82), x2=12.034, P=0.016]. The rate of 24-month survival in the observation group was significant higher than that in the control group[94.9% (74/78) versus 84.1% (69/82), x2=11.465, P=0.010]. ConclusionThe good safety of interstitial chemotherapy with 5-fluorouracil sustained release agent could effectively decrease local recurrence rate and liver metastasis rate of colorectal cancer after radical operation and improve survival time of patients.

  • Clinical Evaluation of Paclitaxel Liposome Combined with Continuous Intravenous Infusion of Large-Dose 5-Fluorouracil as First-Line Chemotherapy for Advanced Gastric Cancer

    ObjectiveTo evaluate the therapeutic effect and adverse reaction of paclitaxel liposome combined with continuous infusion of large-dose 5-fluorouracil(5-fu) in treatment for advance gastric cancer(AGC). MethodsFrom May 2009 to August 2012, 63 consecutive patients with AGC in this hospital were enrolled in this study. All the patients were given chemotherapy including paclitaxel liposome and continuous infusion of large-dose(2.5 g/m2) 5-fu. The efficacy and toxicity of this regimen were observed. ResultsThere was no patient who could not tolerate adverse reaction related to such regimen. Five cases achieved complete response and 31 cases achieved partial response, the overall response rate was 57.1%(36/63). Hematologic toxicity included gradeⅢ/Ⅳleucopenia 8 cases(12.7%) and neutropenia 10 cases(15.9%), while there was no occurrence of gradeⅢ/Ⅳanemia or thrombopenia. Non-hematologic toxicity was fairly mild. ConclusionsPaclitaxel liposome is safe, well tolerated, highly targeted, and has long duration of effect. Paclitaxel liposome combined with continuous infusion of large-dose 5-fu is safe and effective in treatment for patients with AGC.

  • Effect of Genistein Combined with 5-FU on Human Gastric Cancer Cell Line SGC-7901 in Vit ro

    Objective  To study the effect of genistein (Gen) in combination with 5-fluorouracil (5-FU) on human gastric carcinoma cells in vitro. Methods  Human gastric carcinoma cell line SGC-7901 was t reated with Gen and 5-FU alone or in combination for 24 , 48 and 72 hours , respectively. MTT assay was used to investigate the inhibitory effect of Gen and 5-FU on SGC-7901 cells. Transmission electron microscopy was used to observe the ultramicrost ructure changes of cancer cells and the flow cytometry was used to detect the distribution of cell cycle. Results  Gen and 5-FU alone or in combination inhibited the proliferation of SGC-7901 cells significantly in both time-dependent and dose-dependent manner. The combination of Gen with 5-FU had synergistic effect on the growth of SGC-7901 cell line when the inhibition ratio was 20 % to 80 %. When SGC-7901 cells exposed to 18. 8μmol/ L Gen , 8. 84μmol/ L 52FU , and 3. 06μmol/ L Gen combined with 7. 96μmol/ L 5-FU for 72 hours , 62. 97 % , 63. 76 % and 67. 46 % SGC-7901 cells were arrested in G2 / M , S and G0 / G1 phrase , respectively. Gen and 5-FU could both induce apoptosis and arrest cell cycle of SGC-7901 cells. Conclusion  Combination therapy of Gen with 5-FU may take synergistic inhibitory effect on the proliferation of gastric cancer cells by resting cell cycle and inducing cell apoptosis at a certain range of concent ration.

  • Effectiveness and Safety of Sustained-Release Fluorouracil for Gastric Cancer: A Systematic Review

    Objective To evaluate the effectiveness and safety of implanted sustained-release fluorouracil in gastric cancer surgery. Methods Literature search was conducted in the following databases: PubMed, EMbase, The Cochrane Library (Issue 6, 2012), CNKI, VIP and WanFang Data from inception to June, 2012. Randomized controlled trials (RCTs) or quasi-randomized controlled trials on implanted sustained-release fluorouracil for gastric cancer were included. Two reviewers independently identified the literature according to the inclusion and exclusion criteria, and then extracted the data and assessed the quality of the included studies. Then, meta-analysis was conducted using RevMan 5.1 software. Results A total of 7 studies involving 742 patients were included. The results of meta-analysis showed no significant difference in the rate of postoperative complications between the two groups (OR=0.93, 95%CI 0.54 to 1.59, P=0.79), while a significant reduction was found in the recurrence rate in the sustained-release fluorouracil group during 1 to 3 year follow-up (1 year after surgery: OR=0.32, 95%CI 0.22 to 0.46, P=0.02; 2 years after surgery: OR=0.19, 95%CI 0.08 to 0.42, Plt;0.001; 3 years after surgery: OR=0.40, 95%CI 0.24 to 0.67, P=0.004). As for the survival rate, no significant difference was found between the two groups 1 year after surgery (OR=1.98, 95%CI 0.92 to 4.25, P=0.08), while it was significantly higher in the sustained-release fluorouracil group than in the control group 2 to 3 years after surgery (2 years after surgery: OR=2.63, 95%CI 1.17 to 5.91, P=0.02; 3 years after surgery: OR=2.42, 95%CI 1.53 to 3.83, P=0.002). Adverse reaction rates in the sustained-release fluorouracil group were lower than those in the control group, but without significantly differences between the two groups (OR=1.22, 95%CI 0.49 to 3.07, P=0.67). Conclusion Compared with the control group, implanted sustained-release fluorouracil for gastric cancer can significantly reduce the recurrence rate 1 to 2 years after surgery and improve the overall survival rate 2 to 3 years after surgery without increasing the incidences of the postoperative complications and adverse reaction. However, due to the limitation of quantity and quality of the included studies, this conclusion should be further confirmed by more high quality, larger sample and multi-center RCTs.
