  • CLINICAL STUDY OF VARIOUS GROWTH FACTORS ON THE IMPROVEMENT OF IMPAIRED HEALING ULCERS IN PATIENTS WITH DIABETIC DISEASE

    OBJECTIVE To investigate clinical effects and possible mechanisms of various growth factors on impaired healing ulcers of patients with diabetic disease. METHODS Seventy-eight patients were divided into three groups; saline control, epidermal growth factor(EGF) experimental group, and platelet-derived wound healing factor (PDWHF) experimental group. General healing conditions, wound closing index, healing rates and histological changes of the patient’s ulcer wound were observed during 1-8 weeks after treatment. RESULTS The wound closing index and healing rate of ulcers were significantly increased in the EGF and PDWHF experimental groups compared with the control group, while the angiogenesis, fibroblast hyperplasia, and collagen deposit were more obvious in EGF and PDWHF experimental groups than that of control group. The promoting effects on wound healing in PDWHF experimental group were better than in EGF group. CONCLUSION It suggests that local application of certain growth factor alone or various growth factors together is an effective method to improve the condition of impaired healing of diabetic ulcers.

  • Promotion of Rat Skin Healing by Using Natural Polymer Chitosan Derivatives

    We in the present study observed the effect of N-fructose modified chitosan quaternary ammonium derivativeson on rat skin wound healing through animal experiments. Forty rats were randomly divided into eight groups (5 in each group). Four groups among the all 8 groups were the experimental groups, while the other 4 groups were the control groups. Next to the skin along the back of the spine, 1.50 cm×2.00 cm×0.16 cm full-thickness skin was cut to make an excision wound model for every rat. Those in the experimental groups were treated with the N-fructose-modified chitosan quaternary ammonium derivatives ointment dressing the wound, while those in the control groups with sterile medical vaseline processing. We dressed the wounds twice a day to observe the wound healing of all rats in different groups. We then observed the wound healing and wound pathology after 3, 7, 10, 15 days respectively in different groups. Results showed significant differences of the time of wound healing, area of wound healing and volume of wound healing between the experimental groups and control groups (P<0.05). It can be well concluded that N-fructose-modified chitosan quaternary ammonium derivatives does not harm the skin, but could promote skin healing, so that they could be suitable skin repair materials and ideal raw materials for medical dressing.

  • Research progress in adipose tissue promoted wound healing

    ObjectiveTo summarize recent progress in adipose tissue acting as a more efficient and ideal therapy to facilitate wound repair and evaluate the therapeutic values of adipose tissue.MethodsThe related literature about adipose tissue for wound healing in recent years was reviewed and analyzed.ResultsEnormous studies focus on the capacity of adipose tissue to accelerate wound healing including cellular components, extracellular matrix, and paracrine signaling have been investigated.ConclusionAdipose tissue has generated great interest in recent years because of unique advantages such as abundant and accessible source, thriven potential to enhance the regeneration and repair of damaged tissue. However, there is still a need to explore the mechanism that adipose tissue regulates cellular function and tissue regeneration in order to facilitate clinical application of adipose tissue in wound healing.

  • THE EXPRESSION AND SIGNIFICANCE OF TRANSFORMING GROWTH FACTOR-β1 IN THE HEALING PROCESS OF BILE DUCT

    Objective To observe the expression and distribution of transforming growth factor-β1 (TGF-β1) in the healing process of bile duct and discuss its function and significance in the process of benign biliary stricture formation. Methods An injury to bile duct of dog was made and then repaired. The expression and distribution of TGF-β1 in the tissue at different time of the healing process were studied after operation with immunohistochemical SP staining. Results TGF-β1 staining was observed in the granulation tissue, fibroblasts and endothelial cells of blood vessels. High expression of TGF-β1 was observed in the healing process lasting for a long time. Conclusion The high expression of TGF-β1 is related closely with the fibroblast proliferating activity, extracellular matrix overdeposition and scar proliferation in the healing process of bile duct.

  • EXPERIMENTAL STUDY OF PROLIFERATION EFFECTS OF PLATELET DERIVED WOUND HEALING FACTOR ON TISSUE CELLS OF WOUND

    This experiment was designed to observe the proliferative effects of platelet derived wound healing factor (PDWHF) of different concentrations on fibroblasts from rat wounds and on epithelial from human wounds. Cultured fibroblasts from rat wound and epithelia from human wound were randomly divided into three groups. (1) In blank control, the cells were treated with basic medium (BM, contains 1640/0.5% FCS); (2) the positive control, the cells were treated with 1640/10% FCS and (3) in the PDWHF group, the cells were treated respectively with BM/1% PDWHF, BM/3% PDWHF, BM/5% PDWHF, BM/7% PDWHF, BM/10% PDWHF, BM/12% PDWHF, respectively. The Cells were collected after 48 hours culturing with BM or PDWHF, and the cell proliferation was measured by MTT method according to the OD values. The result showed that the PDWHF could remarkably enhance the proliferation of fibroblasts and epithelial cells when its concentration was between 1% and 7%, which was obviously higher than that of the blank control (P lt; 0.01). When the concentration of PDWHF reached 10%, its proliferative effect was not remarkable when compared with the blank control, When the concentration of PDWHF reached 12%, it showed inhibitory effect on fibroblasts and manifested no obvious inhibitory effect on epithelial cells. It was concluded that the PDWHF was a combination of a variety of growth factors. In a certain range of concentration, the PDWHF might effectively promote the proliferation of fibroblasts and epithelial cells. Howerve, when its concentration reached to relatively higher level, its effect was not remarkable any more, or even showed inhibitory effect on cell proliferation.

  • Study on Pressure Ulcer Wound Infection and Its Intervention Effect

    ObjectiveTo research on the types of pathogenic bacteria in wound infection and analyze the effectiveness of long-term use of nano-silver dressing in the treatment of pressure ulcers, in order to provide references for the management of pressure ulcer wound. MethodsFifty-five patients (60 wounds) with stage Ⅲ-Ⅳ pressure ulcer wound treated in all departments between September 2011 and August 2015 were chosen to be our study subjects. Under overall intervention, all the wounds were assessed by the same method, cleansed and debrided, after which nano-silver antimicrobial dressing was used to intervene until the wound healed or the end of 8 weeks. The wounds which were not healed were treated with wet dressing therapy until wound healing. The detection rate of pathogenic bacteria before intervention and 2, 4 and 8 weeks after intervention, change of pressure ulcer healing score and the rate of wound healing were observed. ResultsBefore the intervention, 12 kinds of pathogenic bacteria were detected, including mainly Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus, and coagulase negative Staphylococci. The detection rate of pathogenic bacteria was 92.73% (51/55). With the use of nano-silver dressing during different time periods, the detection rate of pathogenic bacteria and the total score of pressure ulcer were lowered by varying degrees (P<0.01). Four and 8 weeks after intervention, wound bed improved significantly and the detection rate of pathogenic bacteria decreased faster. The healing rate during the intervention period was 23.64% (13/55). ConclusionThe incidence of pressure ulcer wound infection is high. The use of nano-silver wound dressing can effectively remove pathogenic bacteria and promote wound healing.

  • EXPERIMENTAL STUDY OF EXOGENOUS HIGH MOLECULAR WEIGHT NERVE GROWTH FACTOR MIXED WITH BLETILIA STRIATA GELATIN IN PROMOTING WOUND HEALING

    Abstract To observe the effect of exogenous high molecular weight nerve growth factor (HMW-NGF) mixed with bletilia striata gelatin (BSG) in the promotion of healing, the experiment was performed as follow: (1) In serumfree medium, the normal saline, BSG, HMWNGF, and BSG+HMW-NGF were added separately, and then, the chick embryo root ganglions (DRGs) were cultivated in the above prepared media and the axonal growth was observed. (2) 40 SD rats were divided into 4 groups. A wound of 2cm×2cm was made on the back of every rat. No treatment was given in group one. In other groups, BSG, HMW-NGF, andBSG+HMW-NGF were given separately to the wounds once daily. After 3 and 10 days, the wound area of every rat was measured, cells in the wounds were observed under light microscope and were calculated, and the time of healing was recorded. The results showed that BSG, HMW-NGF, especially BSG+HMW-NGF could promote wound healing.

  • STUDY OF REGULATORY CONTROL OF MICROENVIRONMENT OF WOUND ON THE GROWTH OF TISSUE REPAIR CELL

    Abstract To study the regulation of growth and proliferation of tissue-repair cell from wound microenvironment, the effects of wound fluid (WF) on the growth and proliferation of wound fibroblast were studied in vitro. Thirty rats were divided into 6 groups. On the back of every rat, an incision of 0.5~1.0cm was performed a subcutaneous sac was made by blunt dissection. A piece of sponge was put in, and the wound was sutured. After 1,3,7,9,11,15 days, one group of the rats were sacrificed respectively, and WF was collected from the sponge. Two kinds of medium were made with each WF: 1640+1%FCS+10%WF and1640+10%FCS+10%WF. After 48 hours incubation with newly prepared wound fibroblasts, the growth of the cells was observed. It was shown that (1) Under 1%FCS, WFfrom1,3,7 days stimulated cell proliferation, and WF from 9,11,15 days caused cell death. (2) Under 10%FCS, WF from 9,11,15 days inhibited cell growth. It was suggested that the wound microenvironment stimulated the fibroblasts to proliferate for one week after injury, and beyond that further growth seemed to be arrested, and that there might be some growth inhibitory factors present in the microenvironmentduring the late stage of wound healing.

  • THE EFFECT OF ULTRAPURE HYALURONIC ACID WITH DIFFERENT MOLECULAR WEIGHTS ON THE HEALING OF PORCINE FULL THICKNESS SKIN WOUND

    Hyaluronic acid (HA) is an extracellular matrix having a high molecular weight. This study is to investigate (1) whether the ultrapure HA had effect on the healing of full thickness skin wounds in animals, and (2) the relation between the effect of HA and the size of the molecular weight of HA. The ultrapure HA with different molecular weights and HA with intermediate molecular weight were applied topically to the wounds from removal of full thickness of skin and topical application of normal saline was used as control. At 21st, day after the topical application, the tensile strength was reduced in wounds of groups of HA with high and intermediate molecular weights, but not in the group of low molecular weight. Using laser-Doppler scanning of the dynamic changes of the wound perfusion, in the 3rd day the blood supply to the wound was reduced in HA groups of high and intermediate molecular weights, but at the 7th day, the blood supply of all wounds in the two groups was increased. The study revealed that HA with different molecular weights had different actions on the contraction of wounds, the angiogenesis of the granulation tissues and the healing strength of the wounds. It was indicated that hyaluronan could be used for wound treatment or as a carrier, but its accurate composition must be carefully noted.
