  • Correlation of plasma lipocalin-2 with inflammation and predictive value of lipocalin-2 for detecting acute kidney injury in acute pancreatitis

    ObjectiveTo explore the correlation of serum lipocalin-2 (LCN2) with inflammation and the predictive value of LCN2 for detecting acute kidney injury (AKI) in acute pancreatitis (AP).MethodsNighty-one patients with AP, who were admitted to Bazhong Municipal Hospital of Traditional Chinese Medicine between June 2016 and June 2018, were enrolled in the present study. Clinical paramaters were analyzed between patients with AKI (n=29) and patients without AKI (n=62). The correlation of serum LCN2 with inflammation was assessed with Pearson’s correlation analysis. The area under the receiver operating characteristic curve (ROC AUC) for serum LCN2 predicting AKI in AP patients was assessed.ResultsCompared with the patients without AKI, the patients with AKI showed increased serum levels of C-reactive protein [(64.8±10.5) vs. (148.3±21.6) mg/L], procalcitonin [(3.5±2.3) vs. (4.8±3.9) μg/L], urea nitrogen [(5.5±2.1) vs. (6.6±2.8) mmol/L], creatinine [(80.3±28.1) vs. (107.3±30.8) μmol/L], interleukin-6 [(10.1±3.7) vs. (16.2±4.6) pg/mL], and LCN2 [(155.0±37.6) vs. (394.8±53.1) mg/mL], as well as decreased level of calcium [(2.6±1.3) vs. (2.0±1.0) mmol/L], the differences were all statistically significant (P<0.05). The serum level of LCN2 was correlated with C-reactive protein (r=0.694, P<0.05), interleukin-6 (r=0.762, P<0.05), and procalcitonin (r=0.555, P<0.05) in patients with AP. The ROC AUC of LCN2 for predicting AKI was 0.844 (P<0.05) , with a sensitivity of 81.3% and a specificity of 81.4% when the cut-off value was 210.2 ng/mL.ConclusionsSerum LCN2 concentration is elevated in patients with AKI. In patients with AP, serum LCN2 level is positively correlated with C-reactive protein, interleukin-6, and procalcitonin. It can be regarded as a reliable indicator for predicting AKI.

  • Research progress of augmenter of liver regeneration in acute kidney injury

    Augmenter of liver regeneration (ALR) is a newly discovered cytokine that can promote liver regeneration and proliferation of damaged liver cells. In the renal tissue, ALR is mainly expressed in the cytoplasm of the medullary loops, collecting ducts and distal convoluted tubules in the renal medulla, and is low in the glomerular and cortical tubules. Various stimulation, such as ischemiacal, hypoxia, poisoning and inflammatory stimulation, can induce the expression of ALR in the epithelial cells of proximal tubule regeneration and the damaged areas of cortex, and participate in the repair process. Current studies have found that in acute kidney injury (AKI), exogenous ALR can protect renal tubular epithelial cells by inhibiting apoptosis of renal tubular epithelial cells, promoting proliferation of renal tubular epithelial cells, inhibiting the activities of inflammatory cells, and promoting the reduction of renal injury. This paper intends to review the basic characteristics of ALR and the pathogenesis of AKI, summarize the characteristics of the mechanism of ALR in AKI by combing the relevant literature on ALR and AKI in recent years, and provide knowledge reserve and direction reference for the in-depth study of ALR in kidney in the future.

  • The predictive value of preoperative prognostic nutritional index for postoperative acute kidney injury in 584 patients undergoing cardiac surgery

    ObjectiveTo determine the predictive value of the preoperative prognostic nutritional index (PNI) regarding the development of acute kidney injury (AKI) after non-coronary artery bypass grafting (CABG) cardiac surgery.MethodsThe clinical data of 584 patients who underwent elective non-CABG cardiac surgery with cardiopulmonary bypass (CPB) in our hospital from May to September 2019 were reviewed. There were 268 (45.9%) males and 316 (54.1%) females, with a mean age of 52.1±11.6 years. The mean cardiopulmonary time and aortic-clamp time was 124.8±50.1 min and 86.4±38.9 min, respectively. Totally 449 (76.9%) patients received isolate valve surgery. We developed the risk prediction model of AKI using multivariable logistic regression. The predictive values of preoperative PNI, Cleveland Clinic Score (CCS) and risk prediction model were estimated by the area under the receiver operating characteristic curve (AUC) and Hosmer-Lemeshow goodness-of-fit test. The improvement of preoperative PNI to predictive values of CCS or AKI risk prediction models were defined by the net reclassification index (NRI) and variation of AUC.ResultsThe preoperative PNI could neither effectively predict the occurrence of AKI following non-CABG cardiac surgery (AUC=0.553, 95%CI 0.489-0.617, P=0.095) nor improve the predictive effect of other AKI predictive models. The risk prediction model of AKI structured by our study had high predictive value on AKI or severe AKI (stage 2-3) (AUC=0.741, 95%CI 0.686-0.796, P<0.001) and superior to CCS (AUC=0.512, 95%CI 0.449-0.576, P=0.703).ConclusionThe preoperative PNI can neither predict the occurrence of AKI following elective non-CABG cardiac surgery nor improve the prediction values of other AKI prediction models.

  • Early warning value of selenium binding protein 1 on acute kidney injury and its risk factor exposure

    Objective To investigate the early warning value of urinary selenium binding protein 1 (SBP1) in acute kidney injury (AKI) and its risk factor exposure, and compare it with urinary neutrophil gelatinase-associated lipocalin (NGAL). Methods Adult AKI inpatients and medical workers from the Department of Nephrology of the Second Affiliated Hospital of Xi’an Jiaotong University between April 2023 and April 2024 were selected. Patients who underwent percutaneous coronary intervention (PCI) in the Department of Cardiology of the Second Affiliated Hospital of Xi’an Jiaotong University were selected in June 2023. Patients who received cisplatin treatment in the Respiratory Department of the Second Affiliated Hospital of Xi’an Jiaotong University were selected in June 2023. Urinary SBP1 and NGAL levels of patients and medical workers were detected by enzyme-linked immunosorbent assay. Results A total of 14 medical workers and 36 AKI patients were included. Except for gender, alanine aminotransferase, aspartate aminotransferase, total cholesterol (P>0.05), there were statistically significant differences in other indicators between the medical workers and the AKI patients (P<0.05). The urine SBP1 [100.30 (71.50, 138.75) vs.75.60 (65.90, 80.08) pg/mL; U=2.918, P=0.004] and NGAL [423.70 (73.93, 839.80) vs. 14.80 (5.83, 29.98) ng/mL; U=4.668, P<0.001] levels in the AKI group were higher than those in the control group. But the area under the curve of receiver operative characteristic curve of urine SBP1 was smaller than that of urine NGAL (0.768 vs. 0.929). The urine SBP1 level in AKI patients was positively correlated with alanine aminotransferase, aspartate aminotransferase, serum creatinine, and serum glucose (P<0.05), but negatively correlated with estimated glomerular filtration rate and total cholesterol (P<0.05). A total of 14 patients who underwent PCI were included. The urinary SBP1/creatinine levels of PCI patients increased 6 hours after surgery compared to preoperative levels [(39.54 ± 8.00) vs. (19.34±2.90) pg/μmol; F=8.862, P=0.011]. The urea nitrogen level decreased 72 hours after surgery compared to preoperative levels (P=0.036), while there were no statistically significant differences in other indicators at other time points (P>0.05). There was no significant change in urinary NGAL levels before and after PCI treatment in patients. A total of 19 patients received cisplatin treatment were included. After cisplatin treatment, the level of urinary SBP1 increased compared to before treatment (P=0.024), while there was no significant change in the level of urinary NGAL after treatment compared to before treatment (P=0.350). After treatment, the levels of urea nitrogen (P=0.041) and cystatin C (P=0.002) increased compared to before surgery, while there was no statistically significant difference in blood creatinine and estimated glomerular filtration rate compared to before treatment (P>0.05). Conclusions Urinary SBP1 levels have certain diagnostic value for AKI, but the diagnostic efficacy is not as good as urinary NGAL. Urinary SBP1 is more sensitive to renal tubular injury caused by nephrotoxic drugs than urinary NGAL.

  • 体外循环相关急性肾损伤与干细胞治疗研究进展

    体外循环(extracorporeal circulation,ECC)是心脏直视手术不可或缺的辅助及治疗手段。但同时,ECC 带来的全身炎症反应及多器官损伤也给临床提出了挑战。急性肾损伤(acute kidney injury,AKI)是 ECC 术后主要的并发症及死亡原因之一,不仅增加住院时间和住院费用,还明显增加术后患者的病死率,极大地威胁着临床患者的预后。然而,目前国内外针对 ECC 相关 AKI 的治疗手段十分有限,主要集中于围手术期药物使用及肾替代治疗等对症支持治疗两个方面。尽管目前的治疗大多集中于 AKI 的损伤期,内源性细胞修复及以细胞为基础的治疗方案受到越来越多的重视。最新的研究发现干细胞在ECC相关肾损伤的修复过程中发挥着重要的作用。该文对 ECC 相关 AKI 与干细胞治疗研究进展进行了综述。

  • 慢性阻塞性肺病并发急性肾损伤的临床分析

    【摘要】 目的 总结慢性阻塞性肺疾病(COPD)并发急性肾损伤(AKI)的发病机制、临床特点及救治经验。 方法 回顾性分析2008年收治的COPD并发AKI的80例患者的临床资料,初步分析其临床表现、并发症、呼衰程度与AKI的关系,治疗措施与预后的关系。 结果 80例COPD并发的AKI治愈46例(57.5%),好转16例(20%),转为CKD3例(3.75%),死亡15例(18.75%)。 结论 COPD患者是AKI的高危人群,发病率高,并发AKI后易发生多器官功能衰竭(MOF),死亡率增加。早期积极治疗可使多数患者发生的AKI逆转。对于COPD患者避免诱发加重因素,早期识别和早期干预AKI十分重要。

  • Management throughout the whole course of acute kidney injury

    The high incidence and mortality of acute kidney injury (AKI) have brought great challenges to global health. In recent years, China has made some achievements in the epidemiology, risk factors and treatment of AKI. However, further prevention and treatment are still facing difficulties. Based on current new ideas and research progress, this paper summarized and analyzed the management throughout the whole course of AKI, including AKI risk assessment, early prevention, early identification, treatment and follow-up. The aim is to make Chinese nephrologists realize the focus of AKI prevention and treatment, standardize the management of AKI, and explore the prevention and treatment strategy suitable for AKI in China.

  • 2017 Taiwan continuous renal replacement therapy operational manual for critically ill patients

    Acute kidney injury is a common complication and is associated with multiple organ dysfunction syndrome among critically ill patients in intensive care unit. Once renal replacement therapy in required, the mortality rate was high. Using slow and uninterrupted clearance of retained fluid and toxins, continuous renal replacement therapy (CRRT) can avoid hemodynamic instability while provide acid-base, electrolytes, and volume homeostasis. For decades, CRRT has become the dominant form of renal replacement therapy as well as multiple organ support in critically ill patient with acute kidney injury. However, there remains wide practice variation in the CRRT care when clinicians take into account the needs of individual patients, available resources, and limitations unique to an institution or type of practice, despite evidences to guide practice. In addition, CRRT is a complex technology that is resource-intensive, costly, and requires specialized training by health providers.Taiwan Society of Critical Care Medicine organized a group of experts in critical care and nephrology to review the recommendations and provide their clinical practice and concerns to write this operational manual. The purpose of this manual is to provide step-by-step instructions on the practice of CRRT and troubleshooting. In addition, it is designed to help the newbies to carry out this complex treatment correctly and efficiently. We hope that this operational manual is of value to improve clinical skills, quality of care, and patient safety.

  • Impact of Preoperative Coronary Angiography on Postoperative Acute Kidney Injury in Patients with Valve Replacement

    ObjectiveTo explore whether preoperative coronary angiography could increase the incidence of postoperative acute kidney injury for patients with valve replacement. MethodsA total of 638 patients underwent routine cardiac valve replacement in our hospital from January 2013 through September 2015. There were 118 patients with preoperative coronary angiography (a coronary angiography group), and 520 patients without coronary angiography (a non-coronary angiography group). Serum creatinine (Scr), urea nitrogen(Bun), brain natriuretic peptide (BNP), creatine kinase myocardial band (CK-MB), cardiac troponin I (cTnI) values were recorded at 4 time points:before surgery (T0), after surgery 12 h (T1), 24 h (T2), 48 h (T3). The number of patients with acute kidney injury at the time of 48 hours after surgery was recorded. ResultsScr values (91.6±37.7 μmol/L vs. 81.0±27.4 μmol/L, 84.9±23.6 μmol/L vs. 73.5±25.3 μmol/L) increased in the patients who did not undergo coronary angiography at the time of 24 hours and 48 hours after cardiac surgery compared with the patients with coronary angiography with statistical differences. While there was no statistical difference in the incidence of acute kidney injury between the two groups. The cardiac enzymes had no statistical difference between the two groups. ConclusionPreoperative coronary angiography does not increase the probability of postoperative acute kidney injury.

  • Risk factors affecting prognosis in patients undergoing continuous renal replacement therapy for acute kidney injury after extracorporeal circulation surgery

    Objective To explore the risk factors affecting the prognosis of patients with acute kidney injury (AKI) after extracorporeal circulation surgery who receive continuous renal replacement therapy (CRRT). Methods Patients who developed AKI and underwent CRRT treatment after extracorporeal circulation surgery at the First Affiliated Hospital of Chongqing Medical University between May 2019 and May 2024 were retrospectively selected. According to the prognosis, patients were divided into the good prognosis group and the poor prognosis group. Basic information, duration of extracorporeal circulation during surgery, aortic occlusion time, timing and duration of CRRT initiation therapy, relevant laboratory indicators before surgery, during CRRT intervention, and upon discharge or death were collected. The risk factors affecting the prognosis of such patients were analyzed. Results A total of 45 patients were included. Among them, there were 20 cases in the good prognosis group and 25 cases in the poor prognosis group. There was no statistically significant difference in the basic information between the two groups (P>0.05). Compared with the poor prognosis group, the good prognosis group had decreased preoperative urea nitrogen and increased hemoglobin levels, reduced levels of alanine aminotransferase and aspartate aminotransferase during the initiation of CRRT treatment, and reduced levels of white blood cell count, neutrophil percentage, alanine aminotransferase and aspartate aminotransferase and elevated platelet count before discharge or death (P<0.05). The results of multivariate logistic regression analysis showed that the total duration of CRRT treatment [odds ratio (OR)=1.007, 95% confidence interval (CI) (1.000, 1.015), P=0.046], white blood cell count before discharge or death [OR=1.541, 95%CI (1.011, 2.349), P=0.044], and platelet count before discharge or death [OR=0.964, 95%CI (0.937, 0.991), P=0.010] could affect patient prognosis. Conclusions In patients with AKI after extracorporeal circulation surgery, if combined with renal dysfuction and anemia before surgery, liver function damage and secondary infection during CRRT initiation therapy may be related to poor patient prognosis. The longer the duration of CRRT treatment, the higher the white blood cells before discharge or death, and the lower the platelet count are independent risk factors for poor prognosis in patients.
