  • Effect of early graded respiratory severe rehabilitation training for patients with mechanical ventilation under multidisciplinary cooperation mode

    ObjectiveTo explore the effect of early graded respiratory severe rehabilitation training for patients with mechanical ventilation under a multidisciplinary model.MethodsTwo hundred and thirty-six patients were surveyed, who were hospitalized in the intensive care unit of the First Affiliated Hospital of Anhui Medical University from June 3, 2019 to March 31, 2020. They were randomly divided into an observation group and a control group, with 118 patients in each group. The observation group received rehabilitation training using early graded rehabilitation training under the mode of multidisciplinary cooperation, while the control group received routine respiratory rehabilitation training. Diaphragmatic excursion (DE) and diaphragmatic thickening fraction (DTF) of the patients before ventilator weaning were measured by ultrasound. The differences of DE, DTF, peak expiratory flow (PEF), maximal inspiratory pressure (MIP), success rate of withdrawal, duration of mechanical ventilation and intensive care unit (ICU) stay between the two groups were recorded and compared.ResultsAll evaluation indexes were statistically significant between the observation group and the control group (all P<0.05). There were interaction between oxygenation index, PEF, MIP, Acute Physiology and Chronic Health Score, Clinical Pulmonary Infection Score and recovery time.ConclusionRehabilitation training on early graded severe respiratory diseases under a multidisciplinary model can improve the respiratory function of patients on mechanical ventilation and shorten the duration of mechanical ventilation and ICU stay.

  • A review on multi-modal human motion representation recognition and its application in orthopedic rehabilitation training

    Human motion recognition (HAR) is the technological base of intelligent medical treatment, sports training, video monitoring and many other fields, and it has been widely concerned by all walks of life. This paper summarized the progress and significance of HAR research, which includes two processes: action capture and action classification based on deep learning. Firstly, the paper introduced in detail three mainstream methods of action capture: video-based, depth camera-based and inertial sensor-based. The commonly used action data sets were also listed. Secondly, the realization of HAR based on deep learning was described in two aspects, including automatic feature extraction and multi-modal feature fusion. The realization of training monitoring and simulative training with HAR in orthopedic rehabilitation training was also introduced. Finally, it discussed precise motion capture and multi-modal feature fusion of HAR, as well as the key points and difficulties of HAR application in orthopedic rehabilitation training. This article summarized the above contents to quickly guide researchers to understand the current status of HAR research and its application in orthopedic rehabilitation training.

  • Evaluation methods for the rehabilitation efficacy of bidirectional closed-loop motor imagery brain-computer interface active rehabilitation training systems

    The bidirectional closed-loop motor imagery brain-computer interface (MI-BCI) is an emerging method for active rehabilitation training of motor dysfunction, extensively tested in both laboratory and clinical settings. However, no standardized method for evaluating its rehabilitation efficacy has been established, and relevant literature remains limited. To facilitate the clinical translation of bidirectional closed-loop MI-BCI, this article first introduced its fundamental principles, reviewed the rehabilitation training cycle and methods for evaluating rehabilitation efficacy, and summarized approaches for evaluating system usability, user satisfaction and usage. Finally, the challenges associated with evaluating the rehabilitation efficacy of bidirectional closed-loop MI-BCI were discussed, aiming to promote its broader adoption and standardization in clinical practice.

  • 精神分裂症患者全程式康复护理模式的探讨

    目的 探讨精神分裂症患者急性期和恢复期康复训练内容和方法,并探索如何将住院康复和社区康复有机结合。 方法 对2009年6月-2011年8月在精神障碍病房住院治疗的2 132例患者采用开放管理模式,通过制定康复目标,设计康复方案及运用灵活多样的康复训练方法为患者提供康复服务。 结果 灵活多样、轻松愉快的康复训练及开放管理模式,使患者康复训练的依从性大为提高,康复效果明显。 结论 康复训练对改善精神分裂症患者社会功能,提高生活质量有非常重要的作用。与社区医院合作,共同解决出院后患者的后续康复问题,使其得到系统、持续的医疗服务。

  • 不同运动策略对稳定期慢性阻塞性肺疾病患者的影响

    目的 比较不同运动策略对慢性阻塞性肺疾病(简称慢阻肺)患者的康复训练效果。 方法 收集 2016 年 1 月至 2017 年 1 月在华北石油管理局总医院呼吸内科门诊随访的慢阻肺患者 86 例,随机分为策略一组与策略二组,每组 43 例。在常规药物治疗及健康教育基础上,策略一组采用功率自行车训练,策略二组采用地面行走训练。在训练进行至 12 周及 24 周时分别比较两组训练前后第 1 秒用力呼气容积占预计值百分比(FEV1%pred)、第 1 秒用力呼吸容积占用力肺活量百分比(FEV1/FVC)、最大摄氧量(VO2max)、6 分钟步行距离(6MWD)、BODE 指数、慢性阻塞性肺疾病评估测试(CAT)评分、改良英国医学研究委员会呼吸困难量表、Borg 呼吸困难评分是否有差异,并进行两组间比较。 结果 在训练进行至 12 周时策略一组、策略二组患者训练后各组指标均有改善,其中除肺功能指标外差异均有统计学意义(均 P<0.05)。同时两组患者康复运动后指标进行比较,策略一组各组数据均数较策略二组有好转,其中 VO2max、6MWD、BODE 指数差异有统计学意义(均 P<0.05)。在训练进行至 24 周时策略一组、策略二组患者较 12 周时各组指标均有改善,其中 6MWD、BODE 指数、Borg 评分、CAT 评分结果差异有统计学意义(均P<0.05)。同时两组患者康复运动后指标进行比较,策略一组各组数据均数较策略二组有好转趋势,但差异均无统计学意义(均P>0.05)。 结论 在慢阻肺患者康复训练中,功率自行车训练在短期内具有一定的优势,但坚持训练后两种训练方式基本无明显差异。

  • Clinical Research of Mini-plate for the Treatment of Metacarpal and Phalangeal Fractures and Early Rehabilitation

    ObjectiveTo discuss the clinical curative effect of internal fixation for metacarpal and phalanx fractures using stainless steel mini-plate and the early rehabilitation. MethodsThe study included 47 cases treated from January 2006 to June 2011, including 25 cases of 72 phalangeal fractures, 17 cases of 43 metacarpal fractures, and 5 cases of 23 complex metacarpal and phalangeal fractures. All the patients underwent open reduction and micro-plate internal fixation. Three days after surgery, passive function training was initiated. Patients accompanied with blood vessel damage began the training one week later. The training was carried out based on the early rehabilitation schedule. ResultsPrimary healing occurred in all the 47 cases. There was one case of broken plate because of crashing. Postoperative follow-up lasted for 4 to 10 months, and the fractures were healed in 138 lesions. X-ray examination showed that bone union took place at week 8 to 11 averaging at 10. According to the functional evaluation of total autive movement scales, the excellent and good rate was 83.0%. ConclusionThe stainless steel mini-plate in the treatment of metacarpal and phalangeal fractures has a firm fixation and is advantageous. Combined with early rehabilitation training, it can maximize the recovery of hand function with satisfactory clinical results.

  • Study on brain mechanism of rehabilitation training of articulation disorder in cleft lip and palate patients based on functional magnetic resonance imaging

    The cleft lip and palate (CLP) is one of the most common craniofacial malformations in humans. We collected functional magnetic resonance data of 23 CLP patients before rehabilitation training (Bclp) and 23 CLP patients after rehabilitation training (Aclp), who were performing Chinese character pronunciation tasks, and performed brain activation analysis to explore the changes of brain mechanism in CLP patients after articulation disorder rehabilitation training. The study found that Aclp group had significant activation in the motor cortex, Broca area, Wernicke area and cerebellum. While the Bclp group had weak activation in the motor cortex with a small activation range. By comparing the differences and co-activated brain regions between the two groups, we found that rehabilitation training increased the activity level of negatively activated brain areas (cerebellum, left motor area, Wernicke area, etc.) to a positive level. At the same time, the activity level of weakly activated brain areas (right motor area, Broca area, etc.) was also increased. Rehabilitation training promoted the activity level of articulation-related brain regions. So that the activation intensity of articulation-related brain regions can be used as a quantifiable objective evaluation index to evaluate the effect of rehabilitation training, which is of great significance for the formulation of rehabilitation training programs.

  • Observation of the clinical effect of preoperative exercise intervention on posterior lateral approach total hip replacement

    Objective To investigate and explore the clinical effect of preoperative exercise intervention on total hip arthroplasty. Methods Seventy-three patients who underwent primary unilateral total hip replacement (THR) in the Orthopedics Department, the Second Hospital of Jilin University between March 2017 and January 2018 were divided into 2 groups: treatment group (n=37) and control group (n=36). Patients in treatment group received exercise intervention and routine education since 4 weeks before surgery; the control group just received routine education before surgery and both groups underwent the same routine rehabilitation training. The Visual Analogue Scale (VAS) was evaluated and compared separately between the two groups at 4 weeks before surgery (T0), 1 day before surgery (T1), before discharge (T2), 1 month after discharge (T3), and 3 months after discharge (T4). The results of Hip Harris score and Time Up and Go test (TUG) were evaluated and compared separately between the two groups at T0, T1, T3 and T4. Results Time of TUG of the treatment group and the control group at T1, T3 and T4 were (14.59±3.15) vs. (16.31±3.31) s, (13.61±2.76)vs. (15.25±3.08) s, (12.49±2.37)vs. (14.22±2.65) s, respectively, and the differences between the two groups at T1, T3 and T4 were all statistically significant (P<0.05). In terms of VAS and Harris scores, both groups showed significant improvement after surgery at different time nodes (P<0.05), but the differences between the two groups was not statistically significant (P>0.05). Conclusion Preoperative exercise intervention can effectively improve the walking ability of THR patients and reduce the risk of falls after discharge, but it did not reduce postoperative pain or improve postoperative hip Harris score after discharge.

    目的:探讨健康教育及康复训练对子宫全切术患者的愈后效果的影响。方法:将41例子宫全切手术患者分常规组20例和观察组21例。常规组按妇产科护理常规进行术前术后常规护理及相关注意事项的讲解;观察组对患者进行术前术后常规护理同时实施健康教育及康复训练。结果:常规组患者自我认知能力稍差,有缺失感,住院时间稍长;观察组患者术后乐观,配合术后康复训练较为积极,住院时间稍短。结论:认为做好子宫全切术后患者的健康教育并进行及时的康复训练,对其术后康复并提高生活质量起着至关重要的作用。
