  • STUDY ON BIOLOGICAL FIXATION OF FEMORAL STEM PROSTHESIS WITH MULTILAYER MACROPORES COATING BY COMBINED USE OF AUTOGENOUS BONE GRAFTING

    Objective To observe the effect of biological fixation of femoral stem prosthesis with multilayer macropores coating by combined use of autologousbone grafting. Methods The reconstructing femoral stem prostheses were designed personally, proximal 2/3 surfaces of which were reformed by thick multilayer stereo pore structure. Twentyfour adult mongrel canines underwent right femoralhead replacement and were divided randomly into two groups. The autogenous bonemud of femoral head and neck were not used in the control group. The histologicexamination, roentgenograms and biomechanical test were carried out in the 1st,3rd and 6th month after operation to observe the bone formation and fixation inthe exterior and interior sides of the prostheses. Results Onthe whole view,bone reconstruction occurred in experimental group in the 3rd and 6th month. Roentgenograms also proved to be superior to the control group. Histological examinationshowed that both the maximum bone inserting depth(μm) and average engorging ratio(%) of newly formed bone in experimental group surpassed those in the control group. The maximum shear strength of prosthesisbone interface in experimental group was significantly higher than that in the control group(Plt;0.01). Conclusion Intensity of biological fixation can be strengthened remarkably by using femoral stem prothesis with multilayer macropores coating by combined use of autologous bone grafting.

  • PRIMARY ANTERIOR FOCUS DEBRIDEMENT AND BONE AUTOGRAFT WITH INTERNAL FIXATION VIA TRANSPERITONEAL APPROACH FOR TUBERCULOSIS OF LUMBOSACRAL JUNCTION

    Objective To evaluate the cl inical outcomes of primary anterior focus debridement, bone autograft, and internal fixation via transperitoneal approach in treating tuberculosis of the lumbosacral junction. Methods From February 2002 to April 2007, 16 patients with tuberculosis of the lumbosacral junction underwent anterior radical debridement, autologous il iac bone graft (two pieces of il iac bone, 5 cm × 3 cm in size), and internal fixation via transperitoneal approach, including 4 males and 12 females aged 27-63 years old (average 38 years old). The course of the disease ranged from 6 to 18 months (average 10 months). All patients experienced various degrees of pain in the lumbosacral area and toxic symptoms of thetuberculosis. Nine cases were compl icated with radicular pain in the lower extremities, and 3 cases had saddle area anaesthesia. Two cases were initially diagnosed as lumbar intervertebral disc protrusion and treated accordingly. The segments involved by the tuberculosis were L5-S1 level in all cases. The average erythrocyte sedimentation rate (ESR) was 61 mm/hour. Imaging examination confirmed the diagnosis of spinal tuberculosis. All cases received four antitubercular drugs and nutrition support for nerve before operation. Operation was performed when hepatorenal function was normal, and the toxic symptom of the tuberculosis was under control or ESR was decreased. Results Operation was performed safely in all cases without injuries of abdominal viscera, major blood vessel, cauda equina nerve and ureter. All wounds healed by first intention. No recurrence of tuberculosis and formation of sinuses occurred. All cases were followed up for 12-37 months (average 21 months). No such compl ications as tuberculous peritonitis and intestinal obstruction occurred. No postoperative erectile dysfunction and retrograde ejaculation occurred in the 4 male patients. ESR was recovered to normal 3-6 months after operation, and regular X-ray and CT exams showed no displacement of grafted bone. All patients achieved bony fusion 12 months after operation without the occurrence breakage and loosening of titanium plate and screw. The radical pain in the lower extremities and the saddlearea anaesthesia disappeared. Four patients had pain in the il iac donor site, 2 patients had mild pain in the lumbosacral area, and the pain was el iminated after symptomatic treatment. The therapeutic effect was graded as excellent in 14 cases and good in 2 cases according to the therapeutic effect evaluation criteria of Chen and co-workers. Conclusion The surgical treatment of tuberculosis of the lumbosacral junction with primary anterior focus debridement, bone autograft, and internal fixation via transperitoneal approach can achieve satisfying bony fusion and reconstruct spinal stabil ity.

  • CLINICAL ANALYSIS OF LIGHTBULB OPERATION WITH NANO-HYDROXYAPATITE/COLLAGEN FOR THE TREATMENT OF OSTEONECROSIS OF THE FEMORAL HEAD

    Objective To retrospectively analyze the cl inical effect of l ightbulb operation with nano-hydroxyapatite/ collagen in a consecutive series of patients with osteonecrosis of the femoral head (ONFH). Methods From January 2001to July 2005, 26 patients (35 hips) were treated, 16 males and 10 females, aged 19-54 years old (33.5 on average). The course of disease was 12-36 months (18 months on average). Based on the etiology, 15 cases (22 hips) were steroid induced type, 10 (12 hips) were alcohol induced type and the other one (1 hip ) was idiopathic type. According to the system of Association Research Circulation Osseous (ARCO), there were 6 hi ps of stage IIB, 16 hi ps of stage IIC, 9 hi ps of stage IIIA, 3 hi ps of stage IIIB and 1 hip of stage IIIC. The Harris score was 62.2 ± 7.5. All the patients who had undergone l ightbulb operation with nano-hydroxyapatite/collagen were evaluated both cl inically and radiographically. The bone graft mixture rate of nanohydroxyapatite/ collagen and autogenous bone was 1 ∶ 1, and the mixed bone graft was 6 times of the scraped osteonecrosis volume (30-48 mL). Results The incisions of all 26 patients (35 hi ps) obtained heal ing by first intention. The 2 cases, which got lateral femoral cutaneous nerve injury during the operation, recovered 3-6 months after the operation without any treatment. Another 2 cases got heterotopic ossification 3 months after operation, with no special treatment. All the 26 patients (35 hips) were followed up for 2-7 years (3.5 on average). The patients’ bone heal ing began from the 3rd month after operation. The postoperative Harris score was 85.1 ± 16.2, and there was significant difference compared with the preoperative one (P lt; 0.001). There were 15 hips of excellent, 11 of good, 5 of fair, and 4 of poor which received total hip arthroplasty at the end of the follow-up. According to imaging, 5 hips were progressed from preoperative IIC to IIIA, while the other hips were radiologically stable, with no progress of ONFH. Conclusion Lightbulb operation with nano-hydroxyapatite/collagen provides a surgical treatment to treat early ONFH with satisfactory cl inical outcomes. Nano-hydroxyapatite/collagen is beneficial for the repair and reconstruction of ONFH and suitable for femoral-head-preserving operation for the patients with ONFH of stage II.

  • TREATMENT OF MULTI-SEGMENTAL LUMBAR DISC HERNIATION AND SPINAL CANAL STENOSIS

    To explore the treatment of multi-segmental lumbar disc herniation and spinal canal stenosis by laminectomy, removal of nucleus pulposus, fusion of intra-transverse process and general spine system(GSS) fixation. Methods From January 2004 to January 2006, 21 patients with multi-segmental lumbar disc herniation and spinal canal stenosis were treated by laminectomy, removal of nucleus pulposus and GSS pedicle screw spinal system. There were 14 males and 7 females with an average age of 53 years ranging from 46 to 61 years, and with an average disease course of 18 months ranging from 8 months to 15 years. All of the patients were examined by X-ray with AP position, lateral position and dynamic lateral position, CT and MRI, and all of them proved to be with multi-segmental lumbar disc herniation and different degrees of spinal canal stenosis. A total of 47 nucleuses were picked out, and 47 instable segments were filled in with granule selfbone. Results There were 21 patients who were followed up for 1 to 2 years with an average of 13 months. All patients achieved successful fusion and bony union postoperative from 8 to 12 months, and no artificial joint was formed. As to the cl inical results in 21 cases, according to the Macnab outcome criteria, 14 were excellent, 6 were good and 1 was poor, the excellent and good rate was 95.2%. Conclusion The methods of laminectomy, removal of nucleus pulposus, fusion of intra-transverse process and GSS system fixation are effective in treatment of multi-segmental lumbar disc herniation and spinal canal stenosis.

  • ROLE OF RECOMBINANT HUMAN BONE MORPHOGENETIC PROTEIN 2/ COLLAGEN AS AN ONLAY BONE GRAFT ON ADULT RAT CALVARIAL BONE

    Objective To evaluate the bone regenerative potential of reconbinant human bone morphogenetic protein 2(rhBMP-2) / collagen on adult rat calvarial bone. Methods A tight subperiosteal pocket was produced under both sides ofthe temporal muscle in rats. rhBMP-2 / collagen was implanted in one side and collagen alone was implanted in the other side as control. The rats were sacrificed 2, 4 and 8 weeks after operation. The specimen was harvested and examined histologically. For morphometric analysis, the thickness of the temporal bone of both sides was measured and compared. Results The rhBMP-2 / collagen onlay implant resulted in active bone formation and the augmented bone was connected directly with the original bone, whereas the collagen alone resulted in neither bone nor cartilage production. The ossification process in the rhBMP-2 / collagen occurred directly through bone formation, similar to intramembranous ossification. Conclusion rhBMP-2 / collagen is an effective material as a biological onlay implant.

  • CLINICAL APPLICATION OF DECALCIFIED DENTINAL MATRIX OF HUMAN TO REPAIR ALVEOLAR CLEFT

    【Abstract】 Objective To study a new method of alveolar cleft bone graft repair. Methods From April 2005to August 2006, 26 cases (30 sides) of alveolar cleft were treated with trabecular substance of self-il ium combined with the decalcified dentinal matrix of human (DDM). There were 16 males and 10 females, aged 6-12 years. Unilateral alveolar cleft was involved in 22 cases, bilateral alveolar cleft in 4 cases. All cases were accompanied by nasal wing collapse, 5 by deciduous tooth retention, 3 by malposed teeth and 1 by tooth deformity. Anterior occlusal radiographs and panoramic oral radiographs were taken to observe union and bone absorption before and after operation. Results Twenty-three patients achieved heal ing by first intention, oronasal fistulas were closed successfully. Infection occurred in one bilateral alveolar cleft case and bone tissue exposure in 2 cases. Wound healed by rinse or dressing change and inunction with MEBO. The X-ray films after operation showed bone bridge formation in the alveolar cleft. The stabil ity and continuity of body of maxilla were resumed. Four cases (6-9years old) were observed 6.5 months, 22 cases (9-12 years old ) were observed 8.6 months. According to Bergland for evaluation,the X-ray films after 3 months of operation showed 16 cases (16 sides) in class I (53.3%), 7 cases (8 sides) in class II (26.7%),2 cases (4 sides) in class III (13.3%), and 1 case(2 sides) in class IV (6.7%). The overall survival ratio of alveolar bone grafting was 93.3%, and the cl inical success ratio was 80%. Conclusion The DDM is good in alveolar cleft bone graft repair. It is better than using self-il ium only cl inically.

  • 改良腰椎后路椎间植骨融合术治疗退变性腰椎失稳

    目的  总结改良腰椎后路椎间植骨融合术(posterior  lumbar  interbody  fusion,PLIF)治疗退变性腰椎失稳的疗效。  方法   2006 年 5 月- 2008 年 1 月,采用改良 PLIF 治疗退变性腰椎失稳患者 36 例。男 21 例,女 15 例;年龄 38 ~ 61 岁,平均 48.7 岁。病程 6 ~ 26 个月。病变位于 L3、 4 2 例, L4、 5 16 例, L5、 S1 13 例, L4 ~ 5、 S1 5 例。术后定期随访评估临床疗效、植骨融合率和椎间隙高度。  结果  1例术后1周出现切口急性金黄色葡萄球菌感染,对症治疗后痊愈;余 35 例切口Ⅰ期愈合。36 例均获随访,随访时间 16 ~ 26 个月,平均 18 个月。术后 1 年薄层螺旋 CT 扫描三维重建可见完全的骨小梁连接,达骨性融合。术前椎间隙高度为(9.5 ± 1.2)mm,术后 7 d 为(11.2 ± 1.1)mm,末次随访时为(11.0 ± 1.1)mm,手术前后比较差异均有统计学意义(P lt; 0.01),术后 7 d 与末次随访比较差异无统计学意义(P gt; 0.05)。采用日本骨科协会(JOA)下腰痛评分标准,获优 29 例,良 5 例,中 2 例,优良率 94.4%。  结论  改良 PLIF 治疗退变性腰椎失稳最大限度保留了后柱结构,创伤小,植骨融合率高,椎间隙高度维持良好,临床疗效满意。

  • A COMPARATIVE STUDY OF ANTERIOR DECOMPRESSION APPROACH BY USING CERVICAL RETRACTORSYSTEMS AND TRADITIONAL SURGICAL APPROACH TO TREAT CERVICAL SPONDYLOSIS

    To compare and evaluate the whole effect of anterior decompression approach to treat cervical spondylotic myelopathy by using cervical retractor systems and the traditional surgical approach. Methods From April 2001 to August 2004, group A included 30 males and 23 females aging from 31 to 69 years, and the involved time was from 7 months to 15 years. Involved segments included 22 one-segments, 24 two-segments and 7 three-segments. In all 53 cases, anterior windowing decompression and fusion with autograft and titanium plate internal fixation by using traditionalcircular saw were performed. Group B included 48 males and 20 females aging from 33 to 74 years, and the involved time was from 5 months to 18 years. Involved segments included 23 one-segments, 34 two-segments and 11 three-segments. In all 68 cases, anterior undermined far-reaching decompression and fusion with autograft and titanium plate internal fixation by using removing disc merely in the single-level or separately in the multilevels employing self-retractor and Caspar cervical retractor systems via interspinal approach were performed. X-rays and MRI showed cervical disc degeneration, herniation and spinal cord compression. The surgery time, loss of blood, vertebral body fusion time, difference in height of involved segments preor postoperatively and compl ications were counted up and compared between the two groups. Improvement rate of spinal function pre- or postoperatively were valued by using JOA score. Results A total of 92 cases including 42 of group A and 50 of group B were followed up for 3 to 5 years, mean 3.5 years. In group A, surgery time, loss of blood, time of vertebral body fusion, difference in height of involved segments pre- or postoperatively, and improvement ratio of spinal function were(76.80 ± 28.41) min,(564.00 ± 181.96) mL,(12.10 ± 3.58) weeks, (1.30 ± 0.67) mm and 0.49% ± 0.14%, respectively. In group B, they were(57.90 ± 15.01) min,(317.50 ± 136.92) mL,(9.75 ± 1.36) weeks, (3.00 ± 0.56) mm and 0.71% ± 0.17% , respectively. The differences between the two groups were significant in all measured values(P lt; 0.05). Compl ications occurred in 7 cases ofgroup A including 1 spinal cord injury, 1 plate displacement, 1 bonegraft displacement and 4 disfunctions of il iac region. No compl ication happened in group B. Conclusion Anterior decompression approach using cervical retractor systems is significantly superior to the traditional approach as to the whole effect to treat cervical spondylotic myelopathy, and furtherperfects the traditional anterior decompression approach. The modified approach is scientific, safe and easily spread.

  • ANTERIOR EXPANSIVE DECOMPRESSION IN THE TREATMENT OF MYELOPATHY SECONDARY TO CERVICAL SPONDYLOSIS

    Twenty cases of myelopathy secondary to cervical spondylosis had been treated by anterior expansive decompression and interbody iliac bone graft. Follow-up for 8 to 42 months, the therapeutic results were satisfactory without any obvious complications. The techniques of fenestration and bone graft were introduced. The advantages of this operation were discussed.
