  • COMBINED ANTERIOR AND POSTERIOR SURGERIES FOR LUMBARSACRAL JUNCTION TUBERCULOSIS

    To explore the advantage and indication of combined anterior and posterior surgeries for lumbarsacral junction tuberculosis. Methods Eleven cases of the lumbarsacral junction tuberculosis were treated with combined anterior (radical debridement and autograft) and posterior (instrumentation and fusion) surgeries in one stage between January 2002 and December 2006. There were 9 males and 2 females with the age of 20-56 years old. The courseof disease was 4 to 15 months, 6 months on average. The lessons were located at L5, S1 in 7 patients, at L4,5, S1 in 2 patients and at L5, S2 in 2 patients. The involved vertebral bodies were at 2 segments in 7 patients; and 3 segments in 5 patients. The preoperative kyphosis was 5 to 8° with an average 9°. The sinus was associated in 3 patients, 3 patients had radiculopathy; 4 had paeumonophthisis and 9 had abscess. Results The followed-up period was from 6 months to 3 years, 18 months on average. According to Chen score, among the 11 cases, there were excellent in 9, good in 2. All incisions were healed up primarily. After operation, spinal fusion was achieved in 10 cases within 5 months to 7 months, 6 months on average, and pseudoarthrosis in 1 case was found by the CT examination. The postoperative kyphosis was 0 to 4° with the mean of 2° and the radiculopathy in 3 cases all got nerve function recovery. Conclusion Lumbarsacral junction tuberculosis treated with this surgical technique can achieve a high satisfactory rate with restoring the spinal stabil ity, arresting the disease early, providing early fusion, correcting the kyphosis and preventing progression of kyphosis particularly if lumbosacral spine tuberculosis is associated with sinus or preoperative diagnosis cannot exclude suppurative spondyl itis.

  • One-stage posterior retaining part facet joint in laminectomy and debridement combined with short segmental pedicle screw fixation for lumbosacral spinal tuberculosis

    ObjectiveTo evaluate the feasibility and effectiveness of one-stage posterior retaining part facet joint in laminectomy, bone graft and debridement combined with short segmental pedicle screw fixation for lumbosacral spinal tuberculosis. MethodsBetween January 2010 and December 2014, 32 cases of lumbosacral spinal tuberculosis (L4-S1) were treated by one-stage posterior retaining part facet joint in laminectomy, bone graft and debridement combined with short segmental pedicle screw fixation. There were 20 males and 12 females, aged 17-62 years (mean, 43 years). The disease duration was 12-48 months (mean, 18 months). The involved segments included L5, S1 level in 19 cases and L4, 5 level in 13 cases. The effectiveness was evaluated by Oswestry disability index (ODI) and imaging parameters (lumbar-sacral angle correction and Bridwell classification and CT fusion criteria) after operation. ResultsThe operation was successfully completed in all patients; the average operation time was 180 minutes, and the average intraoperative blood loss was 400 mL. All cases were followed up 12 to 67 months (mean, 15.6 months). At last follow-up, common toxic symptom of tuberculosis disappeared, and no internal fixation failure occurred. Neurological function was recovered to normal in 7 patients with neurological symptoms, and American Spinal Injury Association (ASIA) scale was improved to grade E from grade C (2 cases) and grade D (5 cases) before operation. At 1 year and last follow-up, the ODI scores were significantly improved when compared with preoperative score (P < 0.05), but no significant difference was found between at 1 year and last follow-up (P>0.05). The lumbarsacral angle was significantly increased at 7 days, 1 year and last follow-up when compared with preoperative one (P < 0.05), but there was no significant difference between different time points after operation (P>0.05). The bone graft fusion time was 9-24 months (mean, 12 months). At 1 year after operation and last follow-up, X-ray Bridwill bone fusion rates were 87.50% (28/32) and 93.75% (30/32) respectively, and CT fusion rates were 87.50% (28/32) and 90.63% (29/32) respectively; and there was significant difference in interbody fusion between at 1 year and last follow-up (P < 0.05). Drug resistance was observed in 4 cases; Bridwill gradeⅢand gradeⅣfusion was shown in 3 cases and 1 case after adjusting the anti-tuberculosis scheme after 1 year. ConclusionOne-stage posterior retaining part facet joint in laminectomy and debridement can effectively clear the tuberculose focus, intervertebral bone graft combined with short segment pedicle screw fixation can maintain postoperative spinal reliable stability and get satisfactory bone fusion rate, so it is an effective method for the treatment of lumbosacral tuberculosis.

  • Combined Mini-open Anterior Apical Vertebral Excision and Posterior Correction for Severe and Rigid Scoliosis

    目的 探讨前路小切口顶椎切除联合后路矫形手术治疗重度僵硬性脊柱侧凸的可行性及疗效。 方法 2009 年7月-2010年9月,采用前路小切口顶椎切除联合后路矫形手术治疗重度僵硬性脊柱侧凸18例。其中男9例,女9例,年龄10~24岁,平均14.5岁。其中15 例特发性脊柱侧凸(Lenke 2型6例,Lenke 3型1例,Lenke 4型8例),2 例脊髓空洞合并脊柱侧凸,1 例Chiari畸形合并脊柱侧凸。术前剃刀背高度(6.8 ± 2.3)cm,主胸弯Cobb角(99.6 ±10.0)°,主胸弯顶椎偏距(7.3 ± 1.3)cm。 结果 前路手术切口10~13 cm,平均(11.4 ± 1.0)cm;前路手术时间170~300 min,平均(215.3 ± 36.8)min;失血量300~1 300 mL,平均(662.5 ± 274.8) mL。所有患者随访25~39个月,平均30.7个月。末次随访时,剃刀背高度(1.0 ± 0.6)cm,矫正率86.7%;主胸弯Cobb角(31.4 ± 11.4)°,矫正率68.7%;主胸弯顶椎偏距(2.2 ± 0.9) cm,矫正率69.6%。上胸弯、胸腰弯/腰弯的Cobb 角及顶椎偏距亦明显矫正,冠状面及矢状面平衡与术前相比,差异无统计学意义(P>0.05)。未发生神经系统并发症,1例患者在前路手术后入ICU行呼吸支持治疗12 h,1例患者出现椎弓根螺钉穿透椎弓根上壁,2例患者出现钛网位置不佳,随访未见钛网位置改变。 结论 采用前路小切口顶椎切除联合后路矫形治疗重度僵硬性脊柱侧凸安全可行,矫形效果满意。

  • 改良腰椎后路椎间植骨融合术治疗退变性腰椎失稳

    目的  总结改良腰椎后路椎间植骨融合术(posterior  lumbar  interbody  fusion,PLIF)治疗退变性腰椎失稳的疗效。  方法   2006 年 5 月- 2008 年 1 月,采用改良 PLIF 治疗退变性腰椎失稳患者 36 例。男 21 例,女 15 例;年龄 38 ~ 61 岁,平均 48.7 岁。病程 6 ~ 26 个月。病变位于 L3、 4 2 例, L4、 5 16 例, L5、 S1 13 例, L4 ~ 5、 S1 5 例。术后定期随访评估临床疗效、植骨融合率和椎间隙高度。  结果  1例术后1周出现切口急性金黄色葡萄球菌感染,对症治疗后痊愈;余 35 例切口Ⅰ期愈合。36 例均获随访,随访时间 16 ~ 26 个月,平均 18 个月。术后 1 年薄层螺旋 CT 扫描三维重建可见完全的骨小梁连接,达骨性融合。术前椎间隙高度为(9.5 ± 1.2)mm,术后 7 d 为(11.2 ± 1.1)mm,末次随访时为(11.0 ± 1.1)mm,手术前后比较差异均有统计学意义(P lt; 0.01),术后 7 d 与末次随访比较差异无统计学意义(P gt; 0.05)。采用日本骨科协会(JOA)下腰痛评分标准,获优 29 例,良 5 例,中 2 例,优良率 94.4%。  结论  改良 PLIF 治疗退变性腰椎失稳最大限度保留了后柱结构,创伤小,植骨融合率高,椎间隙高度维持良好,临床疗效满意。

  • EFFECTIVENESS OF ISOBAR TTL SEMI-RIGID DYNAMIC STABILIZATION SYSTEM IN TREATMENT OF LUMBAR DEGENERATIVE DISEASE

    ObjectiveTo investigate the short-term effectiveness of ISOBAR TTL semi-rigid dynamic stabilization system (ISOBAR TTL system) in treatment of lumbar degenerative disease. MethodsBetween June 2007 and May 2011, 38 cases of lumbar degenerative disease were treated, including 24 males and 14 females with an average age of 51.2 years (range, 21-67 years). The disease duration was 8 months to 10 years (mean, 4.7 years). In 38 cases, there were 4 cases of grade I spondylolisthesis, 11 cases of lumbar instability and lumbar disc protrusion, 21 cases of lumbar spinal stenosis and lumbar disc protrusion, and 2 cases of postoperative recurrence of lumbar disc protrusion. There were 22 cases of adjacent segment disc degeneration. All cases underwent posterior decompression and implantation of ISOBAR TTL system. The double-segment-fixed patients underwent interbody fusion. Visual analogue scale (VAS) and Japanese Orthopaedic Association (JOA) scores for low back pain were used to evaluate clinical outcomes. The range of motion (ROM) at the semi-rigid dynamic stabilization segment was also measured. ResultsThe other cases achieved healing of incision by first intention, except 1 case of delayed healing. All the patients were followed up 8-53 months (mean, 27.8 months). After operation, ISOBAR TTL system showed reliable fixation, and no loosening, breakage, or kyphosis deformity occurred. No adjacent segment degeneration was observed. The ROM of the fixed segments was 0-1° in 3 cases, 1-2° in 4 cases, 2-3° in 14 cases, 3-4° in 15 cases, and gt; 4° in 2 cases. At last follow-up, the VAS score was 1.93 ± 2.43, and was significantly lower than preoperative score (8.20 ± 1.78) (t=7.761, P=0.000). JOA score was 23.06 ± 7.75, and was significantly higher than preoperative score (4.87 ± 3.44) (t=10.045, P=0.000). According to Stauffer-Coventry evaluation standard, the results were excellent in 32 cases, good in 3 cases, fair in 2 cases, and poor in 1 case, with an excellent and good rate of 92.1%. ConclusionGood short-term effectiveness can be achieved by surgical intervention with ISOBAR TTL system in treatment of lumbar degenerative disease.

  • Safety and efficacy of different doses of tranexamic acid in posterior cervical laminectomy with lateral mass screw fixation and bone graft fusion

    Objective To investigate the safety and efficacy of different doses of tranexamic acid (TXA) in posterior cervical laminectomy with lateral mass screw fixation and bone graft fusion by a prospective clinical study. Methods The middle-aged and elderly patients with cervical spondylotic myelopathy, who were admitted between January 2020 and January 2022 and scheduled to undergo posterior cervical laminectomy with lateral mass screw fixation and bone graft fusion, were studied as the research subjects. Among them, 165 patients met the selection criteria and were included in the study. The patients were allocated into 3 groups (n=55) by random double-blind lottery. Groups A and B were given intravenous infusion of TXA at 30 minutes before operation according to the standards of 15 and 30 mg/kg, respectively; and group C was given normal saline in the same way. There was no significant difference in gender, age, body mass index, and preoperative D-dimer, hemoglobin (Hb), and hematocrit (HCT) between groups (P>0.05). The intraoperative bleeding, intraoperative blood transfusion, postoperative drainage volume, drainage days, and postoperative hospital stay were recorded. The Hb, HCT, and D-dimer were compared before operation and at 3 days after operation. Venous ultrasonography of the lower extremities was taken after operation to assess thrombosis; and the postoperative hematoma and epilepsy were also observed. Results All operations were successfully completed, and the incisions healed by first intention. The differences in intraoperative bleeding volume, postoperative drainage volume, drainage days, and postoperative hospital stay between groups were significant (P<0.05). The above indexes were significantly less in group B than in groups A and C. During operation, 14 patients in group A and 23 patients in group C were transfused, and no patient in group B had blood transfusions. Compared with groups A and C, the blood transfusion volume in group B significantly decreased (P<0.05), and the difference between groups A and C was not significant (P>0.05). There was no significant difference in the differences of D-dimer, Hb, and HCT before and after operation between groups (P>0.05). At 5 days after operation, the venous ultrasonography of the lower extremities showed that the 2 cases of intermuscular venous thrombosis occurred in groups A, B, and C, respectively. No hematoma or epilepsy occurred after operation. Conclusion The application of 15 and 30 mg/kg TXA in posterior cervical laminectomy with lateral mass screw fixation and bone graft fusion can reduce intraoperative bleeding and postoperative drainage volume, postoperative drainage days, and postoperative hospital stay. And application of 30 mg/kg TXA can reduce intraoperative blood transfusion, without increasing the risk of lower extremity venous thrombosis, hematoma, and epilepsy.

  • Treatment of multiple segments of thoracolumbar tuberculosis using posterior unilateral debridement with bone graft and internal fixation

    Objective To explore the effectiveness and related issues in the treatment of multiple segments of thoracolumbar tuberculosis through posterior unilateral debridement with bone graft and internal fixation. Methods The clinical data of 29 patients with multiple segments of thoracolumbar tuberculosis who met the selection criteria were retrospective analyzed between January 2012 and July 2015. There were 17 males and 12 females, with age of 21-62 years (mean, 37.4 years). Lesions contained 3-8 vertebral segments, including 3 segments in 6 cases, 4-6 segments in 17 cases, and 7-8 segments in 6 cases. The center lesions located at thoracic spine in 8 cases, lumbar spine in 10 cases, and thoracolumbar segment in 6 cases, and thoracic lumbar skip lesions in 5 cases. The complications included vertebral abscess in 7 cases, psoas major abscess in 6 cases, sacral spine muscle abscess in 7 cases, iliac fossa and the buttocks abscess in 1 case, spinal canal abscess in 2 cases. Preoperative neurological function was assessed according to the American Spinal Injury Association (ASIA) classification: 1 case of grade B, 3 cases of grade C, 8 cases of grade D, and 17 cases of grade E. The disease duration was 6-48 months (mean, 19.3 months). All the patients were treated with posterior unilateral transpedicular or transarticular debridement with bone graft fusion and internal fixation under general anesthesia. Pre- and post-operative visual analogue scale (VAS) score, Oswestry disability index (ODI), and sagittal Cobb angle were recorded and compared. Bridwell classification standard was used to evaluate bone graft fusion. According to the number and the center of the lesion, the necessity to placement of titanium mesh cage was analyzed. Results All the patients were followed up 18-30 months (mean, 24 months). Cerebrospinal fluid leakage occurred in 3 cases, intercostal neuralgia in 2 cases, wound unhealed and fistula formation in 1 case, and ofiliac fossa abscess recurred in 1 case, and all recovered after symptomatic treatment. During follow-up, no fracture or loosing of internal fixation was found and all the lesions were cured at last follow-up. According to Bridwell classification standard, bone graft achieved bony fusion during 4-9 months after operation. The VAS score, ODI, and Cobb angle at immediate after operation and at last follow-up were significantly improved when compared with preoperative ones (P<0.05). At last follow-up, the neural function of all patients improved significantly when compared with preoperative one (Z= –3.101, P=0.002). The ratio of no placement of titanium mesh cage was significantly higher in patients with more than 6 lesion segments (6/6, 100%) than in patients with less than 6 lesion segments (4/23, 17.4%) (χ2=14.374, P=0.000). And the ratio of placement of titanium mesh cage was not significantly different between the patients with the different locations of center focus (χ2=0.294, P=0.863). Conclusion For treating multiple segments of thoracolumbar tuberculosis, the method of posterior unilateral debridement with bone graft and internal fixation can decrease the damage of posterior spinal structures and surgical trauma.

  • Comparison of Clinical Outcomes Between Two Methods of Posterior Lumbar Interbody Fustion in Adult Spondylolisthesis

    Objective To compare the clinical outcomes of posterior lumbar interbody fusion(PLIF) using simple cage alone fusion with pedicle screw fixationand autogenous bone grafting and cage fusion with pedicle screw fixation in adult spondylolisthesis. Methods From March 2003 to March 2004,Twenty-seven patients with lumbar spondylolisthesis were divided in two groups. In group A, 15 patients were treated by PLIF using simple cage alone fusion with pedicle screw fixation, including 4 males and 11 females, aging 53-68 years. Isthmic defectswere located at L4 in 9 cases, at L5 in 6 cases. Four patients were smokers.Thepreoperative mean disc space height was 5.4±2.3 mm, the mean percentage of slip was 36.8%±7.2%. In group B, 12 patients were treated by PLIF using autogenous bone grafting and cage fusion with pedicle screw fixation, including 3 males and 9 females, aging 56 years. Isthmic defects were located at L4 in 8 cases, atL5 in 4 cases. Five patients were smokers. The preoperative mean disc space height was 5.7±2.5 mm, the mean percentage of slip was 37.8%±6.2%. Two groupswere compared in the amount of blood loss, duration of hospitalization, back pain, radiating pain, fusion rate, the intervertebral disc space height, the postoperative degree of slip and the fusion rate. Results All patientswere followed up for 24-38 months. The mean follow-up was 29(24-36) months in group A and26(24-38) months in group B. There were no statistically significant differences infollow-up period, age,sex, the location of isthmic defects, smoking, the preoperative disc space height and the percentage of slip between two groups (Pgt;0.05).There were no statistically significant differences in the amount of blood loss, the duration of hospitalization, the fusion time between two groups(Pgt;0.05). But there were statistically significant differences in the back pain score, the radiating pain score and the fusion rate between two groups(Plt;0.05).Thepo stoperative disc space height and the degree of slip of the last follow-up were5.8±2.2 mm and 25.6%±7.2% in group A, 6.2±2.5 mm and 24.1%±7.4 % ingroupB, showing statistically significant difference (Plt;0.05). Conclusion The PLIF using autogenous bone grafting and cage fusion with pedicle screw fixations ismore beneficial to improving the fusion rate and preventing longterm instabilities than simple cage alone fusion with pedicle screw fixation in adult spondylolisthesis.

  • 微创腰椎椎间融合术的临床应用进展

    腰椎融合术是治疗腰椎退变性疾患、腰椎感染、创伤、肿瘤以及腰骶部畸形等疾病的传统术式,其开展历史已近百年。然而,由于传统腰椎融合术需对肌肉等软组织进行广泛剥离和长时间牵拉,致使术后慢性腰痛等并发症的发生率明显增加。近年来,伴随脊柱微创外科技术的飞速发展,结合传统腰椎融合术与微创技术的全新手术方式——微创腰椎椎间融合术应运而生。其优势在于能获得与传统腰椎融合术相同或相近手术疗效的前提下,尽可能减少对椎旁软组织的损伤,同时减少术中失血量,缩短术后住院及康复时间,降低术中、术后相关并发症的发生率等。因而近年来也受到了越来越多的脊柱工作者的青睐。现就该类技术的临床应用现状及进展作一综述。

  • Preliminary Application of One level Posterior Lumbar Interbody Fusion with Prospace and Facet Fusion Using Local Autograft

    To evaluate the safety and efficacy of one-level posterior lumbar interbody fusion(PLIF) combined with Prospace and facet fusion using local autograft. Methods Clinical and radiographic data of 76 patients treated by this technique was reviewed from May 2002 to December 2004. Of them, there were 52 males and 24 females, with an average age of 53.2 years (2381 years), including 60 cases of degenerative disc disease, 9 cases of failed back surgery syndrome and 3 cases of spondylolysis. The disese courses were 1.2-8.7 years (mean 3.6 years). The levels of PLIF were:L 2,3 in 2 cases, L 3,4 in 7, L 4,5 in 54, L 5/S 1 in 10, L 4/S 1 in 1 and L 5,6 in 2. After decompression,Prospace was inserted into interbody space bilaterally,and located in disc space 4 mm beyond the rear edge ofthe vertebral body. Local laminectomy autograft was packed both laterally into and between 2 implants. Then the remanent local autograft was placed over facet bed. Pedicle screws were used after insertion of Prospace. Clinical results wereevaluated by the JOA score. Disc height ratio and lumbar lordosis angles were measured on lateral radiographs. Fusion status was determined by evidence of bridge trabeculae across facet joint and interbody space on CT scan without mobility in lateral dynamic X-rays, and no radiolucent gap between Prospace and endplate. Paired t test was used for statistical analysis. Results Mean blood loss and operative time was 384 ml and 178 minutes, respectively. The average JOA score at final follow-up (26.1±2.7) was significantly improved when compared with that of preoperation (14.5±4.0, P<0.05), with a mean recovery rate of JOA score 81.1% (37.5%-100.0%). The fusion rate was 974%(74/76). Mean disc height ratio and the involved segmental lordosis angle were increased from preoperative 0.27± 0.07 and 5.8±2.2° to 0.33±0.06 and 11.3±2.0° respectively at the final followup, and the differences were significant (P<0.05). There were no devicerelated complications. Conclusion This surgical technique combined with Prospace interbody device is a safe and effective surgical option for patients with onelevel lumbar disorders when PLIF is warranted.
